Mechanism patterns and age variations in pediatric cranio-maxillofacial trauma: a 5-year analysis of the national trauma data bank.

PURPOSE: Pediatric craniomaxillofacial trauma presents unique diagnostic and management challenges due to the anatomical and developmental characteristics of children. METHODS: This retrospective study analyzed 795,431 pediatric trauma cases (ages 0-18 years) from the National Trauma Data Bank (2017-2022) to characterize injury patterns and risk factors. RESULTS: Among 119,324 cases (15.0%) involving craniomaxillofacial fractures, incidence increased significantly with age, from 0.93% in infants to 5.59% in adolescents. Motor vehicle collisions were the leading mechanism, doubling fracture r

[Craniofacial trauma. Value and methods of early treatment. 70 cases (author's transl)].

The two essential problems posed by craniofacial trauma are assessed in the light of 70 collected cases. Firstly, the "time" of treatment. Immediate surgery by two teams, neurosurgical and maxillofacial, may be justified in the presence of craniocerebral lesions. This occurs rarely. The cranial problem is often explored during the first 3 or 4 days and with the exception of certain cases of immediate or delayed coma, maxillofacial treatment may be undertaken early. The treatment of cerebrospinal fluid fistulae remains controversial. Secondly, the surgical technique. Accent is placed upon two p

Effect of radiation injury on the growing face.

The literature on the effect of ionizing radiations on developing and rapidly growing tissue has been reviewed, and the effect especially on children who received radiation therapy in the Clinic for Maxillo-Facial and Plastic Surgery of the University of Dusseldorf for various tumours of the head and neck presented. Our findings indicate that we do not yet know the minimal irradiation dosage that may cause growth retardation in infants and children. It has therefore been suggested that surgery should be more frequently considered in the treatment of childhood tumours, since this caused less si

Facial asymmetry in subjects with skeletal Class III deformity.

We investigated the frequency, site, amount, and direction of facial asymmetry in human adults with mandibular prognathism and examined if these characteristics were associated postnatally with cardinal clinical signs that may indicate a predisposition to facial asymmetry. Two hundred twenty young Japanese adults (69 men and 151 women) who exhibited skeletal Class III malocclusions were selected. The sample was divided into a Postnatal Factor Group and a Nonpostnatal Factor Group. The former group included those who had: (1) received orthodontic treatment using a chin cap; (2) exhibited clinic

Negative pressure wound therapy in complex cranio-maxillofacial and cervical wounds.

The care and the management of the healing of difficult wounds at the level of the skull-facial face many problems related to patient compliance and the need to perform multiple dressings, with long periods of healing and, occasionally, a very long hospitalisation period. The introduction and evolution of negative pressure wound therapy (NPWT) in the treatment of difficult wounds has resulted in better healing, with a drastic reduction in terms of time and biological costs to the patient and cost to the health care system. The main aim of this study is to describe and discuss, using out our ex

The psychosocial characteristics and needs of patients presenting with orofacial injury.

Individuals with orofacial injury presenting to urban trauma centers in the United States tend to be disproportionately socioeconomically disadvantaged, young, adult, ethnic minority men. Most injuries are assaultive in origin, suggesting poor impulse control and maladaptive social behaviors. Compared with matched control populations, patients with orofacial injuries are more likely to report higher levels of substance use behaviors and to manifest greater levels of hostility, anxiety, and depression. Although they have significantly greater current and lifetime need for mental health service

[Surgery of asymmetry].

Endeavoring to present, within a few pages, an exhaustive work of all the surgical proposals, regarding the multitude of ethiopathogeneses, is quite utopic. The author considers the evaluation of the asymmetrical patient and underlines in the complementary examinations the advantage of 3D tomodensitometry. A chapter is devoted to presurgical orthodontic preparation aimed at counteracting the compensations, which affect the maxillary and mandibular alveolar support. The therapeutic strategy calls upon "combined" programs. A "range" of osteotomies is suggested within the framework of the most fr

Assessment of accuracy of navigated implant placement in the maxilla.

PURPOSE: The use of computed tomography (CT) based intraoperative navigation has greatly improved surgery in many specialties. In this study, the precision of the SMN system (Zeiss, Oberkochen, Germany) for navigated drilling and following implant placement in the maxilla was evaluated. This study should demonstrate the suitability of navigation systems for computer-assisted implantation in the maxilla to avoid perforation of the maxillary sinus. MATERIALS AND METHODS: Sixty target drillings were carried out on 10 standardized polyurethane milling models after CT scanning. The models were pro

Maxillofacial Fractures: Midface and Internal Orbit-Part II: Principles and Surgical Treatment.

Current clinical assessment and imaging techniques were described in part 1, and this article presents a systematic review of the surgical treatment principles in the management of midface and internal orbit fractures from initial care to definitive treatment, including illustrative case examples. New developments enabled limited surgical approaches by standardization of osteosynthesis principles regarding three-dimensional buttress reconstruction, by newly developed individualized implants such as titanium meshes and, especially for complex fracture patterns, by critical assessment of anatomi

Surgical Treatment of Pediatric Craniofacial Fractures: A National Perspective.

INTRODUCTION: Head trauma is the most common cause of death because of injury in children, and trauma alone is the leading cause of morbidity and mortality in pediatrics. This study aimed to characterize the demographics and economic burden associated with the surgical and nonsurgical repair of craniofacial fractures in the pediatric inpatient population in the United States. METHODS: A retrospective cohort study was performed using the 2012 Kids' Inpatient Database which identified 20,070 patients who had a skull or facial fracture, of whom 6395 (31.9%) were treated surgically. Epidemiologic

Complications of self-drilling intermaxillary fixation screws with a focus on dental root damage.

Intermaxillary fixation is often used in oral and maxillofacial surgery for temporary jaw stabilization. Self-drilling intermaxillary fixation screws are easy to use but can cause dental damage. This retrospective study was performed to examine the complications associated with self-drilling screws, with a focus on dental root damage. Overall, 274 screws were placed in 43 patients between 2010 and 2023 at University Medical Center Utrecht. Patients with missing postoperative imaging or screws placed in edentulous areas were excluded, leaving 162 screws in 28 patients for evaluation. Complicati

An 11-year multicentric surgical experience on pediatric orbital floor trapdoor fracture: A World Oral Maxillofacial Trauma (WORMAT) project.

INTRODUCTION: Trapdoor fractures of the orbital floor occur almost exclusively in the paediatric population. Despite being widely discussed in the literature, their management remains controversial. The objective of this retrospective study was to analyse the surgical experiences on paediatric trapdoor fractures in the maxillofacial centres participating in the WORMAT project. MATERIALS AND METHODS: 14 centres collected data for patients aged ≤16 years operated between January 2011 and December 2022. The demographic, cause and type of fracture, timing from injury to surgery, surgical approach
