[Cerebellar ataxia of early onset: clinical symptoms and MRI findings].

Eight cases of childhood cerebellar ataxia were reported. All these cases showed chronic cerebellar ataxia with early onset, and the other d … of medial sagittal slice of the cerebellum was reduced significantly in all cases and also that of pons was reduced in 5 cases. Different from typical adult onset spinocerebellar degenerations, most of the present cases have achieved slow developmental milestones and the clinical course was not progressive. Genetic factors are suspected in the pathoge …

Ondine's curse in listeria monocytogenes brain stem encephalitis.

A case of necrotizing brain stem encephalitis due to listeria monocytogenes is described in a 48-year-old man who had brain stem encephalitis of complicated course and with selective destruction of the vasomotoric and respiratory centers. He developed that very rare Ondine's curse syndrome, being only able to breathe when awake. The literature on Ondine's curse and brain stem encephalitis due to LM is reviewed. Brain stem encephalitis has a mortality near 100%. The only treatment for Ondine's curse is lifelong artificial ventilation.

Lateral pontine hemorrhage: reappraisal of benign cases.

We reviewed 6 computed tomography-documented cases of primary lateral pontine hemorrhage that occurred in our two institutions over a 2-year period. All patients survived the acute stroke, with excellent functional recovery in 4. The level of consciousness of the patient and the size of the hemorrhage had no consistent bearing on outcome. Both of these features have been considered important prognostic indicators in patients with pontine hemorrhage, but in our series benign outcomes were not restricted only to patients who were alert or had small hemorrhages; on the other hand, severe disabili

Increased ponto-geniculo-occipital (PGO) wave frequency following central administration of neostigmine.

In all mammals so far investigated the occurrence of ponto-geniculo-occipital (PGO) waves precedes the onset and maintenance of desynchronized (D) sleep. As unitary electrographic events, PGO waves provide an index for quantitative evaluation of physiological D sleep or the D sleep-like state evoked by centrally administered acetylcholinesterase inhibitors. The present study characterized PGO wave frequency and time course following central administration of neostigmine bromide (Neo). The results show that Neo produced a dose-dependent increase in PGO wave frequency and time course when inject

Typical and reverse bobbing: a case with localizing value.

Report of a case of typical bobbing occasionally interpolated by reverse bobbing. The causative lesion, which lay in the most dorsal median portion of the pontine tegmentum, was a small primary hemorrhage, diagnosed in life by CT scanning and confirmed at necropsy. The pathological and clinical correlations of the phenomenon are discussed.

Central pontine myelinolysis with widespread extrapontine lesions: a report of two cases.

Two cases of central pontine myelinolysis with extensive extrapontine lesions are described. Both occurred in patients with longstanding quadriplegia in whom severe hyponatraemia, occurring in a setting of respiratory infection, was rapidly corrected with hypertonic saline. The extrapontine lesions resembled those described in extrapontine myelinolysis, but the possibility that an anoxic-ischaemic component may have contributed is discussed. The clinical data in these two cases are consistent with the mounting body of clinical and experimental evidence that disturbances of sodium balance may b

Localization of immunoreactive thyrotropin releasing hormone in the lower brainstem of the rat.

The distribution of thyrotropin releasing hormone (TRH) in individual nuclei of the rat brainstem was examined by radioimmunoassay. TRH was detectable in 36 of 40 brainstem nuclei investigated and the localization of TRH in the brainstem was unlike other known brainstem neuropeptides. By far the highest concentration of TRH in the brainstem (1.2 ng/mg protein) is present in the nucleus of the solitary tract. The concentration of TRH was relatively high in the motor nuclei of the IIIrd, Vth, VIIth, Xth and XIIth cranial nerves, and less high in the area postrema, nucleus gracilis, locus coerule

Spontaneous brain hemorrhage.

Spontaneous brain hemorrhage accounts for about 10% of all strokes and is fatal in about 50% of the cases. Its incidence, in contrast to other types of strokes, has not declined. Hypertension accounts for about half of these hemorrhages; the rest are due to tumors, aneurysms and vascular malformations, inflammatory and degenerative vasculopathies and hematologic and iatrogenic disorders of coagulation. In some patients no cause is ever found. Hypertensive brain hemorrhage occurs in the deep gray nuclei of the hemispheres, the cerebellum, and the pons and results in specific clinical syndromes …

Influence of neonatal cochlear removal on the development of mouse cochlear nucleus: I. Number, size, and density of its neurons.

Right cochleae were aspirated from 6-day-old mice to determine the influence of cochlear integrity on the development of cochlear nucleus (CN). At 45 days of age, cochlear destruction was confirmed histologically, and the CN of unilaterally deafferented and control animals were analyzed morphometrically. The molecular, fusiform, and polymorphic layers of deafferented dorsal CN were reduced in volume, and the polymorphic layer neurons were fewer, smaller, and less dense. The octopus and multipolar cells regions in deafferented ventral CN (VCN) were smaller, and their neurons were fewer, smaller

Experimental Biot periodic breathing in cats: effects of changes in PiO2 and PiCO2.

Periodic breathing of the Biot or cluster type was induced in spontaneously breathing, pentobarbital anesthetized cats by placing bilateral lesions within the pneumotaxic system of the rostral pons. Control lesions positioned outside of the critical nuclei never resulted in Biot breathing. The periodic pattern was characterized by clusters of breaths which were separated by distinct periods of apnea and was clearly not of Cheyne-Stokes quality. Test gas challenges inducing hypoxia and hypercapnia tended to diminish the apneic breatholds, whereas hyperoxia potentiated the periodic breathing by …

Primary pontine hemorrhage: clinicopathological correlations.

In 18 autopsies from patients with primary pontine hemorrhage we studied the sites of bleeding, the volumes and development of hematomas and clinicopathological correlations. A modular optical electronic planimeter was introduced to measure the size of hematomas. The series of patients can be divided into 2 groups from the viewpoint of bleeding sites, their development and clinical symptomatology. These are 1) the tegmentobasilar type and 2) the tegmental type. The precise location of the origin of hemorrhage, and the approximate volume of hematomas can now be determined with the help of compu

Computerized tomography in central pontine myelinolysis.

We describe an autopsy-proven case of central pontine myelinolysis (CPM) with premortem computerized tomographic (CT) visualization of the lesion on two scans, performed with an interval of 2 weeks. This case demonstrates the capability of CT to support the clinical diagnosis of central pontine myelinolysis. Identification of the condition should facilitate prompt initiation of aggressive supportive care.

"Ping-pong" gaze. Periodic alternating gaze deviation.

A clinicopathologic study of patient with periodic alternating gaze deviation is presented. The patient's eye moved rhythmically and conjugately from one extreme lateral position to the other. A 0.45-cm hemorrhage, found in the midline deep vermis, is thought to have disrupted the neural integrator that holds positions of gaze, resulting in periodic alternating gaze deviation. Three previous cases of periodic alternating gaze deviation are reviewed and illustrate the role of the cerebellum in the control of eye movements.

Two patients with central pontine myelinolysis are described. Both were middle aged women presenting with a history of protracted vomiting a … m. Death followed and histopathological examination confirmed classical myelinolysis in the central pons and extensive similar, though not identical, lesions in the cerebral hemispheres in both cases. The pathophysiological basis of the lesions is likely to be a special metabolic susceptibility of oligodendroglial cells in areas where neurones, glial c …
