Rate and loudness manipulations in dysarthria: acoustic and perceptual findings.

Both rate reduction and increased loudness reportedly are associated with an increase in the size of the articulatory-acoustic working space and improved acoustic distinctiveness for speakers with dysarthria. Improved intelligibility also has been reported. Few studies have directly compared rate and loudness effects for speakers with dysarthria, however, although rate reduction and increasing vocal loudness are common treatment techniques. In the current study, 15 speakers with dysarthria secondary to multiple sclerosis, 12 speakers with dysarthria secondary to Parkinson's disease (PD), and 1

[Pseudoglottis after laryngeal trauma with bilateral recurrent laryngeal nerve paralysis].

Voice is produced by a steady flow of air from the lungs, which is segmented at the laryngeal level into a series of air puffs. The segmentation is facilitated by periodic vocal fold vibrations under physiological conditions. In some cases, as in partial laryngectomy, segmentation of air stream is caused by vibrations of the false vocal folds. Here we report a case of an air stream segmentation mechanism at the laryngeal level producing an acceptable voice quality after fracture of the larynx and bilateral vocal fold paralysis. A lateral fixation of the left vocal fold was performed because of

Speech and swallowing function after anterior tongue and floor of mouth resection with distal flap reconstruction.

The purpose of this study was to assess the postoperative functioning of oral cancer patients with resections of the anterior tongue and floor of mouth, reconstructed with distal flap closure. Speech and swallowing performance was assessed for 11 men and 5 women preoperatively and at 1 and 3 months postoperatively following a standardized protocol. Speech tasks included an audio recording of a brief conversation and of a standard articulation test; swallowing function was examined using videofluoroscopy. Data were also collected on the number and duration of speech/swallowing treatment session

Speech results of the Zürich approach in the treatment of unilateral cleft lip and palate.

The purpose of this study was to describe perceptually the speech articulation, voice quality, and velopharyngeal competency of subjects with complete unilateral cleft lip and palate treated by the Zürich approach. The mean age of the 37 subjects was 10.5 years. Although only one subject had had secondary palatal management, no subject was rated as exhibiting a severe articulation or nasality problem. Subjects were rated as exhibiting adequate to marginal velopharyngeal competency 94.5 percent of the time, and the incidence of compensatory articulation errors was low. In comparison with other

Central auditory processing skills in stuttering school-age children with and without dyslexia: a preliminary study.

BACKGROUND & AIM: Central auditory processing disorder (CAPD) has been linked separately to developmental stuttering (DS) and developmental … hotic digits test (DDT), binaural fusion (BF), pitch pattern sequence (PPS) test, and the right ear speech intelligibility in noise (SPIN) tests. The PPS scores were significantly lower in the group with both disorders. There was a significant correlation between some behavioral CAP assessments (e.g., DDT, BF, and PPS) and quantitative stuttering measures in the group o …

Associated Factors With the Prosthetic Vocal Rehabilitation Outcome in Older Patients (Over 70 Years).

INTRODUCTION: Tracheoesophageal puncture (TEP) with voice prosthesis (VP) is considered the gold standard in vocal rehabilitation of total laryngectomized patients, for generating better speech intelligibility and good vocal result. The aspects of aging that may be related to the success of this method of rehabilitation are rarely discussed in the literature. OBJECTIVE: To describe the factors that influence the rehabilitation outcome of the total laryngectomized older patients with voice prosthesis. METHODS: A retrospective cohort study enrolled in the Head and Neck Cancer Surgery Section o…

Determinants of cognitive disorders in Autosomal Dominant Cerebellar Ataxia type 1.

We assessed neuropsychological performances of 22 patients affected by Autosomal Dominant Cerebellar Ataxia type 1. All subjects completed a comprehensive battery of standardized tests requiring a verbal response, without time constraints. In order to verify the hypothesis that disease severity is the major factor in determining the cognitive status in this syndrome, patients were divided into three groups according to the severity of the clinical picture, as evaluated by the Inherited Ataxias Progression Scale (IAPS). Statistical analysis of the three groups' raw scores showed a significant d

Stuttering candidate genes DRD2 but not SLC6A3 is associated with developmental dyslexia in Chinese population.

BACKGROUND: Dyslexia is a polygenic developmental disorder characterized by difficulties in reading and spelling despite normal intelligence, educational backgrounds and perception. Increasing evidences indicated that dyslexia may share similar genetic mechanisms with other speech and language disorders. We proposed that stuttering candidate genes, DRD2 and SLC6A3, might be associated with dyslexia. METHODS AND RESULTS: The study was conducted in an unrelated Chinese cohort with 502 dyslexic cases and 522 healthy controls. In total, 23 Tag SNPs covering the two genes were selected for genotyp

Fitzsimmons syndrome: spastic paraplegia, brachydactyly and cognitive impairment.

Fitzsimmons syndrome is an infrequently described entity comprising slowly progressive spastic paraplegia, brachydactyly, and cone-shaped epiphyses, dysarthria, and low-normal intelligence. Five patients with this syndrome have been reported. The cause remains unknown. Here we describe a 16-year-old boy with Fitzsimmons syndrome. He was noted to toe-walk at age 18 months and spasticity progressed slowly into a spastic gait with contractures. He has mild dysarthria and hypernasal speech. Brachydactyly is notable but cannot be classified into one of the recognized types. A cone-shaped epiphysis

Cognitive functioning in Lesch-Nyhan syndrome.

The present study represents the first effort to assess systematically the cognitive functioning of a population of individuals with Lesch-Nyhan syndrome using standardized psychometric instruments. Seven residents from a special hospital setting participated. They ranged in age from 10 years 1 month to 22 years 3 months (mean 13 years 7 months). Using the Stanford-Binet Intelligence Scale: IV, scores in each of the four domains assessed by this battery (Verbal Reasoning, Abstract/Visual Reasoning, Quantitative and Short-Term Memory), as well as the general composite score, ranged from moderat

Near-total laryngopharyngectomy with pectoralis major myocutaneous flap in advanced pyriform carcinoma.

Successful restoration of phonation, as well as swallowing in laryngopharyngeal surgery for patients with advanced pyriform carcinoma still remains a major challenge. In a five-year period, near-total laryngectomy operations were performed on 21 patients with laryngopharyngeal carcinoma. This report particularly emphasizes the value of near-total laryngopharyngectomy with a pectoralis major myocutaneous flap for four extensive hypopharyngeal carcinoma cases (three of which were of pyriform origin). All four patients regained intelligible shunt speech and smooth swallowing. The follow-up period

Monozygotic twins concordant for autism and hyperlexia.

The authors describe male monozygotic twins, Jon and Jay, who are concordant for autism and hyperlexia. Autism and mental retardation were diagnosed at the age of 2 years 5 months. Jay was the more advanced twin in motor co-ordination, attention span and receptive abilities, but had frequent tantrums. When psychologically assessed at 7 years 2 months, Jon showed borderline/severe mental retardation on the measure of non-verbal intelligence and Jay was moderately retarded. Their receptive language age was greater than their expressive language age: Jon's speech was less mature but more communic

Periventricular leukomalacia: ultrasonic diagnosis and neurological outcome.

The ultrasonographic findings and neurological development of 15 preterm infants, born between August 1981 and July 1984, who developed periventricular leukomalacia (P.V.L.) are described. The incidence of P.V.L. in infants with a birth weight less than 1 500 g was 2.3%. Nine of the 15 infants demonstrated areas of increased echogenicity in the periventricular regions prior to the development of cystic changes. The mean age for the appearance of cysts was 26 days. At follow-up 14 infants showed evidence of abnormal motor development and 8 infants had visual defects. All infants appear to have

Primary veloplasty: long-term results without maxillary deformity. a twenty-five year report.

Primary veloplasty during the first year of life enables normal growth of the upper jaw and of the facial skeleton in patients with clefts. In spite of the remaining cleft in the hard palate, intelligible speech can be learned. The closure of the residual cleft of the hard palate is generally postponed until the age of 12 to 14 years, when the normal growth of the jaw is virtually completed. This method has been used in the clinic by the author and his father for thirty years. The results after primary veloplasty for some hundred adult patients show normal maxillary and cranial growth both cli
