Biologic Approaches to Problems of the Hand and Wrist.

Orthobiologics are not used as frequently in the hand and wrist as in other sites. The most frequently reported is the use of bone morphogenetic protein for the treatment of Kienböck disease. Animal studies have described improved tendon healing with the use of platelet-rich plasma (PRP), but no clinical studies have confirmed these results. PRP has been reported to produce improvements in the outcomes of distal radial fractures and osteoarthritis of the trapeziometacarpal in small numbers of patients. The use of orthobiologics in the hand and wrist are promising, but clinical trials are neces

Hand atrophy in a leprosy patient--treatment with polymethylmethacrylate.

Leprosy, an infectious disease caused by Mycobacterium leprae, affects mostly the skin and peripheral nerves. The polymethylmethacrylate has been used as bone cement, knee and intraocular implants as a bioexpansor, filling the area where it is applied. We describe the case of a Brazilian male with tuberculoid leprosy who developed muscular wasting between the metacarpals of both hands. Ten years after leprosy treatment, he was submitted to five applications of 10% polymethylmethacrylate. The treatment was successful, improving the appearance of his hands leading to a positive impact on the pat

Epithelial-immune cell interaction in dry eye.

Dry eye is a potent stimulus of both innate and adaptive immune systems. At the nexus of the dry eye inflammatory/immune response is the dynamic interplay between the ocular surface epithelia and the bone marrow-derived immune cells. On the one hand, ocular surface epithelial cells play a key initiating role in this inflammatory reaction. On the other hand, they are targets of cytokines produced by activated T cells that are recruited to the ocular surface in response to dry eye. This interaction between epithelial and immune cells in dry eye will be thoroughly reviewed.

Fracture of the hook of the hamate: radiographic visualization.

The diagnosis of fracture of the hook of the hamate is rarely made at the time of the initial injury. Routine roentgenograms of the hand in the standard three positions do not visualize this structure. The carpal tunnel view (CTV) with hyperextension of the wrist may be too painful to position. A computerized tomography of the wrist in the transverse or axial plane will clearly and painlessly identify the fracture. Placing both hands and wrists in the praying position gives excellent comparison and documents any developmental bone abnormality. Scintigraphy, when positive, must be followed by t

[Development of treatments and results of mandibular fractures from 1950 to 1978 at the University Center Hospital in Lyon].

Over the period extending from 1950 to 1978, the treatment of mandibular fractures at the CHU of Lyon followed a dual evolutionary course. On the one hand, fractures of the dental part of the bone were treated more and more often surgically via the endobuccal approach, by osteosynthesis using miniature screwed plates. On the other hand, fractures of the region of the condyle benefited from mechanotherapy according to professor Delaire's technique. These therapeutical trends have led to frank improvement in the outcome of these patients, both from the functional and esthetical points of view.

Chondroma in temporomandibular region--case report and therapeutic considerations.

PURPOSE: Chondromas are benign tumors composed of mature hyaline cartilage. These tumors are quite common in the bones of the hands and feet but extremely rare in jaw bones, and few such cases are reported in the literature. The aim of the present study was to carry out a literature review and present a clinical case of a patient with a chondroma in the right mandibular condyle treated with excision of the tumor. CONCLUSIONS: The importance of early diagnosis and treatment in order to impede or minimize any lasting effects is discussed.

Novel findings in a patient with Weaver or a Weaver-like syndrome.

We report on a young male patient with an overgrowth syndrome, who had normal birth weight. He had a number of manifestations typical of the Weaver syndrome (WS), such as advanced bone age, peculiar craniofacial appearance, and camptodactyly. He also showed severe mental and speech retardation and demineralisation of the bones of the hands and feet. The latter can be considered as unreported manifestations of WS, or the patient could represent an example of a new WS-like syndrome.

[A clinical constelation in disuse: vitiligo, mastocytosis of the bone marrow, circulating heparinoid, chronic leg ulcer, hypoalbuminemia, ulcerative colitis and hepatic fibrosis].

We report an unusual case. Our patient had vitiligo, ulcerative colitis, hepatic fibrosis, hypoalbuminemia, bone marrow mastocytosis, a circulating heparinoid and chronic leg ulcers. The relationship between some of the components of this constellation seems logical. This is the case with bone marrow mastocytosis and circulating heparinoid. Also, between hypoalbuminemia and colitis and liver disfunction. On the other hand, the relationship between other features does not seem clear in the light of current knowledge.

Anatomy at Risk: An App for Evaluating Hand Lacerations and Planning Surgical Incisions.

PURPOSE: Anatomy at Risk (AAR), a website and application, was developed to improve educational access and quality for medical and academic learners pertaining to the hand and wrist. It includes information on anatomy such as vessels, nerves, ligaments, musculature, and bones, as well as physical examinations to be performed by learners in order to help them diagnose injuries. Other resources are typically restricted to noninteractive media such as text-book diagrams and often lack the in-depth information required to gain a proper understanding of anatomy that often presents in the real world

Dynamic Monitoring Reveals Motor Task Characteristics in Prehistoric Technical Gestures.

Reconstructing ancient technical gestures associated with simple tool actions is crucial for understanding the co-evolution of the human forelimb and its associated control-related cognitive functions on the one hand, and of the human technological arsenal on the other hand. Although the topic of gesture is an old one in Paleolithic archaeology and in anthropology in general, very few studies have taken advantage of the new technologies from the science of kinematics in order to improve replicative experimental protocols. Recent work in paleoanthropology has shown the potential of monitored re

From the Luttrell Psalter to the Lobster Boy: Split hand and foot awaken many facets of human nature.

A marginal illustration of a non-ambulatory child in the 14th Century Luttrell Psalter is the earliest currently identifiable image of an individual with Split Hand Split Foot with Long Bone Deficiency (SHFLD). Changes in portrayal of SHFLD over the centuries reflect changes in social perception of disabilities from pious sympathy to scientific curiosity and unfortunately also morbid fascination. Hopefully understanding of the past attitudes toward split hand and foot as reflected in art can help in moving toward acceptance of individuals with this highly visible malformation.

Use of the finite element method to assess impact of current on forearm and wrist during an electrical accident.

Using geometric primitives, a three dimensional finite element model has been developed to assess the pathway followed by electric current during an electrical accident assuming the entry point for the current was on one hand. Results indicate that the area where the risk of soft tissue injury is greatest is in the wrist region. Current entry on the palm or finger fronts was found to be more likely to yield injury to the median nerve than an entry point on the back of the hand. The lesser injury is theorized to result because of current shielding of the nerve by bone.

Treatment of hand injuries by external fixation.

Thirty-five consecutive applications of external fixation to the hand, including 27 acute cases and 8 reconstructive procedures, were studied. In both settings, external fixation was used not only for skeletal stabilization but also for management of the soft tissues. Twenty of the 22 acute fractures healed, and six arthrodeses with interposition bone grafts resulted in fusion. Three septic nonunions resolved, and two united successfully. There were no complications. We recommend external fixation systems in the hand, and several case reports are included in the study to illustrate the various

Recalcitrant Ulnar-Sided Hand Pain in a Collegiate Golfer: A Case Report.

This case report details a diagnosis of myositis ossificans in a collegiate golfer with right ulnar-sided hand pain after striking the ground on a swing. Despite conservative treatment and a thorough workup, the patient's ulnar-sided hand pain did not improve. An ultrasound eventually revealed myositis ossificans within the abductor digiti minimi muscle. The patient received an ultrasound-guided fenestration and injection of lidocaine and dexamethasone into the affected muscle, leading to complete symptom resolution. Myositis ossificans is often secondary to trauma, causing bone formation in s

A solitary unilobed osteochondroma of the hamate: a case report.

Solitary osteochondromas originating from the carpal bones are very uncommon; when they occur, they usually arise from the scaphoid or capitate. We report a solitary, unilobed osteochondroma arising from the hamate that was excised, with no evidence of recurrence at the 3-year follow-up.

Degenerative changes at the scaphotrapezial joint following Herbert screw insertion: a radiographic study comparing patients with scaphoid fracture and primary hand arthritis.

One method of Herbert screw insertion is to mobilize the scaphotrapezial joint and insert the screw through the articular surface of the distal scaphoid. Because of concern that this might predispose to osteoarthritis in the scaphotrapezial joint, we have investigated joint space width and the presence of osteophytes adjacent to the scaphotrapezial joint using high definition macroradiography.Twenty-eight patients with symptomatic primary osteoarthritis of their hands, 23 with a scaphoid fracture successfully treated conservatively and 18 treated with a Herbert screw were studied. The patients

Ulnar lengthening and radial recession procedures for Kienböck's disease. Long-term clinical and radiographic follow-up.

Twenty patients with Kienböck's disease who had undergone a joint levelling procedure, were reviewed at a mean of 11 years following surgery. Clinical and radiological evaluation revealed good long-term results with all patients complaining of less pain than before surgery and with statistically significant increases in range of motion and grip strength. Radiologically a significant number of patients were found still to have lunate fractures and fragmentation. In addition, 13 patients have developed new bone formation in the lunate fossa, and 12 show osteoarthritic changes, particularly at th
