Objective scratch monitor evaluation of the effect of an antihistamine on nocturnal scratching in atopic dermatitis.

We previously reported a simple and easy-to-use device, Scratch Monitor for evaluation of nocturnal scratching. In the present study, the effect of an antihistamine (azelastine hydrochloride) on atopic dermatitis was investigated using this device. In 40 patients with atopic dermatitis, nocturnal scratching was measured by the monitor, when taking the drug and while off medication. Neither the 'sleeping time' nor the 'pre-asleep time' differed significantly, but there was a significant difference in 'scratch rate', 'minute scratch records', 'hourly awake number' and 'awake rate', which indicat

IgE-sensitization to cellular and culture filtrates of fungal extracts in patients with atopic dermatitis.

BACKGROUND: Patients with atopic dermatitis may experience exacerbations of eczema triggered by various inflammatory stimuli. One mechanism may be IgE-mediated reactions to dermatophytes since these patients are more likely to acquire skin infections with dermatophytes and may become sensitized. OBJECTIVE: This study investigates IgE-sensitization to fungi in patients with atopic dermatitis and compares the biologic activity of culture filtrates and cellular fungal extracts. The following allergen extracts were provided as culture filtrates and cellular extracts: Candida albicans, Fusarium mo

Ultrasound biomicroscopic view of detachment of the ciliary epithelium in retinal detachment with atopic dermatitis.

PURPOSE: The authors evaluated the effectiveness of ultrasound biomicroscopy to determine the condition of the ciliary body during perioperative examinations of patients with atopic dermatitis and retinal detachment. METHODS: The authors compared two groups of patients with atopic dermatitis and retinal detachment. Parameters included patient age, gender, eye, cataract, type and location of breaks, macular involvement, detachment of the ciliary epithelium, and preoperative and postoperative best-corrected visual acuities. Group 1 included six patients (nine eyes) who were examined before surg

ICAM-1 expression on keratinocytes in mechanically-injured skin of a patient with atopic dermatitis.

The expression of intercellular adhesion molecule-1 (ICAM-1) on keratinocytes was studied in skin specimens of patch- or scratch-test using Dermatophagoides farinae (DF) antigen in atopic dermatitis (AD). Positive patch test reaction showed exocytosis of lymphocytes and focal expression of ICAM-1 on keratinocytes. Similar lymphocyte infiltration and keratinocyte expression of ICAM-1 were observed in tape-stripped skin which was subjected as the control. In addition, focal ICAM-1 expression on keratinocytes was also observed without exocytosis of lymphocytes. By scratch-test with DF antigen, IC

Immunoglobulins in patients with atopic dermatitis due to mites infestation in Qualyobia Governorate, Egypt.

Atopic dermatitis (A.D.) is a hereditary and distinct form of eczema which may be associated with other atopic manifestations. The aetiopothogenesis of A.D. is unknown and it is widely accepted that both intrinsic and extrinsic factors are involved. House dust mites (HDM) are of great medical importance in causing allergic manifestations in human being. In the present work, a total of 30 A.D. patients with different ages of both sexes were examined for the immunoglobulins level as compared to control group. The IgG, IgM, IgA & IgE were higher in the patients than in the controls. These differe

Patch test reactions to the acetone-soluble fraction of human dander in atopic dermatitis.

An acetone-soluble fraction of human dander provoked positive patch test reactions in 45 (38%) of 120 patients with atopic dermatitis. The positive patch tests were rare in normal controls and clinical controls. The positive patch test reactions histologically showed spongiosis and mononuclear cell migration in the epidermis, edema and mononuclear cell infiltrate in the upper dermis. Scratch tests with the acetone-soluble human dander fraction were positive in only 2 (2%) of the 120 patients examined. It was suggested that in a considerable number of patients with atopic dermatitis, an acetone

Photoreceptor outer segments in the aqueous humor of patients with atopic dermatitis and retinal detachment.

We examined the aqueous humor by electron microscopy of four patients with atopic dermatitis and retinal detachment. All the patients had a moderate number of cells in the aqueous humor. Cataract impaired fundus visualization except for one patient with aphakia. A flat retinal detachment involved a wide area detected by echography. The intraocular pressure in the eyes with retinal detachment demonstrated a marked diurnal fluctuation compared with that of the fellow eye. The aqueous cells consisted predominantly of photoreceptor outer segments. This finding does not support the theory that aque

Immunophenotyping of the cutaneous infiltrate and of the mononuclear cells in the peripheral blood in patients with atopic dermatitis.

Fourteen adult patients with chronic atopic dermatitis and active skin lesions had a skin biopsy and venous blood sample taken on the same day. Absolute numbers of circulating lymphocytes were normal in all patients. Fluorescence-activated cell sorter (FACS) analysis revealed normal numbers of total T lymphocytes and T-helper and T-suppressor subsets (helper:suppressor ratio, 2:1) in the atopic patients' peripheral blood, but an increase in circulating B lymphocytes and in HLA-D-related antigen-bearing cells. The skin biopsy showed a dermal infiltrate of predominantly T-helper lymphocytes (hel

Targeted therapies in atopic dermatitis: toward endotype-driven precision medicine and disease modification.

Atopic dermatitis (AD) is a chronic, relapsing inflammatory skin disease driven by a combination of genetic predisposition, epidermal barrier dysfunction, immune dysregulation, aberrant neuroinflammatory signaling, and environmental factors. Genetic defects in barrier-related molecules, particularly loss-of-function mutations in filaggrin (FLG), represent key heritable determinants of impaired epidermal integrity and enhanced allergen penetration. In recent years, advances in the understanding of its molecular pathogenesis have progressively shifted the therapeutic paradigm from conventional b

Initiating Biologics in Atopic Dermatitis: Global Practices and Recommendations from the AMAZE Project.

INTRODUCTION: Biologics have expanded treatment options for moderate-to-severe atopic dermatitis (AD), but practical guidance on how to initiate and manage these therapies in routine care remains limited. The advancing management of atopic dermatitis and eczema (AMAZE ) project examined global clinical practices and developed expert-informed recommendations to support biologic initiation and long-term management. METHODS: An online survey of 150 dermatologists from North America, Europe, and Japan assessed real-world approaches to biologic management. Findings were interpreted by an internati

Oral Chinese patent medicine alleviates atopic dermatitis by regulating ALOX15-derived oxylipins disorder.

BACKGROUND: Atopic dermatitis (AD) is a chronic inflammatory skin disease often exacerbated by psychological stress, yet the underlying mechanisms remain unclear. Wushe Zhiyang Pills (WZP), an oral Chinese patent medicine, is clinically used for dermatological conditions, but its efficacy and mechanisms in stress-aggravated AD have not been defined. PURPOSE: This study aims to evaluate the protective effects of WZP against AD under stress conditions, elucidate its molecular mechanisms, and identify its bioactive compounds. METHODS: A murine AD model was established using repeated DNCB applic

Dupilumab versus tralokinumab in atopic dermatitis: A propensity score adjusted comparison from BioDay.

BACKGROUND: Dupilumab and tralokinumab for atopic dermatitis (AD) target the type 2 axis through different mechanisms of action, which may lead to variation in effectiveness and safety. Head-to-head trials, however, are lacking. OBJECTIVES: To compare the real-world effectiveness and safety of dupilumab and tralokinumab in AD. METHODS: This prospective cohort study enrolled biologic-/Janus kinase inhibitor-naïve AD patients (≥12 years) from the BioDay registry who initiated dupilumab or tralokinumab between November 2021 and September 2024. Visits were scheduled at baseline, 4 weeks and ever

Patient Perspectives on Dupilumab Therapy for Atopic Dermatitis: Experiences, Expectations and Treatment Impact.

BACKGROUND: Atopic dermatitis (AD) is a chronic inflammatory skin disease characterised by epidermal barrier dysfunction and type 2 immune dysregulation. Dupilumab was the first biologic approved for severe AD and has transformed management of moderate-to-severe disease. Although clinical trials and international real-world studies have demonstrated sustained efficacy and acceptable tolerability, Australian data describing patient-reported experience with dupilumab remain limited. METHODS: We conducted a cross-sectional survey study of adults receiving dupilumab for moderate-to-severe AD at t

Achieving Targeted Combined Endpoints in Patients With Atopic Dermatitis: A Real-World Study.

Real-world evidence on the treat-to-target strategy in atopic dermatitis (AD) remains limited. To evaluate targeted combined endpoints in AD, we conducted a cross-sectional study that included 100 adults with AD receiving treatment ≥ 6 months. Targeted endpoints included Patient Global Assessment (PGA) ≤ 2, Eczema Area and Severity Index (EASI) ≤ 7, visual analog scale (VAS) of pruritus ≤ 4, Patient Oriented Eczema Measure (POEM) ≤ 7, and Dermatology Life Quality Index (DLQI) ≤ 5. Logistic regression and receiver operating characteristic (ROC) curve analysis were performed to identify factors

Atopic Dermatitis Across the Lifespan: Understanding, Measuring and Minimizing Cumulative Life Course Impairment.

Atopic dermatitis (AD) presents a multidimensional burden, often beginning in infancy and accumulating over time. Beyond itch and pain, AD is associated with significant losses of health-related quality of life resulting from sleep disturbance, stigmatization, other atopic diseases and nonatopic comorbidities, among others. These associated conditions affect patients' physical and mental health, development, social interaction and productivity, resulting in what has been termed cumulative life course impairment (CLCI). To alter the trajectory of AD and improve patient outcomes, it is important
