The adult Tillaux fracture: one not to miss.

This case is a rare adult type Tillaux fracture. This fracture should be diagnosed in the emergency department. The fracture requires careful evaluation with awareness of associated injury. Standard radiological views (antero-posterior and lateral) of the ankle may not clearly show the fracture displacement hence, an oblique view is required. The fracture should be managed by the emergency physician, if the displacement is less than 2 mm, by immobilising with a non-weight bearing cast or ankle braces for 6 weeks. If the fracture fragment is displaced more than 2 mm, it should be treated by clo

Pronation-Type Galeazzi-Equivalent Fracture: A Case Report.

Fractures of the forearm are common among children and adolescents. Radial shaft fracture with dislocation of the distal radioulnar joint (DRUJ), called Galeazzi fracture, is unusual in pediatrics. The Galeazzi-equivalent fracture is a variant of the classic Galeazzi fracture that occurs in children and adolescents. It is a radius fracture associated with a distal ulnar displaced physeal injury without dislocation of the DRUJ. Our patient was a male, aged 15 years, who visited our emergency department after falling off a scooter onto his left hand. Left wrist X-rays showed a displaced Galeazzi

Patient-reported outcomes after operative versus nonoperative treatment of pediatric lateral humeral condyle fractures.

Lateral humeral condyle fractures in children are treated with several approaches, yet it is unclear which has the best treatment outcomes. We hypothesized that functional outcomes would be equivalent between treatment types, reduction approaches, and fixation types. Our purpose was to assess patient-reported outcomes and complications by treatment type (operative versus nonoperative), reduction approach (open versus percutaneous), and fixation type (cannulated screws versus Kirschner wires). We retrospectively reviewed data from acute lateral humeral condyle fractures treated at our level-1 pe

Pilon fractures. Treatment protocol based on severity of soft tissue injury.

One hundred seven pilon fractures in 107 patients were treated according to a staged prospective protocol. All pilon fractures were stabilized immediately by the application of calcaneal traction. Open fractures or fractures in patients with multiple injuries were stabilized with traveling traction that was applied in the operating room. A distraction computed tomography scan was obtained before definitive treatment. Treatment groups were based on the degree of soft tissue compromise. Forty-one patients with Tscherne Grade 0 or Grade I injuries underwent open reduction and internal fixation (o

Factors influencing implant failure in cephalomedullary nails for proximal femoral fractures: A retrospective review.

AIMS: The use of cephalomedullary nailing (CMN) to treat extracapsular hip fractures is a well-established fixation method. Nationally, the use of CMN has increased from 67.7% in 2012 to 90.9% in 2025 for subtrochanteric fractures (National Hip Fracture Database). Although rare, implant failure is a recognised complication which poses risk to patients who are otherwise already burdened with high morbidity and mortality. The purpose of this study is to review the outcomes of CMN fixations for proximal femoral fractures, with particular focus on implant failure. We analysed patient demographics,

Surgical Trends in the Treatment of Supracondylar Humerus Fractures in Early Career Practice: An American Board of Orthopaedic Surgery (ABOS) Part-II Database Study.

BACKGROUND: Pediatric supracondylar humerus fractures are the most common elbow injury in children, accounting for 12% to 17% of all childhood fractures. A lack of information exists regarding complication rates on the basis of fellowship status and geography. METHODS: The American Board of Orthopaedic Surgery (ABOS) Part-II database was used to identify candidates who performed either percutaneous or open treatment of humeral supracondylar or transcondylar fractures between 2002 and 2016 on patients aged 0 to 12 years. Candidates were categorized by fellowship training experience. Informatio

The effect on outcomes of the application of circumferential cerclage cable following intramedullary nailing in reverse intertrochanteric femoral fractures.

INTRODUCTION: The aim of this study was to evaluate the effect on union results of the application of cerclage cable around the lateral femoral wall (LFW) in postoperative fracture displacement. PATIENTS AND METHODS: A retrospective evaluation was made of all the cases of reverse intertrochanteric fractures over a 7-year period in a trauma center. The study included 69 fractures treated with intramedullary nailing. The age of the patients, postoperative reduction quality and complications such as mechanical failure and non-union (1.4%) were obtained from the medical records. Changes in the ne

A minimally invasive technique for the reduction of calcaneal fractures using the Endobutton®.

Calcaneal fractures are very common injuries, and closed reduction-percutaneous pinning, open reduction-internal fixation, and primary arthrodesis are the procedures used in the surgical treatment of these injuries. The aim of surgical treatment is to restore the normal biomechanics of the Achilles tendon and rearfoot by anatomic reduction of the articular surfaces. Minimization of the soft tissue complications commonly associated with the open treatment of calcaneal fractures can be achieved using indirect closed reduction and Kirschner wire or screw-assisted minimally invasive percutaneous o

Timing of Fracture Fixation in Ankle Fracture-Dislocations.

Ankle fracture-dislocations may require delayed internal fixation. Our aim was to compare acute open reduction and internal fixation (ORIF) with delayed ORIF, using external fixation or cast splint in ankle fracture-dislocations. Factors that affect the rates of re-operation and Surgical site infection (SSI) were identified. In this retrospective cohort study, patients were included with open and closed ankle fracture-dislocations treated with ORIF from two large peripheral hospitals and one academic center in the Netherlands. This study included 447 patients with an ankle fracture-dislocation

Hip arthroscope-assisted percutaneous reduction and fixation of displaced subcapital femoral neck fracture.

Reduction quality is associated with fracture prognosis. Displaced subcapital femoral neck fracture has highest possibility of avascular necrosis of femoral head and non-union among the femoral neck fractures, which commonly necessitate revisions or hip replacement. This study introduces for the first time of using hip arthroscope to directly visualize and assist the reduction of displaced subcapital femoral neck fracture when closed reduction is unsatisfactory. Due to the minimally invasive advantage of arthroscopic assistance, radiation exposure or intraoperative bleeding can be reduced, ope

Early versus delayed weight-bearing after ORIF of isolated AO/OTA 44-B1 lateral malleolar fractures: a retrospective cohort study.

BACKGROUND: The optimal timing of weight-bearing after surgical fixation of isolated lateral malleolar fractures remains debated. Although delayed mobilization is commonly practiced, early weight-bearing has been proposed as a strategy to improve functional recovery without increasing complications. This study compared early versus delayed weight-bearing following operative treatment of AO/OTA 44-B1 fractures using a homogeneous fracture cohort. METHODS: This retrospective cohort study included 89 adults who underwent open reduction and internal fixation for isolated lateral malleolar fractur

Assessment of pediatric mandibular fracture management in France.

Mandibular fractures in children present management challenges due to pediatric-specific factors such as mandibular growth and the presence of dental germs. There are few surgeons specifically trained to treat these fractures, and there are no national or international guidelines. Management depends on several factors, including the child's age and the location of the fracture. Treatment options include functional treatment, closed reduction with or without immobilization, and surgery. The aim of this study was to assess the current management strategies for pediatric mandibular fractures in F

Characteristics and Treatment of Isolated Pediatric Olecranon Fractures.

BACKGROUND: Olecranon fractures are uncommon injuries in the pediatric population. Minimally displaced olecranon fractures are treated nonoperatively, but there is no consensus on a cutoff point for displacement requiring surgical treatment. The aim of this study is to report a large cohort of children with olecranon fractures and to identify patient and radiographic factors associated with favorable outcomes after treatment. METHODS: A retrospective review was conducted on patients aged 0 to 15 years with olecranon fractures treated at a single institution between 2002 and 2023. Radiographic

Knee dislocations and associated fractures: risk factors for surgical reduction.

INTRODUCTION: Dissociation of the knee joint, or knee dislocations (KD), can lead to severe complications, often resulting in multiligament injuries. A subset of these injuries are irreducible by closed reduction and require open reduction. Identifying KDs that necessitate surgical intervention is crucial for optimal outcomes. While previous studies have explored various risk factors, the influence of associated fractures is less understood. MATERIALS AND METHODS: We queried the Trauma Quality Improvement Program (TQIP) database from 2017 to 2021, for non-congenital closed knee dislocations r
