Compressive lumbar myelopathy presenting as segmental motor neuron disease.

Four patients presented with slowly progressive, bilateral, asymmetric weakness and muscle atrophy in the lower extremities, accompanied by cramps and fasciculations. Sensory symptoms were insignificant. There was no bladder or bowel disturbance. Upper extremities and cranial nerves were normal. Weakness was found in lumbosacral myotomes, ranging from L2 to S1. The tendon reflexes varied, and extensor plantar responses were found in one case with proximal leg involvement. Nerve conduction studies were normal, but segmental chronic and often active denervation confined to the weak myotomes in t

PDZRN3 regulates adipogenesis of mesenchymal progenitors in muscle.

INTRODUCTION: Intramuscular adipose tissue (IMAT) is frequently formed in certain pathological conditions, such as biological aging, and ectopic fat accumulation leads to muscle weakness and a subsequent decline in physical function. Although mesenchymal progenitors (MPs) are present in postnatal skeletal muscle and are the cells from which IMAT originates, the molecular mechanism by which MPs contribute to IMAT formation has not been completely elucidated. Recently, we found that PDZ domain-containing ring finger 3 (PDZRN3), an E3-ubiquitin ligase, was highly expressed in MPs. In this study,

Regenerative Peripheral Nerve Interface (RPNI) Surgery for Mitigation of Neuroma and Postamputation Pain.

BACKGROUND: A neuroma occurs when a regenerating transected peripheral nerve has no distal target to reinnervate. Symptomatic neuromas are a common cause of postamputation pain that can lead to substantial disability1-3. Regenerative peripheral nerve interface (RPNI) surgery may benefit patients through the use of free nonvascularized muscle grafts as physiologic targets for peripheral nerve reinnervation for mitigation of neuroma and postamputation pain. DESCRIPTION: An RPNI is constructed by implanting the distal end of a transected peripheral nerve into a free nonvascularized skeletal musc

Electrodiagnostic Studies as a Diagnostic and Prognostic Tool in Acute Flaccid Myelitis.

INTRODUCTION/AIMS: Acute flaccid myelitis (AFM) is a poliomyelitis-like syndrome, characterized by severe flaccid weakness. Nerve conduction studies (NCS) and electromyography (EMG) may have a role in the diagnosis and prognostication of AFM, but have not been well studied. We aim to describe in detail electrodiagnostic studies in AFM and relate findings to outcome. METHODS: In this retrospective multi-center case series, we analyze electrodiagnostic data from patients with a probable or definite diagnosis of AFM. Patients were enrolled at 11 sites from four countries. A detailed description

Assessment of forearm pronation strength in C6 and C7 radiculopathies.

STUDY DESIGN: Consecutive case series of patients with C6 and C7 radiculopathies. OBJECTIVES: To explore the clinical utility and reliability of manual muscle testing of forearm pronation strength in C6 and C7 radiculopathies. SUMMARY OF BACKGROUND DATA: EMG evidence of denervation of the pronator teres was the most common finding in C6 radiculopathies, and frequently present in C7 radiculopathies. Clinical evaluation of the pronator teres through manual muscle testing of forearm pronation has never been explored; therefore, its clinical utility is unknown as compared with the muscle groups

Regenerative Peripheral Nerve Interface: Surgical Protocol for a Randomized Controlled Trial in Postamputation Pain.

Surgical procedures, including nerve reconstruction and end-organ muscle reinnervation, have become more prominent in the prosthetic field over the past decade. Primarily developed to increase the functionality of prosthetic limbs, these surgical procedures have also been found to reduce postamputation neuropathic pain. Today, some of these procedures are performed more frequently for the management and prevention of postamputation pain than for prosthetic fitting, indicating a significant need for effective solutions to postamputation pain. One notable emerging procedure in this context is th

GFPT1-Associated Congenital Myasthenic Syndrome Mimicking a Glycogen Storage Disease - Diagnostic Pitfalls in Myopathology Solved by Next-Generation-Sequencing.

GFPT1-related congenital myasthenic syndrome (CMS) is characterized by progressive limb girdle weakness, and less prominent involvement of facial, bulbar, or respiratory muscles. While tubular aggregates in muscle biopsy are considered highly indicative in GFPT1-associated CMS, excessive glycogen storage has not been described. Here, we report on three affected siblings with limb-girdle myasthenia due to biallelic pathogenic variants in GFPT1: the previously reported missense variant c.41G > A (p.Arg14Gln) and the novel truncating variant c.1265_1268del (p.Phe422TrpfsTer26). Image-guided botulinum toxin injection in the lateral abdominal wall prior to abdominal wall reconstruction surgery: review of techniques and results.

Ventral hernias represent the most common complication after abdominal surgery. Loss of domain and/or large ventral hernias in patients are especially challenging for surgeons to manage, but preoperative image-guided botulinum toxin injection has emerged as an effective adjunct to abdominal wall surgery. Loss of domain is caused by chronic muscle retraction of the lateral abdominal wall and leads to an irreducible protrusion of abdominal viscera into the hernia sac. Botulinum toxin can be used in the oblique muscles as a chemical component relaxation technique to aid abdominal wall reconstruct

Undirected compensatory plasticity contributes to neuronal dysfunction after severe spinal cord injury.

Severe spinal cord injury in humans leads to a progressive neuronal dysfunction in the chronic stage of the injury. This dysfunction is characterized by premature exhaustion of muscle activity during assisted locomotion, which is associated with the emergence of abnormal reflex responses. Here, we hypothesize that undirected compensatory plasticity within neural systems caudal to a severe spinal cord injury contributes to the development of neuronal dysfunction in the chronic stage of the injury. We evaluated alterations in functional, electrophysiological and neuromorphological properties of

Pulmonary C-fiber activation attenuates respiratory-related tongue movements.

The functional impact of pulmonary C-fiber activation on upper airway biomechanics has not been evaluated. Here, we tested the hypothesis that pulmonary C-fiber activation alters the respiratory-related control of tongue movements. The force produced by tongue movements was quantified in spontaneously breathing, anesthetized adult rats before and after stimulation of pulmonary C fibers via intrajugular delivery of capsaicin (0.625 and 1.25 μg/kg). Brief occlusion of the trachea was used to increase the respiratory drive to the tongue muscles, and hypoglossal (XII) nerve branches were selective

[The role of electromyography in cases of vocal cord paralysis].

BACKGROUND: Several pathologies can involve muscles that control vocal folds. The abnormality can affect peripheral nerves or central nervous system centers. Clinically, muscle function can be assessed by observing the movement of structures themselves or by recording electrical activity of these muscles using (electromyography-EMG). Since EMG is an invasive technique, its use is not very widespread in the diagnosis and management of voice disorders, Laryngeal EMG can be helpful in those patients with voice problems of suspected neurological or neuromuscular etiology. OBJECTIVE: Assess the ro

[Regeneration of the facial nerve in comparison to other peripheral nerves : from bench to bedside].

Despite increasing knowledge of cellular and molecular mechanisms determining the success or failure of peripheral nerve regeneration, no effective treatments for peripheral nerve injury exist. Newly developed and validated approaches for precise numerical assessment of motor deficits have recently allowed testing of novel strategies in experimental animals. One of these approaches is the daily manual stimulation of the denervated musculature. This treatment is effective in cases of cranial nerve lesions with preservation of the sensory input (facial or hypoglossal nerve) and has the potential

[Pregnancy-associated femoral nerve affection].

BACKGROUND: Common neuropathies during pregnancy are carpal tunnel syndrome or peripheral facial paralysis. However, there is little information about femoral nerve affection during pregnancy. CASE REPORT: We report on a female at 30 weeks of gestation, complaining of pain in hips and thighs and gait difficulty. Pregnancy was normally developed. Neurological examination showed a bilateral weakness in both quadriceps and iliopsoas muscles combined with soreness in hips and thighs, without deficits of sensibility. Laboratory findings and results of a lumbar punction were normal. Pelvic ultrasou

Electrophysiologic studies of the median nerve and its palmar cutaneous branches after nerve grafting.

Electromyography (EMG) and nerve conduction studies were performed on a patient with an incomplete lesion of the median nerve involving the motor fibers to the thenar muscles, the palmar cutaneous branches, and the sensory fibers to the index finger. The studies were done before, and 13 weeks, 16 weeks, and one year after grafting. The preoperative electrodiagnostic evaluation showed abnormalities involving the median motor fibers and the palmar cutaneous branch. Electromyography of the opponens pollicis and abductor pollicis brevis before surgery showed severe active denervation. Thirteen wee

Method for counting motor units in mice and validation using a mathematical model.

We hypothesized that patients who fail weaning from mechanical ventilation recruit their inspiratory rib cage muscles sooner than they recruit their expiratory muscles, and that rib cage muscle recruitment is accompanied by recruitment of sternomastoid muscles. Accordingly, we measured sternomastoid electrical activity and changes in esophageal (DeltaPes) and gastric pressure (DeltaPga) in 11 weaning-failure and 8 weaning-success patients. At the start of trial, failure patients exhibited a higher DeltaPga-to-DeltaPes ratio than did success patients (P = 0.05), whereas expiratory rise in Pga w
