Pachydermodactyly: a case report.

Pachydermodactyly is an uncommon variant of digital fibromatosis that has previously been related to knuckle pads. It is a benign condition that needs no investigation for an underlying systemic disease. We present a new case of pachydermodactyly and comment on the available literature.

Cashew nut dermatitis.

The urushiol dermatitis caused by plants of the Anacardiaceae family is the most common cause of acute allergic contact dermatitis. We have reported a case of cashew nut urushiol dermatitis due to ingestion of homemade cashew nut butter contaminated by cashew nut shell oil. With the precautions taken today to avoid contamination of food products with cashew urushiols, it is rare to find a case of cashew nut dermatitis in the United States. We have found no other report of contact dermatitis due to cashew nut butter. Moreover, though hinted at in the literature, there has been no previous detai

The less common occupational dermatoses.

Contact dermatitis and nonmelanoma skin cancer are the most common occupational skin disorders in North America, but there are many other occupational dermatoses that illustrate the wide range of pathological and adaptive responses of the skin to workplace exposure. Discussed are acne, chemically induced leukoderma, nail diseases, contact-related burns, high-pressure injection injuries, contact urticaria, heat reactions and photosensitivity, and infections.

A possible role for superoxide production in the pathogenesis of contact dermatitis.

Superoxide generation by blood monocytes was examined in patients with irritant, nickel, and chromate hand dermatitis. Phorbol myristate acetate stimulated monocytes generated significantly more superoxide in patients with nickel dermatitis, as did patients with hand eczema generally. No significant stimulation of monocyte superoxide generation occurred with either opsonized zymosan or PMA in the presence of excess superoxide dismutase in any of the groups of hand dermatitis. The results indicate a biochemical stimulation of superoxide which may accentuate the immunological damage in the skin

Aquatic hazard Mycobacterium marinum infection.

We describe two patients with Mycobacterium marinum infection and review the pertinent literature. M. marinum infection follows trauma, often trivial, in water or from marine life. Clinical manifestations include superficial cutaneous lesions which are either solitary or multiple in a sporotrichoid distribution, involvement of the deeper structures of the hand and wrist and disseminated disease. Biopsy of infected tissue reveals a mixed suppurative-granulomatous reaction with sparse to absent acid-fast bacilli. Definitive diagnosis is achieved by growing the organism from appropriate specimens

Chromate sensitization and elicitation from cement with iron sulfate.

For some years, iron sulfate has been added to cement manufactured in the Scandinavian countries to prevent sensitization to and elicitation from chromate in cement. Allergic contact dermatitis from chromate is reported here in 3 workers with hand dermatitis and exposure to cement containing iron sulfate. Although iron sulfate had been added to the cement, high chromate concentrations were found in many samples of cement to which these workers were exposed.

[Usefulness of the immunologic profile in "creeping disease." Apropos of a case appearing autochthonously].

A case of "autochthonous" cutaneous larva migrans observed in a patient from the Lazio region (Italy) is described and rarity of the case is stressed. The epidemiological aspects, the pathogenetic problems and the contribution of the sero-immunological methods to the diagnosis are discussed.

Algodystrophy (reflex sympathetic dystrophy) complicating unilateral acrodermatitis continua.

A 65-year-old female suffered trauma to her left hand on which acrodermatitis continua (psoriasis of Hallopeau) developed. It is suggested that the persistent pain of this skin condition, its poor response to treatment and her allied mental state led to the development of a severe algodystrophy with marked underlying osteoporosis.

Herpes simplex lymphangitis. Two cases and a review of the literature.

Lymphangitis and lymphedema are rarely reported complications of herpetic hand or genital infection. The natural history of these complications is gradual resolution over 14 to 21 days. Recognition of this presentation of herpes infection avoids unnecessary surgery and antibacterial therapy. Antiviral therapy may have a role in shortening the duration of symptoms and aborting recurrent lymphangitic episodes.

Long-term ketoconazole treatment of chronic acral dermatophyte infections.

Ketoconazole 200-400 mg was given once daily for a maximum period of 12 months to 31 patients with chronic (mean duration, 12 years) dermatophyte infections of the hands and/or feet. Griseofulvin had previously been withdrawn due to intolerance or lack of effect. All skin and nail infections improved clinically. Fifty percent of the patients with skin infections and 26% of those with nail infections became clinically clear and culture-negative. Six months later, relapses had occurred in 8 of 12 patients (67%) with cleared skin lesions, and in 2 of 5 (40%) with cleared nail infections. Ketocona

Chelating effect of EDTA on nickel.

The chelating effect of disodium ethylenediamine tetra-acetate (EDTA) in nickel-allergic patients was investigated. After pretreatment of the right half of the patient's back with 10% EDTA in a cream, and the left half of the back with the cream base only, various concentrations of nickel sulphate were patch tested on these areas. The blocking effect of the 10% EDTA cream appeared to be significant in comparison with that of the cream base only (p less than 0.01). A barrier cream with 10% EDTA might be of help in nickel-allergic patients with eczema of the hands.

The outcome of primary irritant hand dermatitis.

Four hundred and sixty-two patients suffering from primary irritant hand dermatitis were sent postal questionnaires after 1-16 years regarding the outcome of their dermatitis. Two hundred and thirteen patients replied (a compliance rate of 46%). Of the 213 replies 188 were accepted for analysis, of whom only 58 (30.8%) had complete remission of their dermatitis, whilst 130 patients (69.2%) had continuous, or intermittent disease. Of sixty-four patients who changed their occupation only twenty-one (32.8%) had complete remission of their dermatitis as opposed to thirty-seven (29.8%) of 124 who c

[Hand dermatitis in medical personnel].

Eczema (dermatitis) of the hands of medical personnel is a common skin disease. Occupational dermatitis of the hands arises from frequent contact with irritants and sensitizing agents. Differential diagnosis in hand eczema is usually only possible from the case history. One diagnostic tool is the screening of relevant contact allergies by patch-testing. The most frequent allergen in 114 cases of eczema of the hands in medical personnel between 1976 and 1980 at the Department of Dermatology and Allergology of the Municipal Hospital Schwabing in Munich was formaldehyde in 24 instances (i.e. 21%)
