Object Recognition via Evoked Sensory Feedback during Control of a Prosthetic Hand.

Haptic and proprioceptive feedback is critical for sensorimotor integration when we use our hand to perform daily tasks. Here, we evaluated how externally evoked haptic and proprioceptive feedback and myoelectric control strategies affected the recognition of object properties when participants controlled a prosthetic hand. Fingertip haptic sensation was elicited using a transcutaneous nerve stimulation grid to encode the prosthetic's fingertip forces. An array of tactors elicited patterned vibratory stimuli to encode tactile-proprioceptive kinematic information of the prosthetic finger joint.…

Recovery of an injured corticoreticular pathway via transcallosal fibers in a patient with intracerebral hemorrhage.

BACKGROUND: Several studies have reported on injury of the corticoreticular pathway (CRP), however, little is known about the mechanism for recovery of an injured CRP. We report on a patient with intracerebral hemorrhage (ICH) who showed recovery of an injured CRP via transcallosal fibers, which was demonstrated by diffusion tensor tractography (DTT). CASE PRESENTATION: A 67-year-old man presented with complete paralysis (Medical Research Council: 0/5) of the left extremities at the onset of a right putaminal hemorrhage. At six weeks after onset, he presented with more severe weakness of prox

Surface replacing arthroplasty of a post-traumatic stiff and deviated proximal interphalangeal joint in an adolescent.

This case report describes the 4-year outcomes of proximal interphalangeal joint arthroplasty in a 14-year-old girl with a stiff joint after trauma. At follow-up, active range of motion was 35°, she was pain-free and satisfied with the outcome. Implant arthroplasty seems to be a valuable option for young patients with persistent post-traumatic stiff and deviated PIP joints to-at least temporarily-increase quality of life.

Outcome of the MatOrtho arthroplasty for PIP osteoarthritis with a minimum follow-up of two years.

The aim of this study is to report the early results of the MatOrtho arthroplasty, a newer generation resurfacing implant of the proximal interphalangeal joint. We performed a prospective cohort review of all MatOrtho arthroplasties implanted between 12/2013 and 05/2018 by a single surgeon at a single institution because of primary osteoarthritis, with a minimum follow-up of two years. Patient demographics, diagnosis, implant revision and other surgical interventions were recorded. Subjective and objective outcomes were evaluated, including range of motion, Patient Reported Outcome Measures an

Articular Surgery of the Ischemic Hand in Systemic Scleroderma: A Vascular Basis for Arthrodesis and Arthroplasty.

Hallmark deformities of systemic scleroderma are early onset and progressively disabling flexion contractures of the proximal interphalangeal (PIP) joints often in conjunction with extension or, less frequently, flexion contractures of the metacarpophalangeal (MCP) joints. Although surgical correction is generally recommended, a prevailing reluctance for operative treatment exists owing to the inherent ischemia of the disease with its potentially compromised healing capacity. Nonetheless, with recognition and preservation of the tenuous but well-defined and constant periarticular vascular netw

Two-digit grasp as pointing movements by the opposing digits: evidence based on the uncontrolled manifold analysis.

We used the concept of performance-stabilizing synergies to test predictions of two hypotheses on the organization of reach-to-grasp movements, one assuming separate control of the transport and aperture, and the other assuming that such actions represent pointing movements by the opposing digits. Healthy participants performed series of reach-to-grasp movements to a cylindrical object with and without targets on the object surface with the dominant and non-dominant hands. Joint motion was captured by a motion analysis system. Inter-trial variance in the joint configuration space was analyzed

Arthroscopic Management of Metacarpophalangeal Joint Synovial Chondromatosis.

Synovial chondromatosis is a rare condition where cartilaginous nodules form within the synovium, which can detach and cause loose bodies within the joint. While it typically affects larger joints, involvement of the metacarpophalangeal (MCP) joints is rarely described. We report the case of a 42-year-old female patient who had a slow-growing mass in her right third MCP (MCP III) joint, associated with limited finger motion, swelling, and pain. The diagnosis was confirmed using imaging studies. The patient underwent arthroscopic excision of multiple tumors and synovectomy via dorsal radial and

Upper extremity injuries in the paediatric athlete.

Injuries to the upper extremity in paediatric and adolescent athletes are increasingly being seen with expanded participation and higher competitive levels of youth sports. Injury patterns are unique to the growing musculoskeletal system and specific to the demands of the involved sport. Shoulder injuries include sternoclavicular joint injury, clavicle fracture, acromioclavicular joint injury, osteolysis of the distal clavicle, little league shoulder, proximal humerus fracture, glenohumeral instability and rotator cuff injury. Elbow injuries include supracondylar fracture, lateral condyle frac

An anomalous accessory flexor digiti minimi profundus muscle: a case study.

A well-developed anomalous muscle within the forearm flexor compartment was discovered during a routine cadaveric dissection. The identified muscle originated from the intercompartmental septum on the medial side of the forearm just proximal to the wrist joint and inserted on the proximal phalanx of the fifth digit. When stressed, the tendon of the muscle produced flexion of the metacarpophalangeal joint of the fifth digit. Contiguous muscles, including the flexor digitorum profundus, displayed normal morphology. The muscle appeared to be an accessory belly of flexor digitorum profundus. The c

Percutaneous, intramedullary fracture reduction and extension block pinning for dorsal proximal interphalangeal fracture-dislocations.

PURPOSE: A percutaneous, intramedullary fracture reduction technique for treatment of unstable dorsal fracture-dislocation of the proximal interphalangeal (PIP) joint is described and the clinical and radiological results are reported. METHODS: We conducted a retrospective clinical follow-up of 16 patients with 18 dorsal fracture-dislocations of the PIP joint. After closed joint reduction, we used an extension block K-wire to maintain the joint reduction. Impacted volar articular fragments were reduced percutaneously under fluoroscopy control using a pre-bent K-wire inserted through the intra

[Surgical treatment of freshly closed articular fractures of proximal interphalangeal joints].

Eleven fractures of the base of P2 and 8 fractures of the head of P1 were treated surgically. We review the results after a mean follow-up of 4 years 5 months. Due to their complexity is no real consensus on the treatment of P2 and various treatments were used. This series demonstrated two points: first the was no correlation between the degree of patient satisfaction and the section of digital immobilization; second, residual subluxation and/or arthrosic remodeling was frequent but was not incompatible with a good objective and subjective outcome.

Orthopaedic management in four cases of mucolipidosis type III.

Four patients with mucolipidosis type III, three of them brothers, were seen initially in the first two decades of life. Their main symptoms were carpal tunnel syndrome, trigger fingers and generalized joint stiffness. Radiographs showed spinal deformities and hip dysplasia, but these were not causing pain. Carpal tunnel syndrome was treated surgically but joint stiffness and hip and knee contractures were managed by physiotherapy. Up to the age of 24 none of these patients has had pelvic osteotomy for hip dysplasia; this operation, not yet reported in mucolipidosis type III, may eventually be

Multicentric reticulohistiocytosis.

We describe a 42-year-old man with a five-year history of arthritis mutilans-like destructive joint changes and with a one-year history of nodules on the fingers, ears, oral mucosa, pharynx, larynx, vocal cords, some being ulcerated and haemorrhagic. He was diagnosed as having rheumatoid arthritis; however, biopsies from the nodules on the oral mucosa and ear revealed multicentric reticulohistiocytosis. The large nodule over the olecranon process, simulating a rheumatoid nodule but diagnosed as multicentric reticulohistiocytosis with biopsy; ulcerated and haemorrhagic nodules on the oral mucos

Functional brace in the treatment of diaphyseal fractures of the proximal phalanges of the last four fingers.

The authors describe a system of functional brace for the treatment of displaced fractures of the proximal phalanx of the fingers. Using four digit syndactylization this technique allows simultaneous stabilisation of the fracture and early active mobilization of the proximal interphalangeal joint, where stiffness is known as the principal impairment after such fractures. Among 14 cases treated by this method, fracture healing has been obtained in all cases with only one significant malunion. No severe stiffness has been observed and active motion at the PIP joint has been in a useful sector of

Symptomatic epiphyseal sprains and stress fractures of the finger phalanges in adolescent sport climbers.

The purpose of this study was to document and evaluate patient characteristics, injury mechanisms and clinical outcomes of epiphyseal stress fractures of the finger phalanges in adolescent rock climbers to identify contributing factors to this injury. Twenty-eight climbers with epiphyseal pain treated at our clinic between 2006 and 2018 were included in the study. Sixteen patients completed a questionnaire addressing injury details as well as training regimen before and after the injury. The mean age at the time of injury was 13.7 years (±1.9 years) with the injury occurring predominantly in m
