Electrical bioimpedance spectral markers distinguish balance ability in older adults without provoking fall risk.

BACKGROUND: Balance assessment in older adults is central to fall-risk evaluation and rehabilitation planning, yet widely used clinical tests such as the single-leg stance require postures most likely to provoke a fall. A method capturing neuromuscular contributions to postural control during safer, static postures would meaningfully advance clinical and home-based assessment. METHOD: We investigated whether electrical bioimpedance (EBI) recorded over the extensor digitorum longus (EDL), a muscle contributing to fine ankle-level postural adjustments, carries spectral signatures reflecting bal

Muscle failure and risk of adverse outcomes in older adults: a derivation and multicohort validation study.

BACKGROUND: Sarcopenia is inconsistently defined, and its definition might not adequately capture muscle failure, underlying disability, and other adverse outcomes in older adults. We aimed to develop and validate an evidence-based, outcome-driven model of muscle failure. METHODS: In this derivation and multicohort validation study, we analysed data from the SPRINTT randomised controlled trial (1519 participants; mean age 78·9 years [SD 5·8]; 431 [28·4%] men, 1088 (71·6%) women) and validated findings in five independent cohorts (ilSIRENTE, NHANES, HRS, ELSA, and CHARLS). Limb-girdle muscular dystrophy type R2 manifesting as fetal growth restriction in a woman with two pregnancies: a case report.

BACKGROUND: Limb-girdle muscular dystrophy (LGMD) is a rare inherited myopathy characterized by progressive weakness of the shoulder and pelvic girdle muscles. Limb-girdle muscular dystrophy type R2 (LGMD R2) caused by DYSF mutations, often shows early onset. Pregnancy in women with LGMD R2 is uncommon and may aggravate neuromuscular disease while predisposing to serious obstetric complications, including fetal growth restriction (FGR), preeclampsia, and intrahepatic cholestasis of pregnancy (ICP). This case is unique as the first report of two pregnancies in a woman with LGMD R2, highlighting

Mechanical Power as a Predictor of Outcomes During Mechanical Ventilation in Coronavirus Disease 2019 (COVID-19): An Updated Systematic Review.

Background/Objectives: Mechanical power (MP) quantifies the energy delivered to the respiratory system during ventilation and serves as a promising marker for ventilator-induced lung injury (VILI). According to its original definition by Gattinoni, MP reflects the energy transferred from the ventilator to the respiratory system under conditions of deep sedation, passive breathing, neuromuscular blockade, and volume-controlled ventilation. Its role in coronavirus disease 2019 (COVID-19)-associated acute respiratory distress syndrome (ARDS) remains under investigation. This systematic review aim

Association between mandibular chewing patterns and sleep bruxism characteristics: a clinical observational study.

BACKGROUND: Sleep bruxism (SB) is a sleep-related movement behavior characterized by rhythmic or non-rhythmic masticatory muscle activity that may contribute to tooth wear, jaw discomfort, and temporomandibular disorders. While SB is commonly assessed using polysomnography (PSG) and electromyography (EMG), daytime functional markers that predict SB intensity and phenotype remain insufficiently defined. Chewing patterns reflect neuromuscular coordination of mandibular movement and may share motor control pathways with SB-related masticatory activity. OBJECTIVE: To investigate whether mandibula

Identifying Fatigue-Related Gait Patterns Using Multiple Inertial Measurement Units and Statistical Parametric Mapping: A Continuous Analysis of an Outdoor Full Marathon in Male Recreational Runners.

BACKGROUND: Running is an effective exercise for personal fitness, yet many recreational runners suffer from running-related injuries. Prolonged running induces neuromuscular fatigue, interfering with an individual's preferred running gait and increasing the injury risk. This study aimed to examine gait patterns associated with fatigue in runners during a full marathon by analyzing lower limb segment and pelvis kinematics captured via multiple inertial measurement units (IMU). METHODS: Three IMUs were attached to measure the rearfoot, shank, and pelvis kinematics of 23 male recreational runne

High-Flow Nasal Oxygen versus Mechanical Ventilation Through a Laryngeal Mask During General Anesthesia Without Muscle Paralysis: A Randomized Clinical Trial.

BACKGROUND: Apneic oxygenation with high-flow nasal oxygen is a novel intraoperative respiratory support strategy for patients undergoing general anesthesia, but data about its clinical effects are scarce. We conducted a randomized trial to assess whether high-flow nasal oxygen is noninferior to mechanical ventilation through a laryngeal mask in terms of success rate of intraoperative respiratory support among patients undergoing a 30-minute general anesthesia session. METHODS: Single-center, randomized, noninferiority trial conducted in Italy between May 2022 and June 2023 and involving Amer

Intraoperative Analgesia with Magnesium Sulfate versus Remifentanil Guided by Plethysmographic Stress Index in Post-Bariatric Dermolipectomy: A Randomized Study.

BACKGROUND: Magnesium sulfate reduces pain scores and analgesic consumption. Its use as an analgesic resource in opioid-free or opioid-sparing techniques have also been tested. The evaluation of the antinociceptive potency of drugs and doses indirectly, through hemodynamic oscillations has been questioned. A relatively new algorithm called the plethysmographic stress index has been considered sensitive and relatively specific as a parameter for assessing the need for intraoperative analgesia. OBJECTIVES: The aim of this trial was to assess the intraoperative analgesic capacity of magnesium su

Magnetization Transfer Ratio in Lower Limbs of Late Onset Pompe Patients Correlates With Intramuscular Fat Fraction and Muscle Function Tests.

Objectives: Magnetization transfer (MT) imaging exploits the interaction between bulk water protons and protons contained in macromolecules to induce signal changes through a special radiofrequency pulse. MT detects muscle damage in patients with neuromuscular conditions, such as limb-girdle muscular dystrophies or Charcot-Marie-Tooth disease, which are characterized by progressive fiber loss and replacement by fatty tissue. In Pompe disease, in which there is, in addition, an accumulation of glycogen inside the muscle fibers, MT has not been tested yet. Our aim is to estimate MT ratio (MTR) i

Test-Retest Reliability and Correlates of Vertebral Bone Marrow Lipid Composition by Lipidomics Among Children With Varying Degrees of Bone Fragility.

The reliability of lipidomics, an approach to identify the presence and interactions of lipids, to analyze the bone marrow lipid composition among pediatric populations with bone fragility is unknown. The objective of this study was to assess the test-retest reliability, standard error of measurement (SEM), and the minimal detectable change (MDC) of vertebral bone marrow lipid composition determined by targeted lipidomics among children with varying degrees of bone fragility undergoing routine orthopedic surgery. Children aged 10 to 19 years, with a confirmed diagnosis of adolescent idiopathic

23 Na MRI depicts early changes in ion homeostasis in skeletal muscle tissue of patients with duchenne muscular dystrophy.

BACKGROUND: Duchenne muscular dystrophy (DMD) is a hereditary neuromuscular disease leading to progressive muscle wasting. Since there is a need for MRI variables that serve as early sensitive indicators of response to treatment, several quantitative MRI methods have been suggested for disease monitoring. PURPOSE: To evaluate the potential of sodium (23 Na) and proton (1 H) MRI methods to assess early pathological changes in skeletal muscle of DMD. STUDY TYPE: Prospective clinical study. POPULATION: 23 Na and 1 H MRI of the right leg were performed in 13 patients with DMD (age 7.8 ± 2.4) an

A muscle-driven approach to restore stepping with an exoskeleton for individuals with paraplegia.

BACKGROUND: Functional neuromuscular stimulation, lower limb orthosis, powered lower limb exoskeleton, and hybrid neuroprosthesis (HNP) technologies can restore stepping in individuals with paraplegia due to spinal cord injury (SCI). However, a self-contained muscle-driven controllable exoskeleton approach based on an implanted neural stimulator to restore walking has not been previously demonstrated, which could potentially result in system use outside the laboratory and viable for long term use or clinical testing. In this work, we designed and evaluated an untethered muscle-driven controlla

Clinical and Physiological Events That Contribute to the Success Rate of Finding "Optimal" Cerebral Perfusion Pressure in Severe Brain Trauma Patients.

OBJECTIVE: Recently, a concept of an individually targeted level of cerebral perfusion pressure that aims to restore impaired cerebral vasoreactivity has been advocated after traumatic brain injury. The relationship between cerebral perfusion pressure and pressure reactivity index normally is supposed to have a U-shape with its minimum interpreted as the value of "optimal" cerebral perfusion pressure. The aim of this study is to investigate the relation between the absence of the optimal cerebral perfusion pressure curve and physiological variables, clinical factors, and interventions. DESIGN

Hypernatremic disorders in the intensive care unit.

Hypernatremia, defined as plasma sodium concentration >145 mEq/L, is frequently encountered in critically ill patients admitted to the intensive care unit (ICU). Hypernatremia indicates a decrease in total body water relative to sodium and is invariably associated with plasma hyperosmolality though total body sodium content may be normal, decreased, or increased. Hypernatremia usually occurs as a result of impaired thirst or access to water, with or without increased water losses from renal and extrarenal sources. Critically ill patients in ICU are at high risk of hypernatremia because of thei

Demographics and clinical outcomes of patients admitted to a respiratory intensive care unit located in a rehabilitation center.

INTRODUCTION: A recent survey of respiratory intensive care units (RICU) in Italy showed that RICUs in Italy are mainly (85%) located in acute care hospitals. Forty-seven percent of the patients are admitted from emergency departments, and only 18% are admitted from intensive care units (ICU), so the percentage of patients admitted for difficulty in weaning is low (8%). Patient demographics and admission patterns in RICUs located outside acute care hospitals have not been previously described. METHODS: We analyzed admission patterns, demographics, treatment, and outcomes of patients during th

[Malignant hyperthermia in Austria. I. Epidemiology and clinical aspects].

Investigation of malignant hyperthermia (MH) was started in 1975; by March 1986, 79 suspected cases had been reported. In vitro contracture tests were performed in 66 probands or their parents; in 61 of these (92%), MH was confirmed and 5 (8%) proved negative. In 4 lethal crises, the parents refused biopsy, but because of well-documented clinical histories these were also included as confirmed MH reactions. We were able to analyse 65 cases of documented MH, and 9 patients are still to be investigated. About 18% of all Austrian hospitals (29 of 158) had reported 1-14 MH reactions (mean 2.7/hosp

Gene replacement therapy with onasemnogene abeparvovec in children with spinal muscular atrophy aged 24 months or younger and bodyweight up to 15 kg: an observational cohort study.

BACKGROUND: Given the novelty of gene replacement therapy with onasemnogene abeparvovec in spinal muscular atrophy, efficacy and safety data are limited, especially for children older than 24 months, those weighing more than 8·5 kg, and those who have received nusinersen. We aimed to provide real-world data on motor function and safety after gene replacement therapy in different patient subgroups. METHODS: We did a protocol-based, multicentre prospective observational study between Sept 21, 2019, and April 20, 2021, in 18 paediatric neuromuscular centres in Germany and Austria. All children w
