Correction of the claw hand.

Intrinsic paralysis can be the manifestation of a variety of pathologic entities (stroke, cerebral palsy, Charcot-Marie-Tooth, muscular dystrophy, leprosy, trauma, cervical disease, and compressive and metabolic neuropathies). Patients present with a spectrum of clinical findings dependent on the cause and severity of the disease. The 3 main problems caused by intrinsic weakness of the fingers are clawing with loss of synchronistic finger flexion, inability to abduct/adduct the digits, and weakness of grip. Clawing is defined as hyperextension of the metacarpophalangeal joints and flexion of t

Muscle transpositions in the shoulder and upper arm for sequelae of brachial plexus palsy.

Permanent deficiency in abduction and external rotation of the arm seen after brachial plexus injury may be corrected by a levator scapulae transposition onto the supraspinatus, while the teres major, associated or not with latissimus dorsi, is transferred onto the infraspinatus muscle. Transposed in a bipolar manner, the latissimus may compensate the deltoid muscle. Associated measures have to be carried out in children with deformities caused by obstetrical palsy. A paralysed serratus anterior is best compensated by a transfer of the pectoralis major together with pectoralis minor muscle.

[Healing phases in autogenous posterior cruciate ligament replacement. A decision aid for after-care. A biomechanical and histologic study].

In a sheep model for reconstruction of the posterior cruciate ligament using the central one-third of the autogenous patellar tendon and immediate mobilization, the incorporation course was studied biomechanically, histomorphometrically and radiographically at eight different points in time up to 1 year after surgery. Four different phases of autograft healing were demonstrated on the basis of the histological events. The biomechanical data were correlated with the morphological findings. Phase 1--necrosis--was characterized by a maximum of necrotic autograft tissue 2 weeks postoperatively. In

Extensor hallucis longus transfer for hallux varus deformity.

The hallux varus deformity results from a dynamic imbalance of the tendons that cross the first metatarsophalangeal joint. This condition most frequently occurs after McBride-type operative treatment for hallux valgus. A new procedure, developed to correct this imbalance, involves transferring the extensor hallucis longus beneath the first intermetatarsal ligament into the base of the proximal phalanx, along with an arthrodesis of the first interphalangeal joint. This procedure in fifteen affected great toes gave over-all satisfactory correction of the hallux varus deformity.

Editorial Commentary: Arthroscopically Assisted Lower Trapezius Transfer: Promising Technique Still Searching for the Optimal Indications or Panacea for All Irreparable Rotator Cuff Tears?

Arthroscopic-assisted lower trapezius transfer (LTT) has gained in popularity as a technique to restore function and improve pain in large-to-massive irreparable posterosuperior rotator cuff tears. Surgeons who have adopted this technique have variable indications. While most agree that there is a limit of arthropathy, other factors appear to be contentious. Age, teres minor integrity, and degree of external rotation deficit remain debatable in terms of indications and contra-indications for LTT. The biomechanical narrative around LTT is sound and has been proven. Surgeons have recently raised

Cadaveric biomechanical study of the calcaneonavicular ligament in midfoot medial column collapse comparing two surgical techniques.

INTRODUCTION: The calcaneonavicular ligament (spring ligament) plays a fundamental role in calcaneonavicular static stability and medial longitudinal arch, injury which is related to flatfoot. OBJECTIVE: The primary objective was to compare the biomechanical behaviour of the spring ligament in a healthy foot and after section and repair with augmentation and transfer of the flexor digitorum longus (FDL). As secondary objectives we have the biomechanical comparison between isolated repair with augmentation associated or not with transfer. METHODS: This experimental biomechanical cadaver study

Clubfoot Activity and Recurrence Exercise Study (CARES).

BACKGROUND: Approximately half of treated clubfoot patients initially corrected with the Ponseti method experience relapse that requires additional treatment. The consequences of relapse on childhood activity levels have not been well studied. Ponseti noted lower functional ratings at 18-year follow-up in clubfoot patients who had undergone tibialis anterior tendon transfer for relapse. METHODS: Clubfoot Activity and Recurrence Exercise study (CARES) is an observational, prospective cohort study that compares physical activity in 30 clubfoot patients without and with relapse. Eligible partici…

Reconstruction of Traumatic Central Slip Injuries: Technique Using a Slip of Flexor Digitorum Superficialis.

Multiple reconstruction techniques have been described for correction of boutonniere deformities including direct repair, central slip reconstruction, lateral band reconstruction, transverse retinacular ligament reconstruction, staged reconstruction, and extensor tenotomy. Each technique has been reported to have variable results with complications including capsular contracture, loss of proximal interphalangeal flexion, and residual deformity. We describe a surgical technique for central slip reconstruction using a slip of the flexor digitorum superficialis tendon through a bone tunnel.

Technique of Pronator Teres Rerouting in Pediatric Patients With Spastic Hemiparesis.

Children with spastic hemiparesis can present with forearm pronation deformities that can greatly impair function. In the appropriate setting, pronator teres rerouting can provide active supination while preserving active pronation, which may improve function in these patients. Patient selection is imperative for the success of this procedure because, in the wrong setting, pronator teres rerouting can lead to fixed supination deformity that may actually worsen position and function in these patients.

Biomechanical Assessment of Patellar Advancement Procedures for Patella Alta.

Crouch gait deformity is common in children with cerebral palsy and often is associated with patella alta. Patellar tendon advancement typically is used to correct patella alta and restore normal knee mechanics. The purpose of this study was to determine the mechanical strength of surgical constructs used for fixation during patellar advancement procedures. This study used a cadaveric model to determine which of 3 surgical techniques is biomechanically optimal for patellar tendon advancement in treating patella alta. Twenty-four human cadaveric knees (8 per group) were prepared using 1 of 3 di

Reverse Evans peroneus brevis medial ankle stabilization for balancing valgus ankle contracture during total ankle replacement.

Medial ankle instability secondary to deltoid ligament insufficiency is frequently encountered when performing total ankle replacement and remains a challenge. In the present techniques report, we describe a "reverse" Evans peroneus brevis tendon nonanatomic deltoid ligament reconstruction for medial ankle stabilization harvested through limited incisions using simple topographic anatomic landmarks. The harvested peroneus brevis tendon is brought through a drill hole in the talus from laterally to medially, aiming for the junction of the talar neck and body plantar to the midline. The tendon i

Double arthrodesis through a medial approach for end-stage adult-acquired flatfoot.

Triple arthrodesis has traditionally been the procedure of choice for end-stage adult-acquired flatfoot. The results have been universally good, and it has proven to be dependable and predictable. Nonetheless, complications have been reported following triple arthrodesis in certain patients. Selective arthrodesis of the talonavicular joint and subtalar joint through a single medial approach has been developed as an alternative. The authors especially prefer this procedure with severe transverse plane deformity and often choose this approach as an alternative to triple arthrodesis in high-risk

Modified arthroscopic McLaughlin procedure for treatment of posterior instability of the shoulder with an associated reverse Hill-Sachs lesion.

Traumatic posterior shoulder dislocations are often accompanied by an impression fracture on the anterior surface of the humeral head known as a "reverse Hill-Sachs lesion". This bony defect can engage on the posterior glenoid rim and subsequently lead to recurrent instability and progressive joint destruction. We describe a new modified arthroscopic McLaughlin procedure, which allows for filling of the bony defect with the subscapularis tendon and subsequently prevents recurrence of posterior instability. This technique creates a double-mattress suture providing a large footprint for the subs

Arthroscopic latissimus dorsi transfer.

The patient is placed in lateral decubitus. A 6-cm incision made in the axilla allows access to the latissimus dorsi tendon and its neurovascular pedicle. Holding the arm in internal rotation, the surgeon detaches sharply the tendon off the humeral shaft and then reinforces it with wrapping sutures. Pulling the free limbs of the sutures exposes the under surface of the muscle and helps to identify the neurovascular pedicle. Special lighting retractors suited for a large diameter scope are helpful. Mobilization is completed when 2 cm of the tendon crosses the posterior edge of the acromion. The
