INTRODUCTION: Low back pain is a common disorder that generates many medical consultations. Magnetic Resonance Imaging (MRI) is commonly used in the clinical management of some of these patients. However, the cost of inappropriate MRI use is high, so there is a need to develop guidelines to help physicians make correct decisions and optimize available resources.
OBJECTIVE: To determine the main clinical indications for MRI scanning in adults with low back pain.
MATERIAL AND METHODS: The RAND/UCLA appropriateness method was used: After a systematic review (May 2012), a list of the clinical in

OBJECTIVE: To analyze all cases of childhood neuropathies (under 14 years of age) and report on their profile, pattern, clinical features and management.
METHODS: Children with acute flaccid paralysis, longstanding weakness of extremities, neuroregression and children receiving anti cancer drugs with symptoms suggestive of neuropathy were evaluated for evidence of peripheral neuropathy. The evaluation of children with acute flaccid paralysis was a prospective study from January 1992 through to December 2000. The rest of the patients were studied retrospectively from the hospital medical recor

Although meninges represent a major site of biosynthesis, beta-trace protein (beta-trace) has not been studied in the cerebrospinal fluid (CSF) of meningitis patients. We measured beta-trace in lumbar CSF of normal controls (n = 27) and in patients with various neurological diseases (n = 92) by an immunonephelometric assay. The mean concentration of beta-trace in CSF of control patients was 16.6+/-3.6 mg/l. In bacterial meningitis (n = 41), CSF beta-trace was significantly decreased (8.7+/-3.9 mg/l; P< 0.001), whereas in spinal canal stenosis it was elevated (29.2+/-10.3 mg/l; P= 0.002). In vi

A 28-year-old housewife developed motor and sensory neuropathy in both limbs with facial nerve paralysis and decreased taste sensation, two weeks after cough and sore throat with slight fever. On laboratory examination, mild increase of transaminases was found. Enzyme-linked immunosorbent assay showed that the serum had markedly high titers of IgG antibodies to GM2 (1:3,200) and Ga1NAc-GD1a (1:1,600). Serum IgM-EIA index and CF titer of cytomegalovirus were markedly high. In cerebrospinal fluid examination, protein was increased (110 mg/dl). Blink reflex indicated involvement of bilateral faci

The pathogenesis of the Guillain-Barre syndrome (GBS) is felt to involve alterations in cellular and humoral immune responses. We report the case of a 44-year-old female with stage IV breast cancer who developed GBS 2 days after receiving high dose chemotherapy with bone marrow and peripheral progenitor cell support, and granulocyte-macrophage colony stimulating factor (GM-CSF). Her clinical status stabilized with plasmapheresis. This report lends further support to hypotheses regarding cellular immune alterations in the pathogenesis of GBS.

Whether the conduction abnormalities and lesions underlying them are randomly distributed throughout the peripheral nervous system (PNS) or certain regions are selectively more vulnerable to attack is an unresolved question in Guillain-Barré syndrome (GBS). To examine this question, 15 cases of GBS were comprehensively examined using electrophysiological techniques which allowed close examination, quantitation and comparison of conduction abnormalities in motor and sensory fibres of the upper limb between the spinal cord and the distal extremities of the nerve fibres. Comparison of these studi

Since plasma is largely involved in autoimmune diseases, plasma exchanges have been used to treat these patients. In the first part of this paper the authors review the indications for plasmapheresis, which they divide into two groups: autoimmune diseases proper, and diseases with autoimmune phenomena. In the second part, the results available in the literature are analyzed. In many cases, it is extremely difficult to form an opinion, since the methods vary from one group of workers to the other. Unquestionably, improvement was observed in bullous pemphigus and pemphigoid, in Goodpasture's syn

A study was undertaken in order to establish the normal range of phrenic nerve latencies in children, to determine whether phrenic nerve stimulation can distinguish diaphragmatic palsy from eventration of the diaphragm, and to determine the effect of neuromuscular disorders on phrenic nerve latencies in children. Ninety-four children were examined, of whom 31 had neuromuscular disorder and 63 were controls. Among the controls, phrenic nerve latencies decreased from 32 weeks gestational age to six months postnatal age, despite an increase in stimulus-response distance. However, the latencies we

In a multicentre, randomised trial of prednisolone in acute polyneuropathy of undetermined aetiology (Guillain-Barré syndrome), 21 patients were treated with prednisolone (60 mg daily for one week, 40 mg daily for four days, and then 30 mg daily for three days) and 19 did not have steroid treatment. Patients were graded on a six-point scale by one of two neurologists who had no knowledge of the treatment schedule. Reassessment at one, three, and twelve months consistently showed greater improvement in the control than the prednisolone group but the only statistically significant result was in

BACKGROUND: Chronic inflammatory demyelinating polyneuropathy (CIDP) presents a significant therapeutic challenge, with up to 15% of patients being refractory to first-line treatments.
METHODS: Anti-B cell maturation antigen chimeric antigen receptor T (CAR-T) cell therapy was applied to two patients with highly relapsed and refractory CIDP, followed by safety and efficacy evaluation. Multi-omics analyses were performed on samples of peripheral blood mononuclear cells (PBMCs) collected before and after infusion.
FINDINGS: Both patients had no severe adverse events and achieved drug-free remi

The occurrence of chronic inflammatory demyelinating polyneuropathy (CIDP) related to coronavirus disease 2019 (COVID-19) has rarely been reported. We describe two patients who were diagnosed with CIDP after COVID-19 vaccination. A 72-year-old man presented with a progressive tingling sensation and weakness below both knees for two weeks. He had been vaccinated against COVID-19 (mRNA-1273 vaccine) a month before the appearance of symptoms. Demyelinating polyneuropathy was observed in the nerve conduction studies (NCS). Intravenous immunoglobulin (IVIg) was administered under the diagnosis of G

Neurofascin-155 (Nfasc155) is an essential glial cell adhesion molecule expressed in paranodal septate-like junctions of peripheral and central myelinated axons. The genetic deletion of Nfasc155 results in the loss of septate-like junctions and in conduction slowing. In humans, IgG4 antibodies against Nfasc155 are implicated in the pathogenesis of chronic inflammatory demyelinating polyneuropathy (CIDP). These antibodies are associated with an aggressive onset, a refractoriness to intravenous immunoglobulin, and tremor of possible cerebellar origin. Here, we examined the pathogenic effects of

Distinguishing patients with acute-onset chronic inflammatory demyelinating polyneuropathy from acute inflammatory demyelinating polyneuropathy prior to relapse is often challenging at the onset of their clinical presentation. In the present study, nerve excitability tests were used in conjunction with the clinical phenotype and disease staging, to differentiate between patients with acute-onset chronic inflammatory demyelinating polyneuropathy and patients with acute inflammatory demyelinating polyneuropathy at an early stage, with the aim to better guide treatment. Clinical assessment, stagi

OBJECTIVE: A subset of patients with fibromyalgia (FM) exhibit a large fiber demyelinating peripheral polyneuropathy akin to that seen in chronic inflammatory demyelinating polyneuropathy (CIDP). It has been suggested that this demyelinating process is likely to be immune mediated. Because it is known that similar large fiber neuropathic lesions may be associated with a cutaneous small fiber neuropathy, we sought to determine the prevalence of small fiber neuropathy, as measured by epidermal nerve fiber density (ENFD), in a series of patients with FM and clinically healthy control subjects.
