Transoral Robotic Posterior Glottic Scar Lysis and Partial Arytenoidectomy: Report of a Novel Application of TORS for Benign Glottic Surgery.

OBJECTIVES: Transoral robotic surgery (TORS) for benign glottic conditions is scarcely represented in the literature. We aimed in this study to report on the technical feasibility of an explicitly transoral robotic approach for a patient with posterior glottic stenosis, and to highlight extant limitations in exposure and robotic instrumentation of the glottis. METHODS: Retrospective chart review of a single patient undergoing a TORS approach to maximize the posterior glottic aperture. RESULTS: A 42-year-old female patient with posterior glottic stenosis and a tracheotomy was consented for a …

Cut to Air.

A "cannot ventilate, cannot intubate" scenario is a rare, high-risk anesthesia event. Cricothyrotomy is the final step, but anesthesia training and maintenance of surgical airway skills is variable. The ability to "cut to air" when one performs a cricothyrotomy may be all that prevents a patient from experiencing anoxic brain injury or death. Forty-three Certified Registered Nurse Anesthetists (CRNAs) performed emergency cricothyrotomies on a simulation manikin. Three techniques were available: (1) cricothyrotomy kit, (2) scalpel and tracheostomy, and (3) scalpel/bougie/endotracheal tube. Tech

Prophylaxis and Treatment of Complications After Tracheal Resection.

Tracheal resections are major surgical procedures with a complication rate as high as 44%. Early detection of complications followed by a structured and expedited course of action is critical for achieving a successful outcome. The prevention of complications after tracheal resection starts with a correct indication for resection. A thorough preoperative evaluation, meticulous surgical technique, and good postoperative care in a center that performs airway surgery routinely are important factors for achieving good results.

How to Overcome the Challenges of Non-Detachable Connector Tubes in Submental Intubation for Panfacial Fractures.

INTRODUCTION: Armored removable connector tubes are not always available in operating rooms for routine Altemir submental intubation (SMI) technique. The present study addresses a fiber-optic glidoscopy assisted 2-tubes modification of Green & Moore sequence for submandibular intubation. METHODS: The sample was composed of 11 patients (8 males and 3 females) with panfacial fractures where neither the oral and nasal intubation techniques nor tracheostomy were feasible. The inter-operative procedure duration was recorded and a comparison was made between the present results with those obtained

Observations in a cohort of infants with severe laryngeal dyskinesia Auditory brainstem response may aid in the diagnosis.

OBJECTIVES: Laryngeal dyskinesia is not a well defined clinical entity, and difficult to distinguish from some other laryngeal disorders. We report on the observations, especially auditory brainstem responses, in a series of infants with laryngeal dyskinesia. METHODS: All infants with laryngeal dyskinesia, diagnosed in Sophia Children's Hospital between 1 January 2000 and 31 June 2003, were included in this retrospective case series study. Auditory brainstem responses were available in most infants. The respiratory, neurological and other symptoms, were recorded. RESULTS: Nine infants were d

Effect of slow wave and REM sleep on thyropharyngeus and stylopharyngeus activity during induced central apneas.

The pharyngeal constrictors have been hypothesized to play an important role in the regulation of upper airway (UAW) patency in patients with sleep apnea. However, little research has focused on the activation and control of muscles that determine the lateral and posterior wall of the retropalatal airway dimensions. Our aim was to investigate the effects of slow wave sleep (SWS) and rapid eye movement (REM) sleep on the activation of pharyngeal constrictor (thyropharyngeus; TP) and dilator (stylopharyngeus; SP) muscles during eupneic breathing and induced central apneas. In nine goats, we foun

Acquired subglottic stenosis caused by methicillin resistant Staphylococcus aureus that produce epidermal cell differentiation inhibitor.

Local infection of the trachea in intubated neonates is one of the main risk factors for development of acquired subglottic stenosis, although its role in the pathogenesis is unclear. Methicillin resistant Staphylococcus aureus (MRSA) is often the cause of critical illness in neonatal patients. Two cases are reported of acquired subglottic stenosis following bacterial infection of the trachea, suggesting an association with the staphylococcal exotoxin, epidermal cell differentiation inhibitor (EDIN). EDIN-producing MRSA were isolated from purulent tracheal secretions from both infants. Acquire

Acute external laryngeal injury.

A retrospective study of 35 patients with acute external laryngeal injury who were treated at Maharaj Nakorn Chiang Mai Hospital from January 1989 to 1998 was done. Eighty-six per cent of the patients with blunt trauma had been injured from a motor vehicle accident and the rest had a penetrating injury from a stab wound. The frequent signs and symptoms were hoarseness, skin contusion, pain and subcutaneous emphysema. The severity of the injury was classified into minor and major groups. There were 2 patients in the minor group and 33 in the major group. Both patients in the minor injury group

Tracheobronchial extension of recurrent respiratory papillomatosis.

Endobronchial and pulmonary dissemination reportedly occurs in 5% of patients with laryngeal papillomatosis. It is more frequently observed … of pulmonary involvement. Of the 15 patients with tracheobronchial extension, 80% (N = 12) required tracheotomy before their presentation to us. The number of multiple surgical procedures performed to remove papillomas ranged from 7 procedures in 1 patient with a solitary tracheobronchial lesion to more than 100 procedures in 2 patients with diffuse bronchopu …

Acute epiglottitis in the adult caused by Neisseria meningitidis.

Acute epiglottitis is being increasingly encountered in the adult population. While it has many similarities to its pediatric counterpart, the presentation and associated pathogens are distinct. In addition, although the recent literature has suggested that acute epiglottitis in the adult follows a benign course this is not always the case. Here we describe a case of acute, fulminant epiglottitis in an immunocompetent host, caused by Neisseria meningitidis.

Acute epiglottitis in adults. A review of 48 cases.

Acute epiglottitis was diagnosed infrequently in adults until the late 1960s and early 1970s. Because it is relatively rare, it may present a problem to the physician who sees an adult with sore throat and dysphagia, but does not think of epiglottitis. In this paper, we report our experience with 48 cases of acute epiglottitis in adults between the years 1963 and 1987. A discussion of the diagnosis and treatment of adult epiglottitis is presented. An adult with acute painful dysphagia should be considered to have epiglottitis until the diagnosis is proven otherwise.

Prolonged tracheal intubation in the trauma patient.

Over a 15-month period, 74 trauma patients who were expected to require extended intubation were studied prospectively to evaluate the appropriateness of tracheostomy. Patients were randomized to receive either early (34) or late (40) tracheostomies. The patients also were grouped to determine the difference of early versus late tracheostomy on the development of laryngotracheal pathology and respiratory infections; length of intubation and type of patient injury were studied as possible differential factors. Fifteen per cent (11/74) of the patients developed major laryngotracheal pathology as

Trauma of the larynx and cervical trachea. A 10-year review of 51 patients.

Trauma of the larynx and trachea is not a common condition. But once it happens, the management is not simple. This condition has been calling attention of physicians also because iatrogenic traumas have been increasing. In the 10-year period from 1971 to 1980, 51 patients with trauma of the larynx and/or the cervical trachea were treated in the Department of Otolaryngology, Kurume University Hospital. The purpose of this paper is to review these patients and to discuss the treatment of this difficult condition.
