Rooming-In to Treat Neonatal Abstinence Syndrome: Improved Family-Centered Care at Lower Cost.

BACKGROUND AND OBJECTIVE: The incidence and associated costs of neonatal abstinence syndrome (NAS) have recently risen sharply; newborns with NAS occupy 4% of NICU beds. We implemented a coordinated program for NAS including standardized protocols for scoring, medications and weaning, and a calm rooming-in environment, to improve family-centered care and to decrease both length of stay (LOS) and hospital costs. METHODS: In early 2013, a multidisciplinary quality improvement team began consecutive plan-do-study-act (PDSA) cycles. We trained nurses in modified Finnegan scoring, ensured scoring

Acceptability of neonatal circumcision by pregnant women in KwaZulu-Natal, South Africa.

BACKGROUND: Studies on voluntary medical male circumcision (VMMC) have provided convincing evidence on its efficacy to provide partial protection against female-to-male HIV transmission in circumcised men. The World Health Organization and UNAIDS subsequently formulated recommendations for VMMC implementation that included implementation of neonatal medical male circumcision (NMMC) to all infants up to two months old. Knowledge regarding the acceptability of NMMC by pregnant women who are candidates for granting of consents for NMMC procedures or its ideal placement within health programmes is

A Multi Method Analysis of the Treatment of Perinatal Patients With Substance Use Disorders.

BACKGROUND: In Colorado, suicide and accidental overdose are the two leading causes of death among pregnant and postpartum individuals. People with substance use disorders report mistreatment in hospital settings, yet there is limited understanding of healthcare providers' perspectives on this issue. The study aimed to investigate hospital staff perspectives on the treatment of patients in/around labor. METHODS: We surveyed 26 birthing hospitals in Colorado that are part of the Colorado Perinatal Care Quality Collaborative's substance use quality improvement initiative. 193 individuals from 1

Experiences of healthcare providers caring for pregnant individuals with substance use disorder.

INTRODUCTION: Substance use disorder (SUD) during pregnancy is associated with an increased risk of adverse maternal and neonatal outcomes, yet many patients face significant barriers to accessing treatment, including experiencing bias and stigma from healthcare providers. To inform improvements in care delivery, this study explored the experiences of healthcare providers who care for pregnant individuals with SUD. METHODS: Researchers conducted seven focus group discussions (FGD) and fifteen in-depth interviews (IDI) using semi-structured guides, with participants drawn from both rural and u

Midwifery Practices for the Conversion of Foetus With Breech Presentation: A Qualitative Study.

BACKGROUND: Although the conversion of the foetus with breech presentation to cephalic presentation is well-supported in the literature in terms of its benefits for maternal and neonatal health, there is insufficient knowledge regarding the specific midwifery practices involved in this process. The purpose of this study is to identify the methods used by midwives to convert fetuses with breech presentation to cephalic presentation and how they feel about performing these methods. METHODS: The study employs a descriptive exploratory qualitative design. Individual in-depth interviews were condu

Estimating the impact of label design on reducing the risk of medication errors by applying HEART in drug administration.

UNLABELLED: Medication errors are one of the biggest problems in healthcare. The medicines' poor labelling design (i.e. look-alike labels) is a well-recognised risk for potential confusion, wrong administration, and patient damage. Human factors and ergonomics (HFE) encourages the human-centred design of system elements, which might reduce medication errors and improve people's well-being and system performance. OBJECTIVE: The aim of the present study is twofold: (i) to use a human reliability analysis technique to evaluate a medication administration task within a simulated scenario of a neo

Hemorrhage risk score and peripartum quantified blood loss.

BACKGROUND: Risk assessment tools are used by clinicians to predict which patients might have excessive bleeding. We studied the association between a peripartum hemorrhage risk assessment score using peripartum quantified blood loss (QBL) among those with vaginal deliveries who are often at lower risk for peripartum hemorrhage. METHODS: In this retrospective analysis of 1221 patients with term vaginal deliveries at a public New York City hospital, the Association of Women's Health, Obstetric and Neonatal Nurses (AWOHNN) risk assessment tool was used to categorize patients as low risk, medium

SSSH: Responsive soothing bassinet feasibility study for infants with congenital heart disease after cardiac surgery.

PURPOSE: For infants with congenital heart disease (CHD) requiring surgery, prolonged hospital stays, intermittent caregiver visitation, and constrained unit staffing ratios present barriers to adequately address post-operative stressors and associated need to retain cognitive and physiological reserves. Similar patients requiring high-engagement interventions, such as hospitalized infants with neonatal abstinence syndrome, have found success in utilizing responsive bassinets to soothe infants and save floor nurses' time. However, it remains unclear if such technology can be leveraged in the C

Conceptualizing stigma in contexts of pregnancy and opioid misuse: A qualitative study with women and healthcare providers in Ohio.

BACKGROUND: Women with histories of opioid misuse face drug-related stigma, which can be amplified during pregnancy. While women are often blamed for their drug use and urged to change, the social contexts that create and reinforce stigma are largely unchallenged. Drawing on a multidimensional model of stigma, we examine how stigma manifested across women's pregnancy journeys to shape access and quality of care. METHODS: We triangulate in-depth interviews with 28 women with histories of opioid misuse who were pregnant or recently gave birth and 18 healthcare providers in Ohio. Thematic analys
