Repeated cerebral ischemia caused by extracranial carotid artery dolichoectasia.

We report the case of a 67-year-old man with repeating cerebral embolism caused by a dolichoectatic right common carotid artery. The patient had a history of hypertension, hypercholesterolemia, cigarette smoking, and a postoperative abdominal aortic aneurysm. He presented with a sudden onset of weakness of the left arm and leg. Magnetic resonance imaging revealed old and fresh infarction in the right cerebral hemisphere. Carotid duplex ultrasonography showed a dolichoectatic right common carotid artery with a maximum diameter of 39 mm with thick plaque and strong spontaneous echo contrast. The

Mesencephalic enlarged Virchow-Robin spaces in a 6-year-old boy: a case-based update.

BACKGROUND: Perivascular spaces or Virchow-Robin spaces are normal anatomical structures within the brain, typically less than 5 mm in diameter. Rarely, they can reach much larger sizes and adopt bizarre single or multi-cystic configurations, as was the case in the patient that we report on. When there is such markedly gross dilatation, the phenomenon is known as Giant or tumefactive perivascular space enlargement. CASE REPORT: We report a 6-year-old boy with a multi-cystic mesencephalic lesions presenting with obstructive hydrocephalus secondary to obstruction of the aqueduct of Sylvius due

Comparisons of mechanical versus phase-array intracardiac echocardiography-assisted transseptal puncture in patients with dilated left atrium undergoing percutaneous transvenous mitral commissurotomy.

BACKGROUND: Percutaneous transvenous mitral commissurotomy (PTMC) is an effective treatment for mitral stenosis, but transseptal puncture carries a certain risk of complications. Our previous report has shown the effectiveness of phase-array intracardiac echocardiography (ICE)-guided transseptal puncture in patients with dilated left atrium undergoing PTMC. However, there are few reports comparing the new-generation mechanical versus phase-array ICE-guided transseptal puncture in PTMC. METHODS: Between March 2007 and March 2008, 6 consecutive patients with symptomatic mitral stenosis with dil

Ureteral obstruction by the vas deferens after urostomy.

A male patient with spina bifida and paraplegia, born in 1968, underwent urostomy in 1973. In 1999, he developed urine infections. Intravenous urography showed bilateral hydronephrosis and hydroureter. This patient continued to get recurrent urine infections. In 2009, computed tomography of the abdomen revealed dilatation of the ureters, but the ureters reverted to normal calibre as they passed forward through the anterior abdominal wall. The vas deferens on either side was crossing and kinking the ureter. Magnetic resonance imaging of the abdomen confirmed that the level of obstruction in bot

[Postinfarction cardiac remodeling--clinical consequences].

Despite the considerable progress in the setting of invasive and pharmacological treatment still in about 20-30% patients with myocardial infarction in long-term observation an unfavourable post-infarction remodeling along with development of LV dilatation is to be seen. The heart forms the structural and functional unity, thus morphological changes due to cardiac post-infarction remodeling process are accompanied by systolic and diastolic myocardial dysfunction, which in consequence may lead to appearance of heart failure. In recent years our knowledge regarding causative factors of post-infa

Uveal melanoma presenting as cataract and staphyloma.

Blind eyes can harbor a choroidal melanoma. We report a case of uveal melanoma presenting as staphyloma and complicated cataract in a 45-year-old female. The left eye was blind for six months. She underwent comprehensive ocular examination but fundus examination was precluded due to total cataract. The ultrasound of the eye showed a large mass filling the superior, nasal and inferonasal vitreous cavity with high surface reflectivity and low to moderate internal reflectivity. Magnetic resonance imaging (MRI) confirmed the diagnosis of choroidal melanoma. The patient underwent extended enucleati

[Frontal pneumosinus dilatans associated with multiple cerebral meningioma].

UNLABELLED: Pneumosinus dilatans is rare. It is defined as an expansion of one of paranasal sinuses. We discuss the etiopathogenic hypothesis of pneumosinus dilatans, its epidemiologic and clinical characteristics, and its therapeutic approaches. OBSERVATION: A man, 50 years old, presented frontal pneumosinus dilatans associated with multiple cerebral meningiomas, one of which was appended in the nasal cavity revealed by frontal deformation and nasal obstruction. CT and MRI confirmed the diagnosis. The patient was operated in the neurosurgery department, via the bicoronal approach, allowing t

Mechanisms of varicose vein formation: valve dysfunction and wall dilation.

Varicose veins are a common venous disease of the lower extremity. Although the mechanisms and determinants in the development of varicosities are not clearly defined, recent clinical studies and basic science research have cast some light on possible mechanisms of the disease. In varicose veins, there are reflux and incompetent valves as well as vein wall dilation. Primary structural changes in the valves may make them 'leaky', with progressive reflux causing secondary changes in the vein wall. Alternatively, or concurrently, the valves may become incompetent secondary to structural abnormali

Combined Scimitar syndrome and interruption of the inferior vena cava causing mega-azygous and hemiazygous veins.

Computed tomography angiography detected scimitar syndrome with venous drainage to the inferior vena cava caudal to an interruption of the inferior vena cava in a 48-year-old woman. The hepatic veins drained to the supra-hepatic portion of the inferior vena cava cephalic to the interruption, which was a short area of atresia. Azygous and hemiazygous veins were massively enlarged since they drained the right lung, abdomen viscera exclusive of the liver and lower extremities. Inferior vena cava interruption by a short, focal atresia has not previously been described. The inferior vena cava immed

Management of infants with idiopathic dilatation of the right atrium and atrial tachycardia.

Idiopathic dilatation of the right atrium (IDRA) is a rare anomaly defined as isolated enlargement of the right atrium in the absence of other cardiac lesions known to cause right atrial dilatation. IDRA is a congenital anomaly with unknown pathogenesis and highly variable clinical presentation. Optimal management of severe IDRA is controversial and individualized. Literature reports of long-term follow-up have been limited. We describe a child with IDRA with rapid atrial tachycardia (AT) refractory to both medical and surgical management, and we provide long-term follow-up on our two previous

Aortic valve-sparing reimplantation for dilatation of the ascending aorta and aortic regurgitation late after repair of congenital heart disease.

OBJECTIVE: Aortic regurgitation resulting from progressive dilatation of the aortic root late after surgical correction of congenital heart disease is now widely recognized. We reviewed our experience with aortic valve-sparing operations to access the effectiveness of this approach. METHODS: Three patients, aged 28, 31, and 32 years, had progressive dilatation of the ascending aorta and aortic regurgitation 25, 27, and 28 years, respectively, after surgical repair of complex congenital heart disease. At the time of reoperation, the maximum diameter of the aortic root ranged between 50 and 55

Multiscale model for pulmonary oxygen uptake and its application to quantify hypoxemia in hepatopulmonary syndrome.

This paper presents a novel multiscale methodology for quantitative analysis of pulmonary gas exchange. The process of oxygen uptake in the lungs is a complex multiscale process, characterized by multiple time and length scales which are coupled nonlinearly through the processes of diffusion, convection and reaction, and the overall oxygen uptake is significantly influenced by the transport and reaction rate processes at the small-scales. Based on the separation of length scales, we characterize these disparate scales by three representative ones, namely micro (red blood cell), meso (capillary

A case of severe dilatation of the parotid duct due to fibrinous sialodochitis.

A 50-year-old woman presented with a 10-year history of swelling of the left cheek during and after meals. She regularly pushed the cheek hard to decrease the swelling by excreting the saliva. She did not have pain or fever. She also complained of bronchial asthma and allergic rhinitis. Magnetic resonance imaging (MRI) revealed bead-like dilatation (3x4 cm) of the left parotid duct with hypointensity on T1-weighted MRI and hyperintensity on T2-weighted MRI. Technetium 99m-pertechnetate (Tc-99m) scintigraphy demonstrated hypofunction of the left parotid gland and retention of radioactive substr

Alterations of the thoracic spine in Marfan's syndrome.

OBJECTIVE: The purpose of this study was to determine if the thoracic vertebral elements are altered in patients with Marfan's syndrome. MATERIALS AND METHODS: Thirty patients underwent helical CT of the thorax because of suspected thoracic aortic dilatation and acute dissection. Thirteen had Marfan's syndrome and 17 did not. Two reviewers, unaware of the final diagnosis, evaluated the images by consensus for laminar thickness, foraminal width, dural sac ratios, and vertebral scalloping for T2-T12. RESULTS: At T9-T12, dural sac ratios at the midcorpus level (p = 0.031) and foraminal width (p

Post-laser in-situ keratomileusis ectasia: current understanding and future directions.

PURPOSE OF REVIEW: The aim of this article is to review the causes, risk factors, management, and future research directions for corneal ectasia after laser in situ keratomileusis. RECENT FINDINGS: Complex corneal biomechanical processes influence the integrity of the normal and postoperative cornea, and developing an understanding of these processes facilitates recognition of risk factors for ectasia after laser in-situ keratomileusis. Currently identified risk factors include keratoconus, high myopia, low residual stromal bed thickness from excessive ablation or thick flap creation, and def
