Adults with amblyopia, a common visual cortex disorder caused primarily by binocular disruption during an early critical period, do not respond to conventional therapy involving occlusion of one eye. But it is now clear that the adult human visual cortex has a significant degree of plasticity, suggesting that something must be actively preventing the adult brain from learning to see through the amblyopic eye. One possibility is an inhibitory signal from the contralateral eye that suppresses cortical inputs from the amblyopic eye. Such a gating mechanism could explain the apparent lack of plast…

AIM: To explore the views of consultant ophthalmic surgeons in Wales in the context of planning cataract surgery in patients with amblyopia. To compare prevailing views and preferences with recommendations in published literature.
METHOD: A cross-sectional survey was conducted in which all consultant ophthalmologists working in Wales were invited to complete an online survey designed using the Survey Monkey tool (http://www.surveymonkey.com). The survey included a clinical scenario involving an amblyopic patient with bilateral cataracts with questions designed to elicit responders' preference…

PURPOSE: To inform ophthalmologists of the current status of visual training.
DESIGN: Personal perspective.
METHOD: A perspective and analysis of current practices that include a review of the literature and personal experiences of the author.
RESULTS: Visual training of some sort has been used for centuries. In the first half of the twentieth century, in cooperation with ophthalmologists, orthoptists introduced a wide variety of training techniques that were designed primarily to improve binocular function. In the second half of the twentieth century, visual training activities were taken

Experiencing binocularly conflicting signals early in life dramatically alters the binocular responses of cortical neurons. Because visual cortex is highly plastic during a critical period of development, cortical deficits resulting from early abnormal visual experience often mirror the nature of interocular decorrelation of neural signals from the two eyes. In the preceding paper, we demonstrated that monkeys that experienced early alternating monocular defocus (-1.5, -3.0, or -6.0 D) show deficits in stereopsis that generally reflected the magnitude of imposed monocular defocus. Because thes

A man aged 17 had an amblyopic left eye with central fixation and lost his normal right eye by an accident. After the accident, spontaneous improvement of the visual acuity of the amblyopic left eye was rapid. Just after the accident, overall contrast sensitivity of the left eye was considerably less than that of a normal adult. Improvement of the contrast sensitivity was recorded first at low spatial frequencies and then at high spatial frequencies. However, the contrast sensitivity at medium spatial frequencies remained less than that of a normal adult. These results suggest that lateral inh

This perspective article makes the case for evaluating and training peripheral stereopsis, particularly when the central visual field is compromised in one or both eyes. Examples of clinical conditions that preferentially affect the central visual field include macular degeneration, which affects the central macular region in one or both eyes, and amblyopia where the central field is often affected in one eye, but the peripheral field is largely intact. While binocular acuity may be preserved when the monocular central field of one eye is affected, fine stereopsis is compromised because it req…

Microsaccade research has recently reached a critical mass of studies that allows, for the first time, a comprehensive review of how microsaccadic dynamics change in neurological and ophthalmic disease. We discuss the various pathological conditions that affect microsaccades, their impact on microsaccadic and other fixational eye movement dynamics, and the incipient studies that point to microsaccadic features as potential indicators of differential and early diagnoses of multiple clinical conditions, from movement disorders to attention-deficit hyperactivity disorder to amblyopia. We propose …

Periorbital arteriovenous malformations (AVMs) are congenital lesions that may cause significant morbidity such as amblyopia, cosmetic disfigurement, or chronic pain. Due to the rarity of these lesions, they are frequently misdiagnosed and treated inappropriately. We managed a 6-year-old girl with preseptal AVM by endovascular embolization followed by complete surgical excision after 2 days. She was previously diagnosed as capillary hemangioma and was being treated with intralesional steroid injections and oral propranolol. Neuroimaging revealed an AVM fed by the branches of both external and …

Neuroplasticity refers to the ability of the brain to reorganize the function and structure of its connections in response to changes in the … nosis and treatment of ocular diseases, such as retinal disorders, cataract and refractive surgery, amblyopia, and in the evaluation of surgical materials and techniques. Furthermore, eliciting plasticity could open new perspectives in the development of strategies that trigger plasticity for better medical and surgical outcomes.

Congenital esotropia is defined as an esotropia before the age of 6 months, because of its early onset and often constant angle, it cause heavier binocular visual function damage compared with other types of strabismus. Early surgery can improve binocular visual function, but demand reliable, accurate measurements which is difficult to achieve in children, and it may increase the risk of second surgery, amblyopia and anesthesia. Late surgery can gain reliable strabismus angle, but it reduced the likelihood of recovery binocular visual function, and the contracture of medial rectus after a long…

Early diagnosis and management of strabismus is needed to avoid complications such as amblyopia. In this retrospective cohort study we reviewed the profile and surgical outcome of vertical strabismus patients attending a clinic in Yazd city in the Islamic Republic of Iran. From the medical files of 265 patients, 19.2% were found to have pure vertical deviation and 80.8% had combined horizontal and vertical strabismus. Congenital causes were recorded for 83.0% of cases and 24.9% had a family history of strabismus. Hypertropia (91.7%) was more common than hypotropia (8.3%). There was a statistic…

PURPOSE: A case of optic nerve hypoplasia (ONH) is reported to demonstrate the value of the Heidelberg Retinal Tomograph II (HRT) in assisting with the diagnosis.
CASE REPORT: An 8-year-old black male was referred to our clinic for evaluation of right esotropia and presumed amblyopia. Best-corrected visual acuities were 20/60 OD (right eye) and 20/25 OS (left eye). Cover test showed constant right esotropia, measuring 10 at distance and 16 at near. A smaller optic nerve head was detected OD with absence of the double ring sign. The findings of HRT were consistent with a diagnosis of unilatera…

OBJECTIVE: To study the prevalence of refractive errors and certain eye diseases in children in southern Jordan.
METHODS: In the period bet … an additional 5 patients had unilateral VA of < 6/12. Strabismus was seen in 9 patients (0.5%), and amblyopia in 5 patients (0.3%). Vernal keratoconjunctivitis was seen in 26 patients (1.5%).
CONCLUSION: It is important to conduct a large study, perhaps a multicenter study to evaluate the eye diseases in developing countries.

The authors report on a case of a 4 1/2-year-old male child, who was referred because of a convergent microstrabisms of the right eye combined with amblyopia and minimal anisometropia and progressive decrease of the visual function from 0.6 to 0.05 although occlussion-therapy was performed. Funduscopy showed a melanoma-like choroidal tumor temporal-inferior of the macula with shallow retinal detachment and severe cellular vitreal infiltration. Because of the progression the tumor was resected by the transscleral technique of Foulds (1983). Histology showed a ganglioneuroma of the choroid, whic…
