BACKGROUND: There is an ongoing debate about the best closure technique after distal pancreatectomy (DP). The aim of the closure is to prevent the formation of a clinically relevant post-operative pancreatic fistula (POPF). Stapler technique seems to be equal compared with hand-sewn closure of the remnant. For both techniques, a fistula rate of approximately 30% has been reported.

The present study is designed to elucidate the mechanisms of fat malassimilation following total gastrectomy. Using male Wistar rats, total gastrectomy with an esophagoduodenostomy was performed. We present the case of a 80 years-old male patient who underwent a CT angiogram due to hematemesis and hypovolemic shock. An upper gastrointestinal endoscopy revealed a large clot that closed the antrum and abundant red blood in the pyloric-antrum region. Sclerosis was performed blindly and the bleeding origin was not identified and the success of the sclerosis could not be evaluated. A primary aorta-duodenal fistula was observed by CT angiography, which was treated with an endograft and femoro-femoral bypass. The patient was discharged 14 days after admission.

Sigmoidocutaneous fistulas due to sigmoid colon diverticulitis are very rare. Here we report a case in which laparoscopic sigmoidectomy was used to successfully treat a sigmoidocutaneous fistula due to diverticulitis. Aortic aneurysm has been reported to be the dominant cause of primary iliac-enteric fistula (IEF) in >70% of cases [1]; other less common causes of primary IEF include peptic ulcer, primary aortitis, pancreatic pseudocyst, or neoplastic erosion into an adjacent artery [2, 3]. We describe an unusual case of IEF managed with a staged approach using an endovascular stent-graft as a "bridge" in the emergency setting to optimize the next elective definitive excision of the lesion.

Colocutaneous fistulas within laparotomy wounds are rare and difficult to treat. Surgical repair may be contraindicated or not desired and negative pressure wound therapy may not be successful.
