Development of scales to screen for eight common psychiatric disorders.

The present study was designed to create a group of scales from the items on the Symptom Checklist 90 (SCL-90) to identify common psychiatric diagnoses. Subjects were 1457 adult psychiatric outpatients who completed the Symptom Checklist-90 and a structured diagnostic interview at the time of their initial evaluation. A combination of rational and empirical test construction methods was used to create the SCL-90 Diagnostic Scales, item sets that identify eight common psychiatric conditions: major depression, bipolar disorder, schizophrenia, antisocial personality disorder, somatization disorde

Targeted online health information was associated with more severe temporomandibular disorders.

BACKGROUND: Temporomandibular disorders (TMDs) are typical biopsychosocial conditions often accompanied by anxiety, somatization and even cyberchondria. Targeted online health information is increasingly prominent in digital era yet its psychological impact on TMDs remains underexplored. To examine the association between targeted online health information and TMDs severity, and to explore whether anxiety, cyberchondria and somatization mediate this relationship. METHODS: Participants were evaluated using questionnaires including the five TMDs symptoms (TMDs-5T) scale, 7-item Generalised Anxi

A Novel Screening Tool for Assessing Child Abuse: The Medical Somatic Dissociation Questionnaire-MSDQ.

Somatic dissociation is known to be associated with childhood abuse, particularly with childhood sexual abuse (CSA). Currently, the diagnosis of CSA is hampered by the lack of a validated questionnaire. While some questionnaires are excellent research tools, there is no suitable applied measure for the assessment of distress due to CSA. The current study's objective was to validate a novel questionnaire, designated the Medical Somatic Dissociation Questionnaire-MSDQ, for evaluating somatic dissociation in the healthcare system setting. A total of 541 adults, 160 (30%) male and 381 (70%) female

Early maladaptive schemas in depression and somatization disorder.

Cognitive theories of depression posit that early maladaptive schemas (EMSs) are key vulnerability factors for psychological disorders. In this study, we investigated specific EMSs as shared or distinct cognitive vulnerability factors for depression and somatization disorder. The sample consisted of patients with Major depressive disorder (N = 30) and Somatization disorder (N = 30) from a community hospital or a psychiatric clinic. Participants completed the Structured Clinical Interview for DSM-IV (SCID), the Beck Depression Inventory-II (BDI-II), and the short form of the Young Schema Questi

The association of generalized anxiety disorder and Somatic Symptoms with frequent attendance to health care services: A cross-sectional study from the Northern Finland Birth Cohort 1966.

Objective Generalized anxiety disorder is associated with higher rate of physical comorbities, unexplained symptoms, and health care utilization. However, the role of somatic symptoms in determining health care utilization is unclear. The present study aims to assess the association of frequent attendance of health care services between generalized anxiety disorder symptoms and somatic symptoms. Method This study was conducted cross-sectionally using the material of the 46-year follow-up survey of the Northern Finland Birth Cohort 1966. Altogether, 5585 cohort members responded to the question

Repetitive transcranial magnetic stimulation plus standardized suggestion of benefit for functional movement disorders: an open label case series.

BACKGROUND: We studied suggestion of benefit combined with motor cortex and premotor cortex repetitive transcranial magnetic stimulation (rTMS) in chronic (>2 years) FMDs. METHODS: Patients were identified from our patient records who had clinically definite FMDs and had undergone neuropsychiatric evaluation. Those with chronic FMDs were offered open-label rTMS over the dominant motor cortex. If they failed to improve they received dominant premotor cortex rTMS. The primary outcome was change from baseline to post-rTMS in quality of life measured by the World Health Organization Quality of Li

Do psychiatric disorders first appear preinjury or postinjury in chronic disabling occupational spinal disorders?

STUDY DESIGN: An epidemiologic prevalence study. OBJECTIVES: To clarify the temporal association between work-related injury claims and psychiatric disorders in patients with chronic disabling occupational spinal disorders (CDOSDs). SUMMARY OF BACKGROUND DATA: Few empirical data are available regarding the "chicken-or-egg" question of which occurs first: the injury or incident culminating in the painful CDOSD or the psychiatric disturbance. METHODS: Subjects attended a tertiary interdisciplinary rehabilitation program. Psychiatric disorders were assessed using the Structured Clinical Interv

Risk factors for psychogenic nonepileptic seizures in children and adolescents with epilepsy.

There is evidence that psychogenic nonepileptic seizures (PNES) remain underdiagnosed, especially in children and adolescents. Diagnosis of such events is even more difficult in patients that do have epilepsy, leading to delayed diagnosis and treatment and, consequently, iatrogenic complications. This study aimed to evaluate possible risk factors in children with epilepsy who had PNES. Seizures and epileptic syndromes were classified according to International League Against Epilepsy guidelines. Patients were evaluated with a structured psychiatric anamnesis and classified according to the Dia

[Functional heart pain].

Functional heart symptoms, especially chest pain, are very widespread and, according to the International Classification of Diseases (ICD-10), are described as "somatoform autonomous functional disorders of the cardiovascular system". Although they are very often accompanied by considerable anxiety about having a heart attack, for example, they are initially not recognizable as such and have to be distinguished from somatic complaints. The most prevalent of these symptoms (Table 2) are chest pains, followed by feelings of weakness, a tendency to become easily fatigued and breathing difficultie

[Sensory sensitization, part II: Pathophysiology in dysfunctional disorders. Understanding the inner life of the nerve pathways may explain hitherto unexplainable symptoms].

This article is based on a vast clinical experience from patients presenting with widespread pain syndromes as well as dysfunctional symptoms from inner organs. A literature survey has been performed. Allodynia and hyperalgesia that partly explain the fibromyalgia and local myalgia syndromes seem to arise from a pathophysiological process of nociceptive sensitisation. It is proposed that the concept of "sensory sensitisation dysfunctional disorders" be applied to conditions like bronchial hyperreactivity, Da Costas syndrome, Dercum's disease (Adipositas dolorosa), dry eyes and mouth syndrome,

Polysymptomatic conversion disorder in childhood and adolescence in Japan. Early manifestation or incomplete form of somatization disorder?

We recently demonstrated that the orphan nuclear receptor testicular receptor 4 (TR4) is a potent regulator of corticotroph tumor growth and hormone secretion. The Ras/Raf/MEK/ERK pathway is commonly overactivated in human tumors and we have demonstrated that corticotroph tumor TR4 is activated by ERK1/2-mediated phosphorylation. We evaluated effects of MEK-162, a selective, non-ATP-competitive allosteric inhibitor of MEK1/2, on murine and human in vitro and in vivo corticotroph tumor proliferation and adrenocorticotrophic hormone (ACTH) secretion. MEK-162 treatment dose-dependently inhibited

Advances in behavioral-cognitive therapy of social phobia.

Behavioral-cognitive therapy is a cost-effective treatment for social phobia. The doctor's role is to teach the patient how to do successful self-exposure. The clinician acts as a guide and monitor; there is no need to waste time accompanying the patient into the phobic situation. The patient first reads a self-exposure manual to learn how to confront panic-evoking social cues for prolonged period without avoidance until habituation sets in. This might require an hour daily of self-exposure over weeks or months. As patients habituate to social cues to which they have exposed themselves, they a

Predicting personality in adulthood from college MMPI scores: implications for follow-up studies in psychosomatic medicine.

To assess the long-term predictive utility of Minnesota Multiphasic Personality Inventory (MMPI) content scales, 1,960 individuals who had completed the MMPI in college in 1964 or 1965 were administered two measures of adult personality, the NEO Personality Inventory (NEO-PI) and the Cook and Medley MMPI Hostility scale, in 1988. A comparison group of 274 men and women in the Baltimore Longitudinal Study of Aging were given both MMPI and NEO-PI between 1981 and 1987. Predictive correlations between MMPI scales and NEO-PI factors were qualitatively similar to concurrent correlations, but approx

Validation of the Hierarchical Taxonomy of Psychopathology-Self-Report (HiTOP-SR): Internal structure and construct validity against the MMPI-3 in a community sample.

The present study aimed to evaluate the hierarchical structure and construct validity of the Hierarchical Taxonomy of Psychopathology-Self-Report (HiTOP-SR) measure. Using data from a combined sample of 775 community and undergraduate participants, exploratory factor analysis initially yielded a seven-factor solution reflecting broad psychopathology domains: internalizing, disinhibited externalizing, antagonistic externalizing, low positive emotionality, eating pathology, sexual dysfunction, and anankastia. Subsequent elaboration of a lower order structure based on the first three broader doma

Idiopathic anaphylaxis.

Idiopathic anaphylaxis is not uncommon and can be found in as many as 60% of patients referred to an allergist for anaphylactic events. It is an extremely frustrating condition for patients because they have no means of avoiding a known allergen and events occur spontaneously and unprovoked. The objective of this study was to synthesize and review the medical literature on idiopathic anaphylaxis and present a strategy for diagnosis, prevention, and treatment of episodes. A literature review was performed after a search of PubMed using the key words of idiopathic anaphylaxis. Idiopathic anaphyl
