Asymmetry in cerebral blood flow velocity with processing of facial images during head-down rest.

INTRODUCTION: Ability to interpret facial expression is crucial for non-verbal communication among humans, and could be affected by changes in cerebral circulation during exposure to microgravity or its simulation. METHODS: There were 16 subjects (8 men and 8 women) who were exposed to 24 h of -6 degrees head-down rest (HDR). Transcranial Doppler ultrasonography was used to monitor mean blood flow velocity (MBFV) in the middle cerebral arteries bilaterally during processing of facial images before, at 6 and 24 h of HDR, and after HDR (Pre-, 6H-, 24H-, and Post-HDR, respectively). The laterali

Display color affects motion sickness symptoms in an optokinetic drum.

BACKGROUND: Many stationary participants who view the patterned interior of a rotating cylinder (optokinetic drum) experience motion sickness (MS) symptoms. Most drum interiors have consisted of black and white patterns. An experiment was conducted to investigate the effects of chromaticity on MS onset and severity. METHODS: There were 12 individuals who participated in the experiment (4 men, 8 women, mean age = 25). Keeping rotation speed constant (5 RPM), the color of vertical stripes in an optokinetic drum was manipulated. There were three conditions used: 1) alternating black and white st

Motion sickness: effect of the frequency of lateral oscillation.

INTRODUCTION: Low-frequency lateral oscillation is a cause of motion sickness in some forms of transport. However, the relationship between occurrence of sickness and the frequency of lateral oscillation is not known. This paper presents a study of motion sickness with lateral oscillation at frequencies between 0.0315 Hz and 0.20 Hz. METHOD: There were 120 subjects, in 6 groups of 20, who were exposed for up to 30 min to sinusoidal lateral oscillation with a peak velocity of 1.0 ms(-1) at one of six frequencies (0.0315, 0.05, 0.08, 0.125, 0.16, 0.20 Hz). Subjects provided ratings of their mot

Desloratadine shows no effect on performance during 6 h at 8,000 ft simulated cabin altitude.

INTRODUCTION: Sustained vigilance is required by pilots and crew during flight; therefore, the use of antihistamines with sedating properties is widely prohibited. The purpose of this study was to determine the effects of desloratadine, a long-acting, nonsedating antihistamine, on healthy volunteers placed under conditions of simulated cabin pressure. METHODS: In a double-blind crossover study, 21 subjects randomly received single doses of desloratadine 5 mg, diphenhydramine 50 mg (active control), and placebo on different days separated by washout periods of 7 d. On test days, predose levels

Submarine escape from depths of 30 and 60 feet: 41,183 training ascents without serious injury.

INTRODUCTION: In the case of a submerged, disabled submarine, survivors may be forced to escape by entering the water and ascending rapidly to the surface. The large pressure changes involved may produce pulmonary barotrauma, arterial gas embolism, or barotrauma. To assess the likelihood of such injuries, we retrospectively evaluated medical problems due to submarine escape training among military personnel. METHODS: We evaluated 41,183 controlled ascents performed over the past 21 yr in the escape training tank at Gölcük-Kocaeli, Turkey. Each trainee performed two free ascents from 30 ft and

Dysbaric osteonecrosis screening in submarine escape instructors.

INTRODUCTION: Dysbaric osteonecrosis (DON) is an avascular bone necrosis that can be seen in divers and compressed air workers. Submarine escape instructors constitute a specific group who are exposed to hyperbaric conditions with a constant profile. METHODS: We screened 21 Turkish Navy submarine escape instructors and evaluated 147 skeletal radiographs for dysbaric osteonecrosis. Two instructors who had suspicious DON lesions on the X-rays underwent examination by MRI of the suspected sites. RESULTS: We found no evidence of DON in the radiographs and MRIs of the submarine escape instructors

Intrathecal ligaments and nerve root tension: possible sources of lumbar pain during spaceflight.

UNLABELLED: Lumbar intrathecal ligaments have recently been demonstrated to randomly bind dorsal nerve roots to the dura within the lumbar vertebral column. Lengthening of the vertebral column and associated lumbar back pain experienced by astronauts is common in microgravity. This study was designed to investigate the relationship of lumbar intrathecal ligaments in spinal lengthening as a possible mechanism for back pain. METHODS: A two-part study was designed using 36 vertebral columns from embalmed cadavers. There were 12 vertebral columns studied in mid-sagittal section to demonstrate the

Caffeine restores engagement speed but not shooting precision following 22 h of active wakefulness.

BACKGROUND: Current military missions occasionally require combat readiness of soldiers who might be experiencing a sustained period of activity without sleep. Strategies to overcome the debilitating effects of sleep deprivation include the ingestion of caffeine. Unknown is the efficacy of caffeine use on specific elements of target detection and marksmanship following a modest period of sustained wakefulness. METHODS: There were 20 subjects (mean +/- SD of 26.7 +/- 7.2 yr of age, 179 +/- 6 cm in height, and 84.5 +/- 10.8 kg in weight) who participated in double-blind caffeine and placebo tri

Peptic ulcer data from 4 years of endoscopy in 955 pilots of the Japan Air Self Defense Force.

BACKGROUND: The treatment of peptic ulcer disease has undergone profound changes due to the recognition of Helicobacter pylori as a causative factor. A survey of medical records was made to determine the prevalence of peptic ulcer among pilots of the Japan Air Self Defense Force (JASDF) and to decide on a possible change in JASDF medical policy toward an ulcer-treatment regime involving therapy to eradicate Helicobacter pylori. METHODS: The subjects were 955 male pilots, age 40 or older. Between 1996 and 1999, they underwent gastrointestinal endoscopy 2.47 times on average. Annual ulcer rates

Baroreflex control of heart rate during heating in subjects with low orthostatic tolerance.

BACKGROUND: Heat stress induces a reduction of orthostatic tolerance. The cardiovascular responses, including the cardiac baroreflex response to heat stress, were examined to test the hypothesis that subjects with orthostatically low tolerance demonstrate an impaired baroreflex control of heart rate (HR) during heat stress. METHODS: There were 44 healthy young volunteers who underwent whole body heat stress produced by a hot-water-perfused suit during supine rest for 45 min and 75 degrees head-up tilt (HUT) for 6 min. Esophageal temperature, HR, arterial pressure, and skin blood flow in the f

Cardiovascular effects of anti-G suit and cooling garment during space shuttle re-entry and landing.

BACKGROUND: Many cardiovascular changes associated with spaceflight reduce the ability of the cardiovascular system to oppose gravity on return to Earth, leaving astronauts susceptible to orthostatic hypotension during re-entry and landing. Consequently, an anti-G suit was developed to protect arterial pressure during re-entry. A liquid cooling garment (LCG) was then needed to alleviate the thermal stress resulting from use of the launch and entry suit. METHODS: We studied 34 astronauts on 22 flights (4-16 d). Subjects were studied 10 d before launch and on landing day. Preflight, crewmembers

Acceleration-induced near-loss of consciousness: the "A-LOC" syndrome.

BACKGROUND: There is an insidious phenomenon that can occur when aircrew are exposed to +Gz stress even at levels that are insufficient to cause +Gz-induced loss of consciousness (G-LOC). Under these circumstances aircrew exhibit an altered state of awareness that was termed Almost Loss of Consciousness (A-LOC) by the U.S. Navy in the late 1980's. A-LOC is a syndrome that includes a wide variety of cognitive, physical, emotional, and physiological symptoms. While A-LOC has been observed in centrifuge studies and reported in flight for over 15 yr, a definitive description of the syndrome does n

Gender not a factor for altitude decompression sickness risk.

INTRODUCTION: Early, retrospective reports of the incidence of altitude decompression sickness (DCS) during altitude chamber training exposures indicated that women were more susceptible than men. We hypothesized that a controlled, prospective study would show no significant difference. METHODS: We conducted 25 altitude chamber decompression exposure profiles. A total of 291 human subjects, 197 men and 94 women, underwent 961 exposures to simulated altitude for up to 8 h, using zero to 4 h of preoxygenation. Throughout the exposures, subjects breathed 100% oxygen, rested or performed mild or

Caffeine effects on marksmanship during high-stress military training with 72 hour sleep deprivation.

PURPOSE: Navy SEALs (sea, air, land) are elite special warfare units that conduct unconventional warfare primarily in marine environments. Marksmanship accuracy and sighting time were quantified with 62 male trainees during Navy SEAL Hell Week, which involves the combined stress of sleep loss, operational combat scenarios, and cold-wet environmental conditions. Caffeine was administered to minimize deficits due to sleep deprivation. METHODS: Volunteers dry-fired a disabled rifle equipped with a laser-based marksmanship simulator system to measure shooting speed and accuracy. The target was a

You're the flight surgeon. Anxiety.

Anxiety is a medical mimicker that can imitate both cardiac and neurological symptoms. Anxiety disorder research protocols regularly use hyperventilation or i.v. lactate infusion to trigger panic attacks in susceptible subjects. Susceptible patients experience panic attacks with slight decreases in CO2 or increases in lactate production seen in mild exercise such as stair climbing. To the unsuspecting physician this appears to be dyspnea on exertion. Hyperventilation during rapid eye movement (dream) sleep may trigger panic attacks in patients with panic disorder, mimicking paroxysmal nocturna

Unexpected syncope and death during intense physical training: evolving role of molecular genetics.

Special operations forces (SOF) undergo an unparalleled degree of physical training. The medical officer responsible for these personnel must often deal with episodes of syncope that cannot be readily explained. In the past, loss of consciousness during heavy physical exertion was attributed to inadequate fluid intake resulting in dehydration or abnormalities in temperature regulation. However, many of those diagnoses, in retrospect were probably incorrect. The occurrence of exercise-related syncope in multiple members of the same family suggested that there could be a genetic basis for the un

Motion sickness and tilts of the inertial force environment: active suspension systems vs. active passengers.

BACKGROUND: Maneuvering in vehicles exposes occupants to low frequency forces (< 1 Hz) which can provoke motion sickness. HYPOTHESIS: Aligning with the tilting inertial resultant (gravity + imposed horizontal acceleration: gravito-inertial force (GIF)) may reduce motion sickness when tilting is either 'active' (self-initiated; Experiment 1) or 'passive' (suspension machinery; Experiment 2). METHODS: Exp 1: Twelve seated subjects were exposed to continuous horizontal translational oscillation through the body x-axis (3.1 m x S(-2) peak acceleration, 0.20 Hz) while making head tilts which were
