Do you know this syndrome? Dyspigmentation along the Blaschko lines caused by trisomy 7 mosaicism.

Dyspigmentation along the Blaschko lines is strongly suggestive of a mosaic skin disorder. We report a 9-year-old male patient who presented with swirls and streaks of both hypo and hyperpigmentation involving the entire body. Additionally, he had hypertrichosis, musculoskeletal and minor neurodevelopment abnormalities but no intellectual disability. Cultured fibroblast displayed trisomy 7 mosaicism, which can explain this pigmentary phenotype. Widespread dyspigmentation associated with involvement of other organs should prompt systemic examination to detect additional anomalies and genetic ev

Hemimegalencephaly in a patient with coexisting trisomy 21 and hypomelanosis of Ito.

A male infant with trisomy 21 simultaneously showed clinical features of hypomelanosis of Ito and hemimegalencephaly, with related intractable epileptic seizures. The epileptic seizures were refractory to conventional antiepileptic drugs and persisted until the patient underwent functional hemispherotomy. It is well known that patients with hypomelanosis of Ito may also have cortical dysplasia and hemimegalencephaly and that approximately half of these patients have chromosomal abnormalities. However, to our knowledge, there is no previous report of a patient with trisomy 21 associated with he

False "highlighting" with Wood's lamp.

Wood's lamp evaluation is used to diagnose pigmentary disorders. For example, vitiligo typically demonstrates lesional enhancement under Wood's lamp evaluation. Numerous false positive enhancing lesions can be noted in the skin. We describe a 5-year-old Hispanic boy who had painted his face with highlighter, producing enhancing lesions under Wood's lamp. Physicians who use Wood's lamp should be aware that the appearance of markers and highlighter can mimic that of true clinical illnesses.

Infancy- and childhood-onset dyschromatoses.

The dyschromatoses are a group of pigmentary disorders characterized clinically by mixed and often guttate hypopigmented and hyperpigmented lesions. There are many conditions that present with dyschromatosis, including genodermatoses, inflammatory skin diseases, infections, drug and chemical use, and nutritional disorders. Some conditions have extracutaneous features. Poikiloderma (a combination of hypo- and hyperpigmentation with telangiectasia and atrophy) must be excluded. In this article, we describe the dyschromatoses typically presenting in infancy and childhood, most of which are genode

Progressive macular hypomelanosis: an epidemiological study and therapeutic response to phototherapy.

BACKGROUND: Progressive macular hypomelanosis is a common dermatosis in various continents. Its cause is unknown and proposed treatments have had little effect. OBJECTIVES: To determine epidemiological aspects of progressive macular hypomelanosis in patients referred to a phototherapy clinic between 1997 and 2008 and to evaluate therapeutic response to PUVA (psoralen + UVA) photochemotherapy or narrowband UVB phototherapy. METHODS: Eighty-four patients with progressive macular hypomelanosis were evaluated. After 16 phototherapy sessions, therapeutic response was classified as: unchanged, sli

New and experimental treatments of vitiligo and other hypomelanoses.

This article deals with new and experimental treatments that can be used to treat various forms of depigmenting disorders. Ultraviolet B-focused therapies and new surgical approaches are analyzed in this article. These therapies were mainly or only developed to treat vitiligo, which is the most studied and probably the most challenging of all the hypomelanoses, but the results obtained in trials and clinical experiences about vitiligo sometimes can be referred to other depigmenting disorders.

Clinical manifestations, diagnosis and classification of leprosy.

Mycobacterium leprae, the causative organism of leprosy is slow-growing and the reason is its long incubation period of 2-4 years. Males are predominantly affected and deformity is produced in less than 2% of people affected with the disease. The disease manifests in the skin as macules, papules, nodules, plaques or infiltration. Hypopigmented or erythematous skin patches with definite sensory deficit is one of the clinical cardinal signs by which one can make a definite diagnosis. Demonstration of bacilli in the slit skin smear is the bacteriological cardinal sign used to make definite diagno

Amelanotic/Hypomelanotic melanoma--is dermatoscopy useful for diagnosis?

BACKGROUND: Amelanotic/hypomelanotic melanoma often leads to delayed clinical diagnosis because its clinical appearance can mimic other hypopigmented skin conditions. Dermatoscopy (dermoscopy, epiluminescence microscopy) is thought to be a helpful diagnostic tool in pigmented skin lesions, but it can be used also in non-pigmented skin tumors due to the recognition of vascular structures not visible to the naked eye. OBJECTIVE: 7 amelanotic/hypomelanotic melanomas in 6 patients were analyzed in order to define their dermatoscopic findings using standard pattern analysis. METHODS: Clinical and

Naevus anaemicus with teleangiectatic vessels.

We describe a 20-year-old man with naevus anaemicus on the chest where, after dermabrasion of the epidermis, enlarged teleangiectatic dark-red vessels were seen within the previously pale area. They were clearly different from those seen on dermabrasion at this site in normal skin and in patients with vitiligo where the area is lighter red with only small punctual bleedings from arterial capillaries. The naevus anaemicus and a port-wine stain (naevus flammeus) in the same location is a phenomenon of vascular twin spotting, which was revealed when the epidermis was removed. The area was transpl

The focal facial dermal dysplasias: report of a kindred and a proposed new classification.

BACKGROUND: The focal facial dermal dysplasias (FFDD) are a genetically heterogeneous group of disorders characterized by congenital bilateral scarlike facial lesions, with or without associated facial anomalies. The cases have been reported under various names; thus the nosology is confusing and unclear. OBJECTIVE: Our purposes were to report our kindred, clearly delineate the various types of FFDD reported, and propose a new simplified classification. METHODS: The clinical and histologic changes were examined and genealogy determined for our kindred. The medical literature was reviewed and

Hypopigmented Patches in an Adolescent: A Diagnostic Dilemma and the Value of Clinical Restraint.

Hypopigmented lesions in pediatric patients can present a diagnostic challenge, particularly when clinical and histopathologic findings do n … d histologic findings argued against a definitive diagnosis. The biopsies favored post-inflammatory hypopigmentation, although the clinical history was not entirely supportive. Progressive macular hypomelanosis was considered a diagnosis of exclusion based on the overall clinical context. The patient and family pursued counseling during the course of evaluation, a …

Multi-Omics Analysis Reveals Impacts of SCARB1, TYR, and TYRP1 Knockouts on Pigmentation and Metabolic Pathways in Oujiang Color Common Carp.

Gene knockout (KO) techniques are important for understanding gene function and their impact on organismal traits. This study investigates the effects of targeted KO of SCARB1, TYR, and TYRP1 genes in Oujiang Color common carp, focusing on pigmentation changes, metabolic dysregulation, and host-microbiome interactions. Our multi-omics approach revealed that SCARB1 KO led to complete loss of red pigmentation, while TYR and TYRP1 KOs resulted in hypopigmentation and altered pigmentation patterns. Transcriptomic analysis identified seven key genes (PLA2G4, C3, F2, ERN1, UGP2, purA, and GRIN2A), l…

Case report: Clinicopathological characteristics of SASH1 mutation-related dyschromatosis: a rethinking of the classification of dyschromatosis.

Dyschromatosis, a group of pigmentary dermatoses, accompany both hyper- and hypo-pigmentation, including dyschromatosis symmetrica hereditaria (DSH), dyschromatosis universalis hereditaria (DUH), and familial progressive hyper- and hypo-pigmentation (FPHH). A peculiar phenotype of dyschromatosis presented as multiple lentigines and hypopigmentation with various sizes and shapes was found to be associated with SASH1 mutations and has recently been reported frequently. The current study evaluated the clinical manifestation, pathological pattern, and genetic basis of dyschromatosis in a five-gene…
