Vestibular function in degenerative parkinsonism.

INTRODUCTION: The degenerative pathology in Parkinson's disease (PD), multisystem atrophy (MSA) and progressive supranuclear palsy (PSP) have different spatial distribution in the brainstem. Our aim was find out in what way this pathology affects brainstem vestibular reflex pathways in each of these diseases. METHODS: We studied 30 patients: 15 with PD, 9 with PSP (Richardson syndrome) and 6 with MSA (cerebellar) and compared their results with results from 37 age-matched healthy controls (HC). Motor functions of the patient groups were evaluated with the Unified Parkinson's Disease Rating Sc

AZP2006 in Parkinson's disease models: A promising new neuroprotective treatment involving lysosomal homeostasis and the Progranulin/Prosaposin complex.

The pathohistological hallmarks of Parkinson's disease are aggregated alpha-synuclein and other aggregation-prone proteins (Lewy bodies). Recent studies show mechanistic as well as genetic connections between lysosomal dysfunction and Parkinson's disease pathology. A direct link between lysosomal function and Parkinson's disease might be the degradation of alpha-synuclein within the lysosomal system. Progranulin is necessary for maintaining lysosomal function, facilitating the activity of several lysosomal enzymes. Progranulin's exit from the endoplasmic reticulum and its lysosomal availabilit

When complexity does not pay: benchmarking deep learning and ensemble methods for biomarker discovery.

The integration of multi-omics data holds great promise for identifying robust and clinically relevant biomarkers, yet the increasing complexity of computational methods raises questions about their practical utility. In this study, we present a comprehensive benchmarking framework that evaluates 27 feature selection strategies and 11 predictive models across three real-world disease cohorts: Alzheimer's disease, progressive supranuclear palsy, and breast cancer. We compare traditional machine learning, ensemble-based methods, and state-of-the-art deep learning models in terms of predictive pe

Artificial microRNAs targeting tau enable post-symptomatic functional recovery in aged tauopathy mice.

Tauopathies are a group of neurodegenerative disorders, including Alzheimer's disease, frontotemporal dementia, and progressive supranuclear palsy, characterized by the pathological accumulation of tau protein. While tau reduction has emerged as a promising disease-modifying strategy, most preclinical studies have focused on preventive approaches, and the therapeutic potential after clinical onset remains largely unexplored. This limitation is critical, as patients are typically diagnosed after symptoms emerge. Furthermore, global tau suppression may disrupt physiological tau functions and lea

The triglyceride-glucose index in parkinsonian syndromes: a clinical dead end or a hint at central dysregulation?

AIM OF THE STUDY: This study assessed the triglyceride-glucose (TyG) index for differentiating between Parkinson's disease (PD), multiple system atrophy (MSA), and progressive supranuclear palsy (PSP), and its association with cognitive profiles and clinical status. MATERIAL AND METHODS: This retrospective cross-sectional study involved 329 patients (272 PD, 15 MSA, 42 PSP). Assessments included motor and non-motor symptoms, the TyG index, and metabolic profiles. RESULTS: The TyG index did not differentiate PD from AP; however, PSP patients exhibited significantly lower fasting glucose level

Classification of tauopathies from human brain homogenates through salt-modulated tau amplification.

INTRODUCTION: Tauopathies are a heterogeneous group of neurodegenerative disorders defined by abnormal aggregation of tau protein. Although cryogenic electron microscopy (cryo-EM) has uncovered disease-specific tau structures, translating these insights into diagnostic tools remains difficult. METHODS: We developed a heparin-free, salt-modulated real-time quaking-induced conversion (RT-QuIC) assay using K12 and K11 tau substrates, targeting aggregation-prone regions. This current method improves on previous methodology by minimising the number of required substrates by modulating reaction sal

Levodopa exposure and nigral neuroinflammation in parkinsonian disorders: A postmortem study of 63 cases.

Levodopa remains the cornerstone of treatment in Parkinson's disease (PD), but its long-term impact on neuroinflammation remains unclear, particularly in multiple system atrophy (MSA) and progressive supranuclear palsy (PSP), where levodopa efficacy is limited. This study examined whether chronic levodopa exposure is associated with neuroinflammation or dopaminergic neuronal loss in end-stage PD, MSA, and PSP. Postmortem midbrain tissue from 63 neuropathologically confirmed cases (PD: n = 38; PSP: n = 13; MSA: n = 12) was analyzed. Immunohistochemistry was used to quantify tyrosine hydroxylase

Imaging of Proteinopathies in the Brains of Parkinsonian Disorders.

Neurodegenerative diseases such as Alzheimer's disease (AD), frontotemporal lobar degeneration (FTLD), and α-synucleinopathies-including Parkinson's disease (PD), dementia with Lewy bodies (DLB), and multiple system atrophy (MSA)-are characterized by the accumulation of misfolded protein aggregates. Advances in positron emission tomography (PET) imaging have enabled in vivo visualization of these pathologies, particularly tau and α-synuclein fibrils, facilitating early diagnosis and differential classification. Tau PET tracers such as 18F-florzolotau have demonstrated robust imaging of both AD

Targeted degradation of SNCA/α-synuclein aggregates in neurodegeneration using the AUTOTAC chemical platform.

Parkinson disease (PD) characterized by dopaminergic neuronal loss is caused by aggregation of misfolded SNCA/α-synuclein. We recently developed autophagy-targeting chimera (AUTOTAC), a targeted protein degradation (TPD) technology based on the macroautophagy/autophagy-lysosome pathway (ALP). In this study, we employed AUTOTAC to synthesize ATC161, a chimeric compound that adopts Anle138b as target-binding ligand (TBL) for SNCA aggregates. The autophagy-targeting ligand (ATL) of ATC161 was designed to allosterically activate the autophagy receptor SQSTSM1/p62 (sequestosome 1), a key step for t

Neuroenergetic alterations in neurodegenerative diseases: A systematic review and meta-analysis of in vivo31P-MRS studies.

Phosphorus magnetic resonance spectroscopy (31P-MRS) is applied for non-invasive studies of neuroenergetic metabolism in neurodegenerative diseases. However, the findings are inconsistent and have not yet been tested in meta-analyses. To address this gap, we performed a systematic review of 29 studies and conducted meta-analyses for 9 studies on Alzheimer's disease (AD, n = 140 patients), 9 studies on Parkinson's disease (PD, n = 183 patients), 3 studies on Progressive Supranuclear Palsy (PSP, n = 42 patients), and 2 studies on Multiple System Atrophy (MSA, n = 24 patients). FTLD-MAPT mutations and short 5'UTR Tau mRNAs increase Tau translation.

Tau protein plays a central role in Alzheimer's disease (AD) and other tauopathies. Tau proteins aggregate in degenerating neurons and their expression level is critical for disease development and progression. In addition, the generation of truncated Tau species is involved in the pathological process. In the human brain, several N-terminally truncated Tau species (including Met11- and Met127-Tau) have been detected. However, both the mechanisms of their synthesis and their relevance for the diseases remain largely unclear. Several Tau messenger RNA (mRNA) isoforms with distinct 5' untranslat

Passive tau-based immunotherapy for tauopathies.

Tauopathies are heterogeneous clinicopathological entities characterized by abnormal neuronal and/or glial inclusions of the microtubule-binding protein tau. In secondary tauopathies, i.e., Alzheimer's disease (AD), tau deposition can be observed, but tau may coexist with another protein, i.e., amyloid-β. In the last 20 years, little progress has been made in developing disease-modifying drugs for primary and secondary tauopathies and available symptomatic drugs have limited efficacy. Treatments are being developed to interfere with the aggregation process or to promote the clearance of tau pr

Clinical Aspects of the Differential Diagnosis of Parkinson's Disease and Parkinsonism.

Parkinsonism is a clinical syndrome presenting with bradykinesia, tremor, rigidity, and postural instability. Nonmotor symptoms have recently been included in the parkinsonian syndrome, which was traditionally associated with motor symptoms only. Various pathologically distinct and unrelated diseases have the same clinical manifestations as parkinsonism or parkinsonian syndrome. The etiologies of parkinsonism are classified as neurodegenerative diseases related to the accumulation of toxic protein molecules or diseases that are not neurodegenerative. The former class includes Parkinson's disea

Neuroimaging in Parkinson's Disease: necessity or exaggeration?

INTRODUCTION: Neuroimaging plays an increasingly important role in the diagnosis of parkinsonian syndromes. AIM OF THE STUDY: In this paper, the authors elaborate on the necessity of using magnetic resonance imaging (MRI) in Parkinson's Disease (PD) and its potential role in differential diagnosis versus other neurodegenerative parkinsonian syndromes such as dementia with Lewy bodies, multiple system atrophy, progressive supranuclear palsy and corticobasal syndrome. STATE OF THE ART: The currently known characteristic abnormalities are listed and tabulated, current recommendations are summar

Predictors of survival in frontotemporal lobar degeneration syndromes.

After decades of research, large-scale clinical trials in patients diagnosed with frontotemporal lobar degeneration (FTLD) are now underway across multiple centres worldwide. As such, refining the determinants of survival in FTLD represents a timely and important challenge. Specifically, disease outcome measures need greater clarity of definition to enable accurate tracking of therapeutic interventions in both clinical and research settings. Multiple factors potentially determine survival, including the clinical phenotype at presentation; radiological patterns of atrophy including markers on b

P301 L, an FTDP-17 Mutant, Exhibits Enhanced Glycation in vitro.

BACKGROUND: Frontotemporal dementia and parkinsonism-linked to chromosome-17 are a group of diseases with tau mutations leading to primary tauopathies which include progressive supranuclear palsy, corticobasal syndrome, and frontotemporal lobar degeneration. Alzheimer's disease is a non-primary tauopathy, which displays tau neuropathology of excess tangle formation and accumulation. FTDP-17 mutations are responsible for early onset of AD, which can be attributed to compromised physiological functions due to the mutations. Tau is a microtubule-binding protein that secures the integrity of polym

How specific are non-motor symptoms in the prodrome of Parkinson's disease compared to other movement disorders?

The clinical diagnosis of Parkinson's disease (PD) based on motor signs is often preceded by several non-motor symptoms that can indicate early prodromal neurodegenerative processes. Such prodromal symptoms can aid the early detection of PD, but their specificity for prodromal PD in comparison to prodromes of other movement disorders is still largely unclear. We here aim to give a first insight into the published evidence of prodromal non-motor symptoms in PD, dementia with Lewy bodies (DLB), multiple system atrophy (MSA), Huntington's disease (HD), progressive supranuclear palsy (PSP) and spi

Can Autonomic Testing and Imaging Contribute to the Early Diagnosis of Multiple System Atrophy? A Systematic Review and Recommendations by the Movement Disorder Society Multiple System Atrophy Study Group.

BACKGROUND: In the current consensus diagnostic criteria, the diagnosis of probable multiple system atrophy (MSA) is based solely on clinical findings, whereas neuroimaging findings are listed as aid for the diagnosis of possible MSA. There are overlapping phenotypes between MSA-parkinsonian type and Parkinson's disease, progressive supranuclear palsy, and dementia with Lewy bodies, and between MSA-cerebellar type and sporadic adult-onset ataxia resulting in a significant diagnostic delay and misdiagnosis of MSA during life. OBJECTIVES: In light of an ongoing effort to revise the current cons

Wild-type Cu/Zn-superoxide dismutase is misfolded in cerebrospinal fluid of sporadic amyotrophic lateral sclerosis.

BACKGROUND: A subset of familial forms of amyotrophic lateral sclerosis (ALS) are caused by mutations in the gene coding Cu/Zn-superoxide dismutase (SOD1). Mutant SOD1 proteins are susceptible to misfolding and abnormally accumulated in spinal cord, which is most severely affected in ALS. It, however, remains quite controversial whether misfolding of wild-type SOD1 is involved in more prevalent sporadic ALS (sALS) cases without SOD1 mutations. METHODS: Cerebrospinal fluid (CSF) from patients including sALS as well as several other neurodegenerative diseases and non-neurodegenerative diseases
