We report a case of trapezoid nonunion diagnosed 3 months post injury. A 25-year-old male patient felt continuous wrist pain after being injured. His radiograph showed non-union with sclerotic change and bone resorption. He was planned for surgery, and this was done using a minimally invasive technique. Via a dorsal approach, we percutaneously drilled to the nonunion site and freshened the fracture ends. Bone plugs were harvested from the iliac crest using a bone marrow biopsy needle and packed into the nonunion site. The fracture was then fixed with a cannulated headless compression screw. Th

We investigated the effect of an interfragmentary gap on the final compression force using the Acutrak 2 Mini headless compression screw (length 26 mm) (Acumed, Hillsboro, OR, USA). Two blocks of solid rigid polyurethane foam in a custom jig were separated by spacers of varying thickness (1.0, 1.5, 2.0 and 2.5 mm) to simulate an interfragmentary gap. The spacers were removed before full insertion of the screw and the compression force was measured when the screw was buried 2 mm below the surface of the upper block. Gaps of 1.5 mm and 2.0 mm resulted in significantly decreased compression force

Arthroscopy allows the surgeon to use smaller incisions and still have predictable outcomes in the treatment of scaphoid fractures. Similar to large joint arthroscopy, the ability to visualize the fracture site allows one to not only fine tune the reduction and to assess the vascularity of the fracture fragments but to evaluate and treat any associated soft tissue injuries that may affect the end result. Specialized equipment and a basic knowledge of wrist arthroscopy however are required.

Volar perilunate fracture dislocation is an extremely rare carpal injury, but associated with metacarpophalangeal joint dislocation of both the ipsilateral index and middle finger has never been reported. We report one case of a 28-year-old man following a high-energy trauma. After performing closed reduction of the metacarpophalangeal joint injury, open reduction of the wrist injury through volar approach was done, and a K-wire fixation was used to stabilize the scaphoid fracture and lunotriquetral joint. After a 16 months?follow-up period, the wrist regained a full range of motion without sy

Various classifications of scaphoid fractures have been based on plain radiography, but there are difficulties in defining the actual fracture line without an appreciation of the three-dimensional anatomy. Radiological fracture lines were therefore mapped on transparent methylmethacrylate models of the bone. An analysis of 91 acute fractures showed that 11 were apparently incomplete. The other 80 showed three basic anatomical patterns: transverse through the waist, oblique in the plane of the dorsal sulcus, or of the proximal pole. There was some variation and comminution in these patterns, bu

A relatively high failure rate is associated with bone grafting with or without screw fixation in scaphoid nonunions. Complicating factors include avascular necrosis, cystic degeneration, and osseous size discrepancy or compromise. The Ender blade plate is suitable for adding stability in these cases, in spite of the necessity for late removal and the possibility of articular impingement. Twenty patients were treated with this technique. Nine of these patients had had previous unsuccessful attempts at nonunion repair. Nineteen had satisfactory results, but one had a persistent nonunion. The us

Pediatric carpal injury is unusual. Because of its rarity and imaging difficulties, the diagnosis is often delayed. Open reduction is advocated for displaced injuries. Pin fixation provides temporary stabilization of displaced injuries without permanently compromising joint motion. In older children, intercarpal fusion may be elected for treatment of intercarpal instability.

A 40-year-old woman presented with Madelung deformity and severe arthritic changes at both the radiocarpal and the distal radioulnar joints. She was treated by using her lunate as an osteochondral graft into the radius allowing reconstruction of the lunate fossa. The scaphoid and triquetrum were removed concomitantly and a Sauve-Kapandji procedure was performed. Complete bone healing was achieved. Ten years later, an excellent functional result was maintained, with a pain-free wrist, an acceptable wrist joint range of motion, as well as a favorable aesthetic appearance. This procedure may be i

Isolated fractures of the trapezoid are rare. Of all of the fractures of the carpus, the most common is the scaphoid, which represents 68.2% of all carpal fractures (Papp, Orthop Clin N Am 38:251-60, 2007). In contrast, trapezoid fractures represent 0.4% of all carpal injuries. Because it is an infrequently fractured carpal bone, there is a wide variety of treatment plans, including rest, immobilization, and surgery (Green and Pederson, Green's Operative Hand Surgery, Elsevier, Philadelphia, 759 pp, 2005). In this report, we describe a patient who presented with an isolated fracture of the tra

A retrospective study was undertaken of 22 carpal scaphoid fracture patients younger than 18 years. Of these, 17 patients received conservative treatment, and 5 received treatment involving screw fixation with or without a bone graft from the iliac crest. Clinical outcomes were evaluated using the Cooney score. Radiographs were analyzed with respect to the site of the fracture, the course of the fracture gap, displacement, and time to union. According to the Cooney score, in 94% of patients, good or excellent results were obtained. A total of 94% of patients considered their result as good or

Isolated fractures of the capitate, though rare, are probably often missed entirely. As in scaphoid fractures, repeated radiographs, tomography, or bone scanning are necessary for the diagnosis. Reviewing the literature, we found that most capitate fractures reported were transverse or combined with other carpal injuries, including the scapho-capitate fracture syndrome. Our patient presented an isolated oblique intraarticular fracture of the capitate. An old fracture of the styloid process of the ulna was seen at the primary examination. This special type of capitate fracture has not been prev

Between 1984 and 1993, 131 scaphoid nonunions were treated with iliac crest bone grafting and the Herbert-screw. 105 patients were followed between one and ten years with an average followup period of 4.5 years. Bony union was achieved in 93 patients with improvement of range of motion and grip strength. In 14 patients with DISI-instability, the angulation deformity was incompletely corrected with an average reduction of the scapho-lunate angle of eleven degrees. Limited athritic changes at the radial styloid were noted in 58%, intercarpal in 25% of the patients. Most frequent reasons for pers

Treatment of the destroyed scaphotrapezial (ST) joint in young patients is challenging due to the limited available options as they might not tolerate the stiffness, loss of grip strength and long-term complications of partial fusions, bone resections, and interposition implants or arthroplasties. This report presents the results of a free osteocutaneous joint transfer used to reconstruct the ST joint in a young patient suffering from ST destruction and carpal collapse. This 34-year-old patient was previously treated for a right scaphoid nonunion with distal scaphoid excision and interposition

We present an unusual case involving the simultaneous dislocation of the trans-scapho-retro-lunate in the right wrist and a posterolateral dislocation in the right elbow joint with a distal radius fracture in a 23-year-old male with no notable medical history. These injuries occurred when he fell from a height of around 12 meters onto his outstretched right hand. The patient presented to the hospital in a normal upper limb trauma position with no discomfort to critical functions. Predominant symptoms at the emergency department were discomfort in the right wrist, hand, and ipsilateral elbow, a

OBJECTIVE: To determine whether trapezioscaphoid (TS) joint alterations are associated with calcium pyrophosphate dihydrate (CPPD) crystal deposition disease and, if so, to determine the nature of these alterations.
METHODS: Radiographs of 160 wrists with evidence of chondrocalcinosis were evaluated with regard to TS joint abnormalities, and findings were compared with a similar number of radiographs in an age and sex matched control population in whom no evidence of chondrocalcinosis or other calcification in the wrist was seen. Two radiologists in consensus recorded radiographic findings in

Regardless of the treatment used, 25 to 45% of scaphoid fractures do not heal. Open surgery compromises vascularization and destabilizes ligament attachments. The aim of this retrospective study was to assess the value of retrograde percutaneous screw fixation of Alnot stage IIA and IIB scaphoid non-union in Schernberg zones 2 to 4. This series included 38 patients with a mean age of 31 years. Based on the Alnot classification, there were 16 stage IIA non-unions (12 in zone 3 and 4 in zone 2 according to the Schernberg classification) and 22 stage IIB non-unions (9 in zone 3 and 13 in zone 2).

Caring for hand and wrist injuries in the elite athlete brings distinct challenges, with case-by-case decisions regarding surgical intervention and return-to-play. Metacarpal fractures, thumb ulnar collateral ligament tears, and scaphoid fractures are common upper-extremity injuries in the elite athlete that can be detrimental to playing time and future participation. Treatment should therefore endure the demand of accelerated rehabilitation and return-to-activity without compromising long-term outcomes. Fortunately, the literature has supported emerging management options that support goals s
