PURPOSE: Congenital palmar nail (distal dorsal dimelia [dDD]) of the hand is a rare malformation most commonly affecting the little finger. The purpose of this report was to review the features and associations of this rare disorder and discuss the suspected underlying etiology in light of our current understanding of developmental biology.
METHODS: In this retrospective cohort study from 3 practices, we describe our collective experience and review the reported literature on this disorder both as an isolated condition and in conjunction with other anomalies.

Contracture involving flexor aspect of the fingers is one of the common sequelae encountered after burns, infection, and trauma. In the treatment of contractures, the surgeon is often challenged by the paucity of skin flaps to cover the joint area. It is then covered with skin grafts, leading to recurrence. Articular fractures of the fingers in children may result in severe sequelae that impair function of the hand. All displaced fractures must be reduced and fixed. Undisplaced fractures must be securely immobilized (including neighboring joints) and checked frequently for a possible displacement. Surgical procedures and the results of reconstruction of the mirror hand anomaly are presented. The thumb was reconstructed by pollicization of the radial third finger and multiple muscle transfer. The function of wrist, forearm, and elbow improved with arthroplasty of the affected joints. The results of the follow-up are satisfactory. We believe that surgical treatment of the mirror hand should be started in early life.

Thirty-nine patients were retrospectively reviewed after a wrist arthrodesis using a Synthes wrist fusion plate and iliac crest bone graft. Information was obtained from review of patient files, a questionnaire to assess pain, function and work status, and clinical assessment of grip strength, forearm rotation and fingers motion. This study reviews complications of treatment in 123 mallet fingers treated operatively and nonoperatively. In the 84 digits splinted there was a 45% rate of complications, mostly skin related; however, these complications were almost always transient. For the 45 surgically treated digits the complication rate was 53%, with 76% of these complications still present at a mean follow-up of 38 months.
