A cortical potential reflecting cardiac function.

Emotional trauma and psychological stress can precipitate cardiac arrhythmia and sudden death through arrhythmogenic effects of efferent sympathetic drive. Patients with preexisting heart disease are particularly at risk. Moreover, generation of proarrhythmic activity patterns within cerebral autonomic centers may be amplified by afferent feedback from a dysfunctional myocardium. An electrocortical potential reflecting afferent cardiac information has been described, reflecting individual differences in interoceptive sensitivity (awareness of one's own heartbeats). To inform our understanding

Low concentrations of pentobarbital enhance excitability of rat hippocampal neurons.

BACKGROUND: Although the excitation phase observed during anesthetic induction and emergence is familiar to anesthesiologists, the cellular mechanisms of this phenomenon are not well understood. At anesthetic concentrations approximately one-tenth those required for surgical anesthesia, subjects demonstrate increased responsiveness to noxious stimulation. We previously estimated that the decrease in nociceptive reflex threshold is maximal at pentobarbital concentrations of approximately 5 microM. Here we used the rat hippocampal slice preparation to examine whether 5 microM pentobarbital incre

O2 sensing at the mammalian carotid body: why multiple O2 sensors and multiple transmitters?

Carotid bodies are the sensory organs for detecting systemic hypoxia and the ensuing reflexes prevent the development of tissue/cellular hypoxia. Although every mammalian cell responds to hypoxia, O2 sensing by the carotid body is unique in that it responds instantaneously (within seconds) to even a modest drop in arterial PO2. Sensing hypoxia in the carotid body requires an initial transduction step involving O2 sensor(s) and transmitter(s) for subsequent activation of the afferent nerve ending. This brief review focuses on: (a) whether the transduction involves 'single' or 'multiple' O2 sens

Spastic bladder and spinal cord injury: seventeen years of experience with sacral deafferentation and implantation of an anterior root stimulator.

INTRODUCTION: Spinal cord injured patients with a suprasacral lesion usually develop a spastic bladder. The hyperreflexia of the detrusor and the external sphincter causes incontinence and threatens those patients with recurrent urinary tract infections (UTI), renal failure, and autonomic dysreflexia. All of these severe disturbances may be well managed by sacral deafferentation (SDAF) and implantation of an anterior root stimulator. MATERIAL AND METHOD: Between September 1986 to December 2002, 464 paraplegic patients (220 female, 244 male) received a SDAF-SARS. Almost exclusively the SDAF wa

The trigeminal tract and nucleus procedures in treatment of atypical facial pain.

BACKGROUND: Atypical facial pain (AFP) is a throbbing pain situated deep in the eye and malar region, often radiating to the ear, neck, and shoulders. The pain generally is not within any dermatomal or anatomical boundaries. Atypical facial pain is distinct from trigeminal neuralgia and its variants. Therefore, the treatment of AFP should be specified. There is also no consensus in the treatment of AFP. Two different treatment procedures on the trigeminal tract and nucleus in a series of cases with AFP are presented. METHODS: Between 1989 and 2005, 17 patients with AFP, in whom previous thera

Midbrain projecting dorsal column nucleus neurons in a reptile.

The origin of midbrain projecting cells in the dorsal column nucleus (DCN) was investigated in reptiles, Caiman crocodilus, using a retrograde tracer. Labeled neurons were confined to a caudal-central portion of the DCN. Midbrain projecting DCN neurons had round, oval, or triangular soma and were small. While neurons that project to the spinal cord in Caiman are also located in the caudal half of the DCN, midbrain projecting cells are located more dorsally and are smaller than those whose axons terminate in the spinal cord. Taken together, these observations suggest that the DCN in Caiman is s

Sensory determinants of thermal pain.

It is still unclear whether the quality of painful thermal sensation is determined only by conduction in specific, dedicated nociceptive channels (i.e. C or Adelta nociceptors) or whether it is a result of integrated activity in both nociceptive and non-nociceptive systems. To evaluate this question, we conducted quantitative and qualitative somatosensory testing in spinal cord injury subjects who suffered from partial or complete loss of thermal sensibility. Testing was performed in skin areas, below the level of the lesion, which were either lacking any thermal sensibility, lacking only one

Sensory and affective components of orofacial pain: is it all in your brain?

In this paper, we shall review several chronic orofacial pain conditions with emphasis on those that are essentially refractory to treatment. We shall present a review of current and past literature that describes the various pain phenomena as well as their underlying central mechanisms. New data concerning refractory pain will be used to underscore the importance of central processing of pain, with particular emphasis on neuropsychological and cognitive function and capacity that may play important roles in pain processing and maintenance of the pain state. Further, neurophysiological data sh

Bilateral distribution of vagal motor and sensory nerve fibers in the rat's lungs and airways.

This study combined single and transneuronal labeling to define the origin of midline-crossing vagal fibers projecting to the rat's lungs. Injections of the beta-subunit of cholera toxin (CT-beta) into the lungs labeled similar numbers of neuronal somata in the nucleus ambiguus and dorsal motor nucleus of the vagus on both sides of the medulla, even though vagal stimulation increased lung resistance 50% less in the contralateral than in the ipsilateral lung. Unilateral cervical vagotomy prevented CT-beta labeling of ipsilateral neuronal somata and sensory fibers, indicating that lung-bound vag

Activity-dependent regulation of dopamine content in the olfactory bulbs of naris-occluded rats.

Several lines of evidence strongly suggest that reduced olfactory nerve activity results in decreased bulb dopamine content. In the present study, high performance liquid chromatography with electrochemical detection was used to assess catecholamine levels in bulbs from postnatal day 60 rats that had undergone either unilateral naris cautery or a sham surgery on day 30. Thirty days of odor deprivation dramatically reduced dopamine and dihydroxyphenylacetic acid levels in functionally-deprived bulbs (ipsilateral to occluded nares) as compared to contralateral controls, while norepinephrine and

The function of sensory nerve fibers in lumbar radiculopathy. Use of quantitative sensory testing in the exploration of different populations of nerve fibers and dermatomes.

STUDY DESIGN: The function of sensory nerve fibers in patients with lumbar radiculopathy and in control individuals was evaluated using quantitative sensory testing. OBJECTIVES: To investigate the effect of lumbar nerve root compression on different populations of nerve fibers and to explore the function of sensory nerve fibers in neighboring nerve roots not involved in the mechanical compression. BACKGROUND DATA: Results from experimental and clinical studies indicate that chronic compression of lumbar nerve roots affects the large myelinated nerve fibers. The majority of nerve fibers invol

[Effects of different natural stimulants on the activity of the rabbits' carotid chemoreceptor].

The effects of different natural stimulants on the afferent unit discharge of rabbit carotid chemoreceptors were studied in the in vitro carotid body- sinus nerve preparation. A total of 32 chemoreceptive units were recorded. The results were as follows: (1) Of the 32 units, 10 (31%) showed chemosensory responses only to PO2 decrease; 9 (28%) to all stimulants (PO2 decrease, PCO2 increase and pH decrease); 9 (28%) to PO2 decrease and PCO2 increase; 3 (9%) to PO2 decrease and pH decrease; only one to pH decrease. (2) The potency of the three natural stimulants in eliciting the changes in intens

[Multimodal evoked potentials and central mechanisms of afferentation in trigeminal neuralgia].

Multimodal evoked potentials in patients with trigeminal neuralgia are analyzed in the paper. The comprehensive studies of cortical trigeminal somatosensory evoked potentials, visual evoked potentials, and brainstem auditory evoked potentials have revealed their changes that are indicative of the impaired central mechanisms of afferentation in patients with trigeminal abnormality. The findings are discussed in the light of the theory of generator mechanisms of neuropathological pain syndromes.

The Duke experience with the nucleus caudalis DREZ operation.

The nucleus caudalis DREZ operation has been performed in three phases at Duke. Between 1982 and 1988 radiofrequency (RF) lesions were made in the trigeminal nucleus extending from the C2 root to the obex using a straight electrode. Complications include ipsilateral arm ataxia due to spinocerebellar tract injury and ipsilateral lower limb weakness from the pyramidal tract. The former occurred at least transiently in 90% of cases. The electrode employed from 1988 to 1989 had proximal insulation protecting the spinocerebellar tract. Since 1989 a ninety degree bend has been added to the electrode

Microsurgical DREZotomy (MDT) for pain, spasticity, and hyperactive bladder: a 20-year experience.

Since 1972, micro-DREZ-tomy has been performed in 367 patients: with cancer pain in 81, neurogenic pain in 139, hyperspasticity in 135, and hyperactive neurogenic bladder in 12. MDT consists of an incision and bipolar coagulations performed ventro-laterally in the Dorsal Root Entry Zone (DREZ) at the entrance of the rootlets into the dorso-lateral sulcus. The lesion is directed at 45 degrees ventro-medially, and 2-3 mm deep according to the pre-operative neurological status and the desired effects. MDT 1 degree interrupts the small (nociceptive) fibres regrouped laterally and the large (myotat

Evidence for uptake of vital dye by activated rat peritoneal mast cells: an in vitro imaging study.

Uptake of material from surrounding medium by activated rat peritoneal mast cells (PMCs) was studied using in vitro peritoneal eluate cells, the vital fluorescent dye sulforhodamine B (SFRM-B), secretagogue compound 48/80, and an imaging technique. PMCs, which undergo different states of degranulation, are shown to possess the ability to take up (by endocytosis) SFRM-B in an activity-dependent manner. The endocytosed dye is incorporated in the granules and can be discharged into the medium when the cells are reactivated. Both the uptake and the discharge processes are calcium-dependent. The re

Deficits in the function of small and large afferent fibers in confirmed cases of carpal tunnel syndrome.

Nerve conduction studies and measurements of thermal thresholds (hot and cold) were performed in 25 healthy control subjects and 24 patients with carpal tunnel syndrome: first, to compare the extent of abnormality demonstrable in the function of large and small afferent axons; second, to correlate these abnormalities with clinical severity; and third, to validate a modified form of palmar stimulation as an additional neurophysiological test in suspected carpal tunnel syndrome. The routine nerve conduction studies compared the amplitudes and conduction velocities from digit II and digit V to wr

Capsaicin-induced activation of fine afferent fibres from rat skin in vitro.

A preparation of the hindpaw-skin together with the saphenous nerve from the adult rat was maintained in vitro. This was used to characterize the properties of sensory receptors with slowly conducting nerve fibres (C- and A delta) and to evaluate the effects of capsaicin and the capsaicin antagonist, capsazepine. Mechano-heat sensitive C-fibres were the most sensitive to capsaicin (threshold < 0.3 microM) applied to the receptive field. Other types of C-fibres were less sensitive (mechano-cold sensitive fibres threshold 1 microM) or insensitive (high- and low-threshold mechano-sensitive fibres

Cerebellar ataxia and muscle spindle sensitivity.

1. The cerebellum has long been known to participate in movement control. One of the enduring theories of cerebellar function is that it "tunes" and coordinates sensorimotor traffic in other parts of the CNS. In particular, it has been implicated in the control of the sensitivity of muscle spindle stretch receptors through the fusimotor system. 2. The stretch sensitivity of spindle primary endings can be varied approximately over a 10-fold range by fusimotor efferent action. For many years it has been believed that cerebellar dysfunction is associated with reduced drive to the fusimotor system

Vestibular primary afferent projection to the cerebellum of the rabbit.

The vestibular primary afferent projection to the cerebellum of the rabbit was studied with retrograde and orthograde tracers. We injected individual lobules of the cerebellum with horseradish peroxidase (HRP) or wheat germ agglutinin-HRP (WGA-HRP). Following these injections the numbers of labeled and unlabeled cells in Scarpa's ganglion were counted. Approximately 64-89% of the cells in Scarpa's ganglion were labeled retrogradely following uvula-nodular injections. About 2% of the cells in the ipsilateral Scarpa's ganglion were labeled after injections of the flocculus. Virtually no cells we
