We used specular microscopy to study the corneal endothelium of eight aphakic patients who had undergone epikeratophakia. The average post-operative period was 16 months with a range of 12 to 18 months. Corneal thickness increased from 0.52 mm +/- 0.01 mm to 0.88 mm +/- 0.04 mm with the addition of the onlay graft. The average pre-operative endothelial cell count was low (1,450 per mm2 +/- 218) and did not differ significantly (p greater than .9) from the average postoperative cell count (1,438 per mm2 +/- 218). Despite the increase in corneal thickness and low endothelial cell counts, we saw

In 14 selected cases of retinal detachment the vitreous cavity was filled with Healon (0.8-4.0 ml) during subretinal drainage. Follow-up periods ranged between 2 and 7 months. In 12 cases the retinal detachment was cured; in 2 eyes redetachment with massive periretinal proliferation occurred. In addition to transient Tyndall phenomena, which were frequently seen in the anterior chamber and vitreous cavity, the intraocular pressure increased to as much as 45 mm Hg postoperatively in 5 cases. Owing to failure of the antiglaucoma therapy in 3 cases the Healon filling had to be removed.

Eyes with corneal disease receiving transplants from premature, full gestational newborns or postnatal donors yield refractive results with a myopic shift. When the donor cornea is from a newborn (40-41 week gestation) this myopic shift was sufficient to correct 5 of 7 aphakic eyes to an average postsurgical refraction of -1.0 diopter (spher equiv). One premature cornea gave -6.0 diopters while six postnatal infant corneas gave an average correction of +8.6 diopters (spher equiv). The steep corneal curvature resulting from surgery explains the additional refractive power noted. The surgical di

Our clinical study was designed to determine whether "through-and-through" corneal sutures would prevent the postoperative intraocular pressure rise that occurs in 70% of aphakic eyes after uncomplicated penetrating keratoplasty. Twenty-five aphakic patients received the same operative procedure except that the corneal buttons of ten of them were sutured into place with "through-and-through" sutures. The intraocular pressures of patients with the "through-and-through" sutures were significantly lower than those of the other patients during the entire postoperative course.

Pseudoexfoliation syndrome (PEX) presents a significant clinical challenge due to its diverse ocular manifestations, including glaucoma and zonular weakness of the lens, which can lead to irreversible visual impairment if left untreated. We report a case of a 78-year-old male presenting with bilateral visual impairment persisting for four years, with a more pronounced decline in the right eye over the past six months. Examination revealed aphakia with pseudoexfoliative material in the right eye, a cataract with pseudoexfoliative material in the left eye, and a notable intraocular pressure (IOP…

PURPOSE: The purpose of this research was to describe a low-cost, accessible method for reducing the risk of posterior donor dislocation in Descemet membrane endothelial keratoplasty (DMEK) performed in eyes with aphakia and a large iris defect (unicameral eyes)-the "safety-net suture."
METHODS: We review 3 cases of aphakic bullous keratopathy in unicameral eyes treated successfully using DMEK aided by a simple technique to create a temporary, partial barrier between the anterior and posterior chambers. The safety-net suture technique is based on a continuous 10-0 polypropylene suture placed …

In an aphakic eye with corneal edema, performing Descemet's stripping automated endothelial keratoplasty (DSAEK) combined with implantation of intraocular lens can be a challenge. This case report describes a surgical technique for postsurgical aphakia with endothelial decompensation in a 42-year-old female with Marfan syndrome and subluxated lens. This technique comprised implanting DSAEK with fibrin glue-assisted sutureless posterior chamber intraocular lens. The donor lenticels were formed on a 60-kHz femtosecond laser platform (IntraLase®). Two partial-thickness scleral flaps and sclerotom…

The development and growth of the vertebrate ocular lens is dependent on the regulated proliferation of an anterior monolayer of epithelial … into fiber cells, dramatically altering lens structure and ultimately leading to microphthalmia and aphakia.
