Death of a classified worker probably caused by overexposure to gamma radiation.

This paper describes the case of an industrial radiographer who was seriously overexposed to gamma radiation. The exact circumstances of this exposure were not established but it was concluded that he was repeatedly irradiated probably to a total average whole body dose of at least 10 Gy over several years. Also, a much larger dose to a hand required its partial amputation. He developed myelodysplasia, which progressed to acute myeloid leukaemia from which he died. Karyotypic examination of the leukaemic blasts showed changes very similar to those associated with secondary leukaemia that may d

Anti-inflammatory effects of eicosapentaenoic acid on experimental skin inflammation models.

Anti-inflammatory effects of eicosapentaenoic (EPA) and docosahexaenoic acids (DHA) were examined on three models of skin inflammation induced in mice by topical application of an arachidonic acid (AA) solution, ultraviolet-B (UVB) irradiation, and contact sensitization with dinitrofluorobenzene. Ear oedema reactions induced by AA and UVB irradiation were significantly suppressed in mice fed a daily dose of 300 mg/kg EPA for 2 weeks. The contact hypersensitivity reaction was not impaired by EPA. None of the skin reactions was significantly inhibited in mice fed DHA or safflower oil. The result

Intensity and area increase of UVB-induced erythema: two variables used for studies of the influence of topically applied drugs.

The area of UV erythema produced by a small beam head was found to increase with increasing doses. The aim was to investigate whether measurement of the area could be a more useful indicator of UV-induced damage than classic visual grading. Topical pretreatment with all-trans retinoic acid (tretinoin) and betamethasone valerate was used to test the applicability of the method in pharmacological studies. We used a round outlet head (5 mm2) connected by optical fibre to a monochromatic irradiator, and doses ranging from 0.05 to 0.2 Joule of 300 mm UV light were applied to the skin of 6 healthy s

[Anastomosis of free flaps to the inferior epigastric artery].

The inferior epigastric vessels are a reliable and easily accessible vascular pedicle for free tissue transplantation to the perineum, genital area, and the abdominal wall. Two cases serve to illustrate the procedure: the first involves a post-radiation ulcer following recurring carcinoma of the vulva with loss of the symphysis; the second involves a reconstruction of the abdominal wall employing a two-fold latissimus dorsi flap in a patient with malignant fibrous histiocytoma.

Dose fractionation and regeneration in radiotherapy for cancer of the oral cavity and oropharynx. Part 2. Normal tissue responses: acute and late effects.

The early responses of normal tissues of the oral cavity and oropharynx in 498 patients, and the slowly-developing responses in 268 patients who survived a minimum of 18 months after radiotherapy for squamous cell carcinoma were analyzed. The severity of acute responses correlated with dose intensity. The incidence of severe late responses increased with increase in dose per fraction and was characterized by a low alpha/beta ratio. Severe late responses were significantly associated with severe acute responses independently of dose per fraction and total dose, and were also ameliorated slightl

[Comparative evaluation of the treatment of radiation skin injuries with oxycort ointment and Peruvian balsam].

The regenerating action of Peruvian balm and oxycort was compared in experiments on rats with experimental radiation skin injuries. Radiation skin ulcers entailed severe morphological and biochemical abnormalities, there was also observed an unstable healing provided the treatment was not applied. Oxycort ointment slightly reduced the period of healing. However, formation of secondary microdefects was conducive to recurrent ulcer in the future. Peruvian balm exerted a pronounced bactericidal action, strongly intensified the reparative processes, and, therefore, reduced the period of healing an

Surgical problems in the excision and repair of radiated tissue.

Surgical procedures for tissues damaged by radiation are fraught with the possibility of multiple and serious complications. A knowledge of the altered pathophysiology of such tissues (chiefly diminished blood supply, fibrosis, and direct cellular reparative potential) is necessary for the surgeon to make a proper judgment. Recognition of the pathognomic signs in the skin is important. The possibility of malignant change must be kept in mind. Surgical treatment must include the excision of radiated tissues back to good blood supply, and then proper and adequate coverage by the simplest and bes

The toxicity of hybrid techniques combining hypofractionated whole breast radiotherapy with concomitant tumor bed boost in patients with breast cancer.

BACKGROUND: We examined the acute toxicity of hypofractionated breast radiotherapy (HFRT) with hybrid techniques combining field-in-field intensity-modulated RT (FIF-IMRT) to the whole breast (WB) with inverse-IMRT for concomitant boost (CB) to the tumor bed. METHODS: This prospective exploratory study enrolled patients with early-stage breast cancer (EBC) or breast ductal carcinoma in situ (DCIS) from January 2020 after breast-conserving surgery. All patients consistently received a 16-fraction hybrid 40 Gy FIF-IMRT plans for WB, CB 8 Gy inverse-IMRT to the tumor bed (hybrid-CB group). Ident

Is there an impact of autoimmune rheumatological diseases on cutaneous toxicity in breast cancer adjuvant radiotherapy? A mono-institutional experience.

AIMS: Autoimmune rheumatological diseases (ARDs) have historically represented an absolute or relative contraindication for radiotherapy (RT) due to increased RT-related toxicity and the potential exacerbation of rheumatologic disease. ARDs are more frequent in females (F:M 4:1). Breast cancer (BC) is the most common malignancy, accounting alone for 31% of female cancers. This study compared acute and late cutaneous toxicity in ARDs and non-ARDs population undergoing adjuvant breast RT. METHODS: Data of patients with BC and ARDs treated between 2013 and 2023 were retrospectively reviewed. The

Heart-sparing radiotherapy with three-dimensional printing technology after mastectomy for patients with left breast cancer.

The purpose of this study was to analyze the effectiveness of electron beam therapy (EBT) with patient-tailored bolus (PTB) using three-dimensional printing technology to reduce heart and lung doses during post-mastectomy radiotherapy (PMRT). For 28 patients with left breast cancer, we designed customized virtual bolus for PMRT to compensate for surface irregularities on computed tomography images and developed optimized plans for EBT. As comparison between the PTB and tangential plans, the PTB plan reduced unnecessary exposure to heart and ipsilateral lung with better target coverage compared

Differentiation of irradiation and cetuximab induced skin reactions in patients with locally advanced head and neck cancer undergoing radioimmunotherapy: the HICARE protocol (head and neck cancer: immunochemo and radiotherapy with erbitux) - a multicenter phase IV trial.

BACKGROUND: In order to improve the clinical outcome of patients with locally advanced squamous cell carcinoma of the head and neck (LASCCHN) not being capable to receive platinum-based chemoradiation, radiotherapy can be intensified by addition of cetuximab, a monoclonal antibody that blocks the epidermal growth factor receptor (EGFR). The radioimmunotherapy with cetuximab is a feasible treatment option showing a favourable toxicity profile. The most frequent side effect of radiotherapy is radiation dermatitis, the most common side effect of treatment with cetuximab is acneiform rash. Inciden

A UVB wavelength dependency for local suppression of recall immunity in humans demonstrates a peak at 300 nm.

UVB radiation is a potent environmental carcinogen that not only causes mutations in the skin but also profoundly suppresses skin immune responses. Although this UVB-induced suppression of antitumor immunity has a key role in skin cancer development, the wavelengths within UVB causing greatest in vivo immunosuppression in humans are as yet unknown. We have identified a wavelength dependency for immunosuppression in humans across the UVB spectrum. We established linear dose-response curves for UV-induced local suppression of recall contact hypersensitivity responses at four wavelengths between

Unexpected toxicity in a patient treated with 3D conformal accelerated partial breast radiotherapy.

INTRODUCTION: With the publication of several trials demonstrating equivalence between partial breast irradiation and whole breast irradiation, the popularity of partial breast radiation has increased. Methods of delivery include interstitial brachytherapy, intracavitary brachytherapy, and 3D conformal irradiation, although the least amount of data exists on 3D conformal. Despite the relative paucity of data on 3D conformal, it is the most common method delivered on the National Surgical Adjuvant Breast and Bowel Project (NSABP) B-39 trial. To date, minimal toxicities and good cosmetic results
