Liver granulomatosis is not an exceptional cause of hypercalcemia with hypoparathyroidism in dialysis patients.

In 4 of our patients on chronic dialysis, we were intrigued by the association of hypercalcemia +/- hyperphosphatemia and normal intact PTH, with anicteric cholestasis without cytolysis. This picture occurred in 2 patients after they resumed dialysis because of a transplant rejection and in a third one after discontinuation of corticosteroids, prescribed for an idiopathic thrombocytopenia. No patient was under calcitriol, CaCO3 therapy, and their hypercalcemia persisted on a low calcium dialyzate (1.25 mmol/l). Obvious etiologies of hypercalcemia were not found: vitamin D or A intoxication, hy…

A direct effect in vitro of phosphate on PTH release from bovine parathyroid tissue slices but not from dispersed parathyroid cells.

BACKGROUND: Phosphate retention has long been considered to be of importance for the pathogenesis of secondary hyperparathyroidism in chronic renal failure. Hyperphosphatemia in vivo is associated with alterations of calcium and vitamin D levels, both of which are known to alter the parathyroid hormone (PTH) release independently. MATERIALS AND METHODS: We have investigated the direct effect of phosphate on PTH release in vitro using two different preparations of bovine parathyroid tissue: Acutely dispersed bovine parathyroid cells and tissue slices of 0.5 x 0.5 mm were prepared from bovine p…

No significant lipid changes were observed with hyperphosphatemia. In multivariable analyses, HDL-C was positively associated with eGFR (β = 48.8, q = 0.003) and inversely associated with BUN (β = -14.3, q = 0.0014), while RC showed an inverse association with eGFR (β = -19.9, q = 0.0005) and a positive associatio …

Clinical Evaluation of Low-Dose Magnesium Carbonate as a Phosphate Binder in Chronic Hemodialysis Patients.

Background and Objectives: Hyperphosphatemia is a key component of chronic kidney disease-mineral and bone disorder and is associated with higher rates of cardiovascular events and mortality in patients undergoing dialysis. Phosphate binders are essential for phosphorus reduction, and their evaluation relies on several factors, such as chemical composition, binding capacity, and safety profile. Unfortunately, long-term management is hindered by high pill burden and poor adherence. Magnesium phosphate binders have proven effectiveness in reducing phosphate levels. Nevertheless, despite their ef…

Intestinal paracellular phosphate absorption resists dietary adaptation but is upregulated in CKD.

BACKGROUND AND HYPOTHESIS: The paracellular absorption of phosphate (Pi) contributes the bulk of intestinal absorption under conditions of h … lar Pi fluxes. In a mouse model of CKD, active intestinal Pi transport is not downregulated despite hyperphosphatemia and paracellular Pi fluxes elevated. The loss of junctional barrier integrity might be driven driven by uremic toxins such as p-cresol and could contribute to systemic phosphate imbalance in CKD. Our findings highlight the intestinal epithelial tigh …

Real-world data for effectiveness and safety of palopegteriparatide in patients with chronic hypoparathyroidism: the PaTH REAL study.

CONTEXT: Chronic hypoparathyroidism (HypoPT) is a rare endocrine disorder characterized by hypocalcemia and hyperphosphatemia, requiring lifelong treatment with activated vitamin-D and oral calcium supplements. Palopegteriparatide, a novel long-acting prodrug of PTH (1-34), has demonstrated efficacy in randomized controlled trials, but real-world data on its long-term effectiveness and safety remain limited. OBJECTIVE: To evaluate the efficacy and safety of long-term palopegteriparatide therapy in a real-world cohort of patients with HypoPT. DESIGN AND SETTING: Multicenter prospective cohort…

Association between CKD-MBD and mortality in older patients with advanced CKD-results from the EQUAL study.

BACKGROUND: Chronic kidney disease-mineral and bone disorder (CKD-MBD) is a common complication of CKD; it is associated with higher mortali … effect of phosphate, with the highest mortality risk found in patients with both hypercalcemia and hyperphosphatemia. PTH level was associated with CV mortality, but not with non-CV mortality, whereas phosphate was associated with both CV and non-CV mortality in most models. Vitamin D deficiency in chronic kidney disease: Myth or reality?

BACKGROUND AND OBJECTIVE: Chronic kidney disease (CKD) is a public health problem, which has a prevalence of 17.2% in India. As kidney funct … s were found to be increased in only 20% of cases. Calcium1 levels were normal in all CKD cases and hyperphosphatemia was observed in 5% of CKD patients. CONCLUSION: We found that most of our CKD patients (75%) had normal vitamin D levels. Therapeutic Effect of Iron Citrate in Blocking Calcium Deposition in High Pi-Calcified VSMC: Role of Autophagy and Apoptosis.

In chronic kidney disease (CKD), the first cause of mortality is cardiovascular disease induced mainly by vascular calcification (VC). Recently, iron-based phosphate binders have been proposed in advanced CKD to treat hyperphosphatemia. We studied the effect of iron citrate (iron) on the progression of calcification in high-phosphate (Pi) calcified VSMC. Iron arrested further calcification when added on days 7-15 in the presence of high Pi (1.30 ± 0.03 vs 0.61 ± 0.02; OD/mg protein; day 15; Pi vs Pi + Fe, p < 0.01). We next investigated apoptosis and autophagy. Adding iron to high-Pi-treated V…

Control of metabolic predisposition to cardiovascular complications of chronic kidney disease by effervescent calcium magnesium citrate: a feasibility study.

AIMS: Cardiovascular (CV) complications are common in chronic kidney disease (CKD). Numerous metabolic disturbances including hyperphosphatemia, high circulating calciprotein particles (CPP), hyperparathyroidism, metabolic acidosis, and magnesium deficiency are associated with, and likely pathogenic for CV complications in CKD. The goal of this feasibility study was to determine whether effervescent calcium magnesium citrate (EffCaMgCit) ameliorates the aforementioned pathogenic intermediates. METHODS: Nine patients with Stage 3 and nine patients with Stage 5D CKD underwent a randomized cross…
