Optimizing the detection of bipolar II disorder in outpatient private practice: toward a systematization of clinical diagnostic wisdom.

BACKGROUND: We review a clinical diagnostic approach to validate a redefinition of bipolar II disorder (BPII), which bypasses several conservative steps in the DSM-IV Mood Module of the Structured Clinical Interview for DSM-IV Axis I Disorders, Clinician Version (SCID-CV) to make detection of BPII more "clinician-friendly." METHOD: 563 consecutive private outpatients presenting with a DSM-IV-diagnosed major depressive episode (MDE) were included in the analyses. We used a modified SCID-CV in a semistructured way, used a duration of hypomania > or =2 days (rather than the 4-day floor cutoff re

Association of Human Papillomavirus Status With Suicide Risk Among Patients With Head and Neck Cancer.

IMPORTANCE: Human papillomavirus (HPV) is strongly associated with head and neck cancer, and HPV status is considered a prognostic factor. Being a sexually transmitted infection, HPV-related cancers may have greater risk of stigma and psychological distress; however, the potential association of HPV-positive status with psychosocial outcomes, such as suicide, is understudied in head and neck cancer. OBJECTIVE: To investigate the association between HPV tumor status and suicide risk among patients with head and neck cancer. DESIGN, SETTING, AND PARTICIPANTS: This population-based retrospectiv

Effectiveness of discharge planning interventions on health-related outcomes among postpartum women: a systematic review and meta-analysis.

BACKGROUND: Postpartum women frequently experience psychological distress, physical morbidities, and sub-optimal maternal-neonatal care skills. Structured discharge planning may mitigate these problems, yet its overall effectiveness and the active ingredients underpinning benefit remain unclear. OBJECTIVE: To synthesis randomized and quasi-experimental evidence on the effectiveness of discharge planning interventions for improving postpartum women's health outcomes and to identify intervention characteristics that modify this effectiveness. METHODS: Nine electronic databases (including PubMe

Harnessing leadership experiences for improved healthcare delivery: A scoping review of the experiences of healthcare leaders in navigating crisis.

BACKGROUND: The COVID-19 pandemic placed unprecedented strain on health care systems worldwide, exposing and amplifying longstanding vulnerabilities in leadership and management structures. Although the roles of frontline healthcare workers have been widely documented, the experiences of healthcare leaders remain understudied. OBJECTIVE: To systematically synthesize global evidence on healthcare leaders' experiences in navigating crises, with a particular focus on the pandemic; and to derive actionable strategies to strengthen crisis preparedness, response capacity, and health system resilien

Did national implementation of interpersonal counseling improve adolescent depression treatment pathways in primary care? Protocol for a longitudinal cohort study.

BACKGROUND: Implementation of evidence-based interventions is one of the proposed responses to increased demand for treatment of adolescent depression. While the efficacy of interpersonal psychotherapy to treat depression of adolescents (IPT-A) is well established, the shorter adolescent interpersonal counseling (IPC-A) was implemented in primary care in Finland due to better feasibility. We present a protocol for a prospective evaluation of whether implementation of IPC-A has improved the naturalistic treatment flow and outcome of adolescents with sustained depression. We will evaluate the co

Simple Mobile technology health management tool for people with severe mental illness: a randomised controlled feasibility trial.

BACKGROUND: Severe mental illness (SMI) is associated with care delivery problems because of the high levels of clinical resources needed to address patient's psychosocial impairment and to support inclusion in society. Current routine appointment systems do not adequately foster recovery care and are not systematically capturing information suggestive of urgent care needs. This study aimed to assess the feasibility, acceptability, and potential clinical benefits of a mobile technology health management tool to enhance community care for people with severe mental illness. METHODS: This random

Optimizing Social-Emotional-Communication Development in Infants of Mothers With Depression: Protocol for a Randomized Controlled Trial of a Mobile Intervention Targeting Depression and Responsive Parenting.

BACKGROUND: Postpartum depression interferes with maternal engagement in interventions that are effective in improving infant social-emotional and social-communication outcomes. There is an absence of integrated interventions with demonstrated effectiveness in both reducing maternal depression and promoting parent-mediated practices that optimize infant social-emotional and social-communication competencies. Interventions targeting maternal depression are often separate from parent-mediated interventions. To address the life course needs of depressed mothers and their infants, we need brief, a

An Online Minimally Guided Intervention to Support Family and Other Unpaid Carers of People With Dementia: Protocol for a Randomized Controlled Trial.

BACKGROUND: About three-quarters of people with dementia live in their own homes, with help from family members and/or other unpaid carers, such as friends or neighbors. Often, unpaid carers themselves experience negative consequences, such as stress, burden, and symptoms of depression or anxiety. Research has shown that these consequences can be alleviated by psychosocial and psychological interventions. Moreover, there are indications that those interventions can be effective when offered online. OBJECTIVE: This paper describes the protocol of a randomized controlled trial (RCT) that will t

Psychological distress, self-determination, and social capital associated with quality of life among retirees in Sarawak: a cross-sectional structural equation modelling analysis.

BACKGROUND: Population aging is accelerating in middle-income countries; however, evidence on the psychosocial factors associated with later-life quality of life (QoL) remains limited in Southeast Asian settings. Psychological distress, self-determination, and social capital have each been linked to well-being in older populations, but few studies have examined these constructs together among retirees in geographically dispersed, multiethnic populations. This study examined the associations between psychological distress, self-determination, social capital, catastrophic health expenditure (CHE

Development of Virtual Reality Health Literacy: Delphi Expert Consensus Study.

BACKGROUND: Virtual reality (VR) is a promising tool in health care, offering immersive and interactive environments that can enhance patient education, rehabilitation, and mental health interventions. However, effective patient engagement with head-mounted display (HMD)-based immersive VR depends on a combination of functional competencies and readiness-related determinants that have not yet been systematically defined. OBJECTIVE: This study aimed to conceptualize an initial framework of VR health literacy, focused on HMD-based immersive VR in clinical settings, and to achieve expert consens

A Multi-institutional Prospective Observational Study of Stereotactic Radiosurgery for Patients With Multiple Brain Metastases (JLGK0901 Study Update): Irradiation-related Complications and Long-term Maintenance of Mini-Mental State Examination Scores.

PURPOSE: The JLGK0901 study showed the noninferiority of stereotactic radiosurgery (SRS) alone as initial treatment of 5 to 10 brain metastases (BMs) compared with 2 to 4 BMs in terms of overall survival and most secondary endpoints (Lancet Oncol 2014;15:387-95). However, observation periods were not long enough to allow confirmation of the long-term safety of SRS alone in patients with 5 to 10 BMs. METHODS AND MATERIALS: This was a prospective observational study of Gamma Knife SRS-treated patients with 1 to 10 newly diagnosed BMs enrolled at 23 facilities between March 1, 2009, and February

Problems in psychotherapy with suicidal patients.

OBJECTIVE: The authors studied recurrent problems in psychotherapy with suicidal patients by examining the cases of patients who died by suicide while receiving open-ended psychotherapy and medication. METHOD: Therapists for 36 patients who died by suicide while in treatment filled out clinical, medication, and psychological questionnaires and wrote detailed case narratives. They then presented their cases at an all-day workshop, and critical problems were identified in the cases. RESULTS: Six recurrent problem areas were identified: poor communication with another therapist involved in the

General practitioners' management of mental disorders: a rewarding practice with considerable obstacles.

BACKGROUND: Primary care improvement is the cornerstone of current reforms. Mental disorders (MDs) are a leading cause of morbidity worldwide and widespread in industrialised countries. MDs are treated mainly in primary care by general practitioners (GPs), even though the latter ability to detect, diagnose, and treat patients with MDs is often considered unsatisfactory. This article examines GPs' management of MDs in an effort to acquire more information regarding means by which GPs deal with MD cases, impact of such cases on their practices, factors that enable or hinder MD management, and pa

Clinical Reasoning in the Ward Setting: A Rapid Response Scenario for Residents and Attendings.

INTRODUCTION: There is a need for educational resources supporting the practice and assessment of the complex processes of clinical reasoning in the inpatient setting along a continuum of physician experience levels. METHODS: Using participatory design, we created a scenario-based simulation integrating diagnostic ambiguity, contextual factors, and rising patient acuity to increase complexity. Resources include an open-ended written exercise and think-aloud reflection protocol to elicit diagnostic and management reasoning and reflection on that reasoning. Descriptive statistics were used to a

Patient requests for euthanasia and assisted suicide in terminal illness. The role of the psychiatrist.

Psychosocial assessment and treatment are critical elements of care for terminally ill patients who desire hastened death. Most patients, in saying that they want to die, are asking for assistance in living--for help in dealing with depression, anxiety about the future, grief, lack of control, dependence, physical suffering, and spiritual despair. In this article, the authors review current understandings of the psychiatric aspects of requests by terminally ill patients for assisted suicide and euthanasia; describe an approach to the common problems of physical, psychological, social, and spir
