The Utility of Postoperative Radiographs 2 Years After Primary Total Knee Arthroplasty.

BACKGROUND: Radiographs are routinely used to evaluate patients postoperatively after total knee arthroplasty, but no evidence-based guidelines exist regarding their use. The purpose of this study is to quantify the use of radiographs within 2 years of primary total knee arthroplasty by one surgeon and to determine if routine studies in asymptomatic patients altered patient management. METHODS: Patients undergoing consecutive primary total knee arthroplasties between 2008 and 2010 were identified. Patients undergoing revision or additional simultaneous procedures or those with less than 6 mon

Vertebral body fracture after anterior cervical discectomy and fusion with zero-profile anchored cages in adjacent levels: a cautionary tale.

BACKGROUND CONTEXT: Zero-profile (also called self-locking, anchored or stand-alone cages) have been recently proposed as an interesting alternative for anterior cervical discectomy and fusion (ACDF), as they are supposed to reduce the rates of post-operative cage extrusion without necessarily incurring in the additional surgical time and increased rates of dysphagia associated with plating. Nevertheless, the exact indications of zero-profile anchored cages have not yet been established in the literature. PURPOSE: To report the first case of a vertebral body fracture between the blades of zer

The RASE Technique for Extraction of a Protruding Aorto-Ostial Coronary Stent.

BACKGROUND: Significant protrusion of an aorto-ostial coronary stent into the aortic root may complicate transcatheter aortic valve replacement (TAVR) by increasing the risk of valve-stent interaction, TAVR balloon rupture, or stent deformation. Safe management strategies remain poorly defined. CASE SUMMARY: An 84-year-old man undergoing pre-TAVR evaluation was found to have ostial right coronary artery disease; this was treated with percutaneous coronary intervention, resulting in 7-mm stent protrusion into the aorta, precluding safe TAVR. After heart team evaluation, the patient underwent i

Automated detection and classification of dental trauma in periapical radiographs using deep learning: a study based on the Andreasen classification.

OBJECTIVE: The aim of this study was to evaluate the performance of a deep learning (DL) model in automatically identifying dental trauma types on selected periapical radiographs, classified according to the Andreasen system. METHODS AND MATERIALS: Selected periapical radiographs were annotated based on the Andreasen classification. Using these annotations, a YOLOv8-based DL model was developed to classify trauma types. Because of the large number of trauma subtypes and the limited dataset size, labels were later consolidated into two main categories, and a second model was trained. The perfo

Applicability of combined high-frequency and contrast-enhanced ultrasound in finger extensor tendon injuries: three case reports.

BACKGROUND: By combining high-frequency and contrast-enhanced ultrasound (CEUS), the position of the severed end of a finger extensor tendon injury and the injury classification can be determined as part of a comprehensive preoperative evaluation in clinical practice. However, there have been no reports of high-frequency ultrasound combined with CEUS for the preoperative diagnosis of human finger extensor tendon injury. CASES PRESENTATION: One case of complete rupture of the extensor tendon was diagnosed by ultrasound, which was completely consistent with the surgery; one case of incomplete r

Primary Arthroscopic Repair of a Traumatic Isolated Subscapularis Tendon Rupture in an Adolescent Patient.

Subscapularis tendon tears in the pediatric population are most commonly associated with an avulsion fracture of the lesser tuberosity. Isolated subscapularis tendon ruptures are infrequently reported. According to previous reports, the diagnosis of subscapularis tendon injuries in pediatric patients is often delayed and management is typically conservative. When operative management is indicated, an open deltopectoral approach has been used and may include concomitant open reduction and internal fixation of the lesser tuberosity. The authors report the case of a healthy 15-year-old boy who ha

Analysis of occupant kinematics and dynamics in nearside oblique impacts.

OBJECTIVE: The objective of this article is to analyze the kinematics and dynamics of restrained postmortem human surrogates (PMHS) exposed to a nearside oblique impact and the injuries that were found after the tests. METHODS: Three male PMHS of similar age (64 ± 4 years) and anthropometry (weight: 61 ± 9.6 kg; stature: 172 ± 2.7 cm) were exposed to a 30° nearside oblique impact at 34 km/h. The test fixture approximated the seating position of a front seat occupant. A rigid seat was designed to match the pelvic displacement in a vehicle seat. Surrogates were restrained by a 3-point seat belt

Whole-spine dynamic magnetic resonance study of contortionists: anatomy and pathology.

OBJECT: Whole-spine magnetic resonance (MR) images were obtained using a cylindrical 3-T MR imaging system in 5 contortionists to assess the pathological changes possibly associated with the practice of contortion. Whole-spine dynamic MR images were obtained using a 1-T open MR imaging system in 2 of these contortionists with the purpose of defining the range of motion (ROM) achieved during extreme contortion. The range of spinal motion in this unique population was then quantified. METHODS: The study included 5 female contortionists 20-49 years of age. Imaging was performed using open 1-T an

Latissimus dorsi free flap in orthopedic reconstructive surgery.

In complex musculoskeletal diseases, soft-tissue reconstruction is usually the key to a good clinical result. Selection of an ideal flap is a critical step in successful reconstruction. We performed latissimus dorsi free flap transfers in 52 patients with orthopedic disorders from 1988 to 1993. Recipient sites were the lower extremities (44), upper extremities (six) and buttocks (two). The indications for flap surgery comprised chronic ulcer (seven), osteomyelitis (nine), compound fractures (28), crushing or avulsion injuries (six) and malignancy (two). All of the flaps survived and the therap

Spontaneous laryngeal disease in the canine.

This report describes the type and incidence of spontaneous laryngeal disease in the dog. Signs of laryngeal disease are similar to those in other species. Dogs are usually presented with inspiratory obstructive dyspnea or stridor, since earlier signs are often missed. Severe, congenital abnormalities may occur unrecognized due to neonatal death. Subtle anomalies are seldom presented. Congenital paralysis of the laryngeal musculature has been seen in the Bouvier des Flandres and the Siberian Husky. Laryngeal obstruction occurs commonly in brachycephalic dogs (Bulldog, Boxer, Boston Terrier, Pu

A new minimally invasive surgical technique for medial retinaculum repair following traumatic patellar dislocation.

A new surgical method is introduced offering a less invasive approach to reattach the medial retinaculum following acute patellar dislocation. This retrospective analysis comprised 12 cases of medial retinacular repair in 10 patients. The surgical technique achieved reinforced reattachment of the torn region of the medial retinaculum for improved patellar support and stabilization. During follow-up, no recurrent patellar dislocations occurred, except where one patient reported a subjective feeling of patellar dislocation. The average Kujala score for our sample group after 2 years was 89.2. A

Finger reconstruction with a free neurovascular wrap-around flap from the big toe.

From 1983 to 1998, 16 cases of finger reconstruction with a free neurovascular wrap-around flap from the big toe were treated. Fourteen cases were successful, and two cases failed. The authors reviewed these cases on the average of about 38 months after surgery. Pinch power was 51 percent of the unaffected normal hand, and two-point discrimination was 7.6 mm. The mean resorption of the grafted bone was 13 percent in width and 9 percent in length. There were no complications such as fracture of the grafted bone, nonunion, and pulp dislodgement. This procedure provided length, stability, and ade

Screw Fixation of Pediatric Tibial Tubercle Fractures.

BACKGROUND: Tibial tubercle fractures are rare injuries that account for <1% of physeal fractures. These fractures are thought to be increasing in frequency, particularly in young, adolescent males who participate in basketball and other sports with repeated running and jumping. The tibial apophysis becomes mechanically vulnerable as the proximal tibial physis closes from posteromedial to anterolateral, enabling the quadriceps to overpower the chondroepiphysis and avulse the proximal tibial epiphysis from the tibial metaphysis. DESCRIPTION: Position the patient supine with the leg on a bump o
