Emergency Medical Services Outcomes Project (EMSOP) II: developing the foundation and conceptual models for out-of-hospital outcomes research.

Development of methodologically acceptable outcomes models for emergency medical services (EMS) is long overdue. In this article, the Emergency Medical Services Outcomes Project proposes a conceptual framework that will provide a foundation for future EMS outcomes research. The "Episode of Care Model" and the "Out-of-Hospital Unit of Service Model" are presented. The Episode of Care Model is useful in conditions in which interventions and outcomes, especially survival and major physiologic dysfunction, are linked in a time-dependent manner. Conditions such as severe trauma, anaphylaxis, airway

Generative models of living development: an interaction-centered proposal and its implications for artificial life research.

Artificial life and integrative systems neuroscience increasingly seek to explain how agency is organized through development, not merely how an already-formed controller optimizes a predefined task. This article advances an interaction-centered theory that links computational generative models, which infer hidden causes and candidate action policies, with experimental ecologies, which generate the developmental histories to be explained. The framework distinguishes reward and prior preference from concern, normativity, habit, and action policy. Concerns denote system-relative dimensions of re

Utilizing shared frailty with the Cox proportional hazards regression: Post discharge survival analysis of CHF patients.

Understanding patients' survival probability as well as the factors affecting it constitute a significant concern for researchers and practitioners, in particular for patients with severe chronic illnesses such as congestive heart failure (CHF). CHF is a clinical syndrome characterized by comorbidities and adverse medical events. Risk stratification to identify patients most likely to die shortly after hospital discharge can improve the quality of care by better allocating organizational resources and personalized interventions. Probability assessment improves clinical decision-making, contrib

Occupational safety and health risks among plumbers: A qualitative study of workplace hazards, safety practices, and system-level gaps in Texas.

BACKGROUND: The plumbing trade exposes workers to ongoing workplace hazards that pose significant health risks. This issue is well known but remains unresolved. A key question remains: why have current safety measures, such as training programs, regulatory standards, and personal protective equipment (PPE) requirements, failed to reduce injury rates or significantly improve safety in the plumbing field. OBJECTIVES: Despite well-documented hazards in plumbing work, qualitative research examining why existing safety measures continue to fail plumbers-particularly in trade-specific U.S. contexts

A Service-Constrained Two-Stage Stochastic Programming Framework for Hospital Pharmacy Medication Supply Under Demand Uncertainty: Heterogeneous Demand, Conditional GARMA Scenarios, and Longitudinal Validation.

Background/Objectives: Hospital pharmacies must maintain medication availability despite heterogeneous and time-varying demand. Statistical fit alone may not yield well-calibrated future scenarios or the best replenishment decision. This study prospectively evaluates a decision-support framework combining probabilistic demand modelling, conditional temporal adaptation, explicit service requirements, and stochastic inventory optimization. Methods: Forty-eight monthly requested-demand observations for 13 anonymized medicines from a Chilean public hospital were evaluated through 12 expanding-wind

Shift work and BMI in nursing professionals: A cross-sectional mediation analysis through psychological distress and irregular eating patterns.

BackgroundShift work is a well-established occupational risk factor for psychological strain in healthcare professionals, yet its association with physical health outcomes such as body mass index (BMI) remains inconsistent. Drawing on a biopsychosocial and occupational health psychology framework, this study examined the psychological and behavioral pathways that may underlie the association between shift work and BMI in nursing professionals.ObjectiveTo examine, using a cross-sectional structural equation modeling approach, whether psychological distress and irregular eating patterns were ass

Social return on investment of physical activity and sport (PAS): The Japan findings.

This study investigates Japan's national Social Return on Investment (SROI) of physical activity and sport (PAS) by adapting the pioneering UK model to reflect Japan's sociocultural and policy context. As this study is the first national-level SROI model for PAS in Asia, it addresses a key research gap and responds to calls for geographical diversification. The conservative base-case model quantified the social value of PAS by monetizing outcomes with stronger consensus evidence, including health improvements and subjective well-being, while accounting for sport-related injuries to avoid overc

Specialist integrated haematological malignancy diagnostic services: an Activity Based Cost (ABC) analysis of a networked laboratory service model.

AIMS: Specialist Integrated Haematological Malignancy Diagnostic Services (SIHMDS) were introduced as a standard of care within the UK National Health Service to reduce diagnostic error and improve clinical outcomes. Two broad models of service delivery have become established: 'co-located' services operating from a single-site and 'networked' services, with geographically separated laboratories linked by common management and information systems. Detailed systematic cost analysis has never been published on any established SIHMDS model. METHODS: We used Activity Based Costing (ABC) to constr

Finding needles in a haystack: a methodology for identifying and sampling community-based youth smoking cessation programs.

BACKGROUND: Surveys of community-based programs are difficult to conduct when there is virtually no information about the number or locations of the programs of interest. This article describes the methodology used by the Helping Young Smokers Quit (HYSQ) initiative to identify and profile community-based youth smoking cessation programs in the absence of a defined sample frame. METHODS: We developed a two-stage sampling design, with counties as the first-stage probability sampling units. The second stage used snowball sampling to saturation, to identify individuals who administered youth smo

Schizophrenia treatment in the developing world: an interregional and multinational cost-effectiveness analysis.

OBJECTIVE: Schizophrenia is a highly disabling disease and is costly to treat. We set out to establish what are the most cost-effective interventions applicable to developing regions and countries. METHODS: Analysis was undertaken at the level of three WHO subregions spanning the Americas, Africa and South-East Asia, and subsequently in three member states (Chile, Nigeria and Sri Lanka). A state transition model was used to estimate the population-level health impact of older and newer antipsychotic drugs, alone or in combination with psychosocial intervention. Total population-level costs (i

All-in-one spinal cord stimulation and its economic impact.

OBJECTIVES: This study estimates the economic and organizational impact of adopting the All-in-One Spinal Cord Stimulation (SCS) implantation strategy compared to the traditional two-stage approach for chronic pain patients in Spain. We assess savings, resource utilization, and the success rate threshold above which it would be more profitable to perform the procedure in a single phase. METHODS: A retrospective study analyzed Real-World Data (RWD) from chronic pain patients treated with SCS between 2012 and 2023. Data included socio-demographics, resource use, and post-implant procedures. A he

Eco-conscious internal communication and psychological drivers of eco-centric creativity: A three-wave PLS-SEM study of Chinese SMEs.

In response to the demands of Sustainability 4.0 and the Sustainable Development Goals (SDGs), this study explored the role of eco conscious internal communication in creating eco centric creativity among employees of small and medium sized enterprises (SMEs). We hypothesized that the link between supervisor environmental priority is mediated by sustainability-oriented mindfulness and eco conscious personal identity and that supervisor environmental priority strengthens these pathways. A three-wave survey design was employed; 800 questionnaires were distributed to SME (Small & Medium Enterpris

Usability of a cross-system housing stability program for juvenile courts: A multimethod study of probation counselor perspectives.

BACKGROUND: Youth in contact with the juvenile legal system experience disproportionate rates of homelessness. While system contact is a critical intervening point, juvenile courts do not typically offer housing services. One solution is to refer youth to evidence-based, community-based services to meet housing-related needs, but a myriad of individual and organizational factors often impede court staff from making such referrals. Housing Stability for Youth in Courts (HSYNC) is a novel, cross-system service linkage model for court-involved youth facing housing instability. HSYNC was developed

Nurses' perspectives on implementing sleep protection for premature infants in the neonatal intensive care unit: a qualitative study.

BACKGROUND: The normal development of sleep-wake cycles is crucial for the long-term neurological health of preterm infants, yet sleep protection practices remain suboptimal. Given China's large population of preterm infants and its distinct cultural background and healthcare policies compared to Western countries, NICU nurses in China may face unique challenges. However, our understanding of the barriers and facilitators encountered by nurses in implementing sleep protection for preterm infants is limited. METHODS: From November 2023 to February 2024, we conducted semi-structured interviews
