Does Integrated Fixation Provide Benefit in the Reconstruction of Posttraumatic Tibial Bone Defects?

BACKGROUND: Limb salvage in the presence of posttraumatic tibial bone loss can be accomplished using the traditional Ilizarov method of distraction osteogenesis with circular external fixation. Internal fixation placed at the beginning of the consolidation phase, so-called integrated fixation, may allow for earlier removal of the external fixator but introduces concerns about cross-contamination from the additional open procedure and maintenance of bone regenerate stability. QUESTIONS/PURPOSES: Among patients deemed eligible for integrated fixation, we sought to determine: (1) Does integrated

Elastic intramedullary nailing as a complement to Ilizarov's method for forearm lengthening: a comparative pediatric prospective study.

PURPOSE: Most of the techniques for forearm lengthening involve external fixation to achieve stability and provide progressive distraction. We introduce the use of elastic stable intramedullar nailing (ESIN) in combination with external circular assembly for the procedure. The purpose of this prospective study was to compare Ilizarov's classical technique with this combined technique. METHODS: Fifty-seven patients, with forearm length discrepancies or deformities either congenital or acquired, were prospectively followed-up. Patients were divided in two groups: 35 had only external fixation,

Outcomes of surgically treated posterior pelvic fractures in an Australian population: A multicenter study.

BACKGROUND: Unstable posterior pelvic-ring fractures are rare and difficult to manage. There are many injury patterns, they are associated w … nfection rate of 15.6 % vs 4.6 %. Length-of-stay was increased in patients managed with temporising external fixators, who had bilateral injuries, where infection occurred or were restricted to non-weight bearing postoperatively. CONCLUSIONS: This study highlight bilateral posterior-pelvic ring injuries as a greater management challenge than unilateral injuries. I …

Management of extremity and pelvic fractures in earthquake: our observations and recommendations.

BACKGROUND: Earthquakes frequently cause injuries to the musculoskeletal system. Studies conducted after earthquakes often report on injured … n in the first few days, and Kirschner wires were the first treatment applied at a mean of 74 h and external fixators at a mean of 94.3 h. CONCLUSION: Injuries from falling objects from roofs and jumping from heights are important in earthquakes. In addition to lower extremity fractures, pelvic fractures are common in earthquake disasters. Most pelvic fractures ar …

Distal radius fractures-Regional variation in treatment regimens.

OBJECTIVES: After recent technical innovations of fracture surgery implants, treatment traditions are changing for distal radius fractures, the most common orthopaedic injury. The aim of this study was to determine if the choice of surgical method for treatment of distal radius fractures differ between healthcare regions in Sweden. METHOD: The study was based on all (n = 22 378) adult patients who were registered with a surgical procedure due to a distal radius fracture during 2010-2013 in Sweden. Consecutive data was collected from the Swedish National Patient Registry. RESULTS: The proport

Staged treatment and internal fixation of floating ankle: A case report and literature review.

RATIONALE: Floating ankle is a rare traumatic condition characterized by a combination of tibial and ipsilateral foot fractures, with the ankle remaining intact. It is usually caused by high-energy trauma and also presents with serious soft tissue damage. Its treatment is mainly restricted to external fixation, which results in poor outcomes. We present a patient with a floating ankle who underwent staged treatment and achieved full internal fixation, subsequently returning to normal activity. PATIENT CONCERNS: A 26 year- old man had an accident with an reel machine and sustained an open frac

Limits of internal fixation in long-bone fracture.

Alternatives to internal fixation of long-bone fracture comprise, depending on location, external fixation or joint replacement. Limitations comprise risk of infection and functional outcome quality, which vary according to technique. The present study examines these limitations, based on comparative or large-scale studies from which certain significant results emerge. Four main questions are dealt with: (1) the present role of locking plates; (2) conditions for intramedullary nailing in Gustilo grade IIIb open fracture; (3) the limitations of conversion from external fixation to intramedullar

Ilizarov Ring Fixator in the Lower Limb for 2000 Days: A Case Report.

Ring external fixators developed by Gavriil Abramovich Ilizarov from Russia are used to treat the difficult cases of infected non-union, shortening of limbs by bone lengthening, and deformity correction in joints and bones in isolation or in combination. Fixation of the involved bone with the ring is commonly achieved using four rings, each having two Ilizarov wires that are passed along the superior surface of the ring, then across the bone, and exiting out on the opposite side touching the surface of the ring. The case report herein reports a patient who kept the ring fixator on his thigh wi…

Groin flap immobilization by axillary brachial plexus block anesthesia.

Pedicled groin flaps are still widely used as soft tissue coverage in hand surgery. Various methods have been described for immobilization of flaps, such as Ace wraps, plaster, and external fixators. The hands of 5 females and 12 males, totaling 17 patients, with severe injury were reconstructed with groin flaps between 2001 and 2005. Patients were operated on under a combination of axillary block and general anesthesia. Motor block effect of axillary anesthesia was used for immobilization of the upper extremities at groin flap coverage. A combination of axillary brachial plexus block plus gen…

The segmental tibial fracture.

A series of 31 segmental tibial fractures is reported. Eighty-four percent of the patients sustained multiple trauma and 80% of these were open fractures. The series includes treatment with 20 external fixators, seven unreamed intramedullary nails, two casts, and two amputations. Complications included a 48% incidence of elevated compartment pressures necessitating fasciotomy. Despite rapid early decompression, there was a 19% incidence of residual motor and sensory deficit. Other complications observed were a 35% incidence of wound infection, nonunion, and malunion. Eighty-one percent of dela…

The treatment of tibial fractures by elastic nailing and functional plaster cast.

From June 1984 to December 1985, 32 fractures of the tibia were treated by elastic nailing by the Ender method with functional plaster casts according to the Sarmiento method. They represented 28% of all tibial fractures treated during the same period by the traditional conservative method or by external fixators. Elastic nailing was reserved for unstable fractures produced by high velocity trauma. Union occurred in all cases after an average of 13 weeks. Complications were insignificant: shortening by 1.2 cm (1 case); recurvatum (1 case); varus (1 case); valgus (1 case), both less than 8 degr…

Open reduction and internal fixation of tibial pilon fractures.

Although it is evident that the fracture of the tibial plafond is a complex, often debilitating injury, its management is not clear. These injuries generally fall into one of two categories. The low-energy, rotational type of fracture has been shown to have excellent clinical and functional results with open reduction and internal fixation. The high-energy, compression type of fracture has had uniformly moderate results and historically high complication rates. Some authors think that bridging external fixation with or without limited internal fixation should be employed in high-energy fractur

Treatment of femoral defective osteomyelitis with minimal invasive plates.

Although recent treatment modalities reduced chronic osteomyelitis, it is still a challenging problem for both orthopaedic surgeons and patients. Especially treatment of femoral osteomyelitis with defective nonunion is reasonably difficult. Most accepted treatment option is external fixation. But the duration needed for external fixation may be too long for patient compliance. To reduce external fixation duration several methods were described. In this study we described a method for reconstruction of infected femoral defective nonunions using minimal invasive plates.

Correction of lower limb deformities with fixator assisted nailing.

INTRODUCTION: Deformities of the lower extremities can be congenital or acquired. Various surgical treatments have been employed for such disorders including osteotomy followed by either external fixation, internal fixation or external fixator assisted internal fixation. The aim of surgery is correction of deformity and restoration of mechanical axis and joint line. External fixator assisted internal fixation with intramedullary (IM) nail insertion is considered the gold standard, however, it is less commonly practiced as expertise required are usually not available at most centers. This study

Tissue deformation controlling fracture healing.

To achieve optimal flexibility in biological internal fracture fixation two questions require clarification: which biomechanical parameter controls healing and what are the boundary conditions thereof? Fracture movement interacts with callus and local stress and strain are influencing the reaction of the tissue cells. A linear gradient of strain was created inside a sheep tibia osteotomy using an active external fixator. The effect of different amounts of strain applied at 10 stimulation cycles/day on the occurrence of callus and on enabling osseous connection of the fragments was evaluated us
