Pregnancy after hematopoietic stem cell transplant: an international multicenter study.

Maintenance of fertility is important to patients undergoing hematopoietic stem cell transplantation (HCT) because many are of childbearing age and treatment is frequently sterilizing. Pretransplant fertility preservation counselling is currently limited by a paucity of data. Pregnancy after transplant is an infrequent event and although small studies provide anecdotal information, interpretation of larger data sets can be confounded by lack of detail. In this multicenter study, patients who received a transplant between January 1995 and December 2015 who subsequently became pregnant/partners

Enablers and barriers to assisted reproduction and fertility services for Pacific peoples in Aotearoa New Zealand: a qualitative study.

The uptake of assisted reproductive technologies (ART) has increased rapidly in recent decades, and yet migrant and ethnic minority communities continue to experience barriers to accessing these services. In Aotearoa New Zealand, the Pacific diaspora have the lowest participation in ART services despite having the highest rates of infertility. The reasons for this phenomenon have yet to be fully explained. In this qualitative cross-sectional study, 13 participants (12 women, one man) self-identifying as Pacific people who had experienced infertility participated in 45-90-minute talanoa (semi-s

Baseline Cardiometabolic, Hepatic and Renal Profile of Infertile Women and Pregnancy Status Following Assisted Reproduction Technology Procedure.

BACKGROUND: Routinely, the focus of pre-assisted reproduction technology (ART) assessments is ovarian reserve hormones and semen parameters. However, a growing body of research suggests that systemic metabolic health is a critical determinant of reproductive outcomes. The study therefore sought to evaluate disparities in baseline cardiometabolic, hepatic, renal and haematologic factors in infertile women and their pregnancy status following the ART procedure. METHODS: This cross-sectional baseline assessment with prospective follow-up study was conducted at The Chosen Hospital and Fertility C

The landscape of chromosomal aberrations in couples seeking assisted reproductive treatment.

STUDY QUESTION: What are the prevalence, type, and stratified risks (reproductive failure subtype, sex, age, semen quality, and prior adverse pregnancy events) of chromosomal aberrations in couples seeking ART treatment in a large-scale cohort study? SUMMARY ANSWER: The prevalence of chromosomal aberrations in couples with reproductive failure was 3.42%, with a higher prevalence in younger individuals, men with poorer semen quality, and those experiencing multiple adverse pregnancy outcomes. WHAT IS KNOWN ALREADY: Reproductive failure is a global health issue, with chromosomal aberrations pl

Evidence-based interventions to restore or improve female fertility in women aged 30-42 years: a systematic review by etiology and evidence level.

IMPORTANCE: Female infertility affects approximately one in six couples worldwide and disproportionately impacts women aged 30-42 years, yet evidence on which interventions improve clinically meaningful outcomes (live birth and safety) is heterogeneous. OBJECTIVE: The aim of this study was to synthesize evidence on interventions that restore or improve fertility in women aged 30-42 years, grouped by etiology. EVIDENCE REVIEW: We conducted a PRISMA (Preferred Reporting Items for Systematic reviews and Meta-Analyses)-compliant systematic review (PROSPERO CRD420251109428). PubMed and Scopus wer

Transcript profiling and gene regulation of the human pre-implantation embryo: parental effects and impact of ARTs.

BACKGROUND: Infertility is a growing global challenge, with ARTs significantly improving birth rates for infertile couples. However, ART conceptions are associated with a higher risk of negative obstetrical and perinatal outcomes, with potential long-term effects on offspring health. Many pre-implantation embryos exhibit abnormal morphokinetics, implantation failure, or arrested development. ART procedures and parental factors are suspected to perturb the embryonic transcriptome, potentially affecting molecular and epigenetic events during gametogenesis and early development. The timing and me

Mental health in expectant mothers after medically assisted reproduction and spontaneous conception: the nuMoM2b study.

BACKGROUND: Mental health disorders are common during pregnancy and affect up to 20% of individuals. Women who conceive via medically assisted reproduction (MAR) may face increased psychological stress due to treatment burden and uncertainty, potentially elevating their risk of mental health conditions. However, evidence comparing mental health in women conceiving via MAR versus spontaneous conception remains inconsistent. METHODS: We analyzed data from 9289 nulliparous women in the prospective US-based nuMoM2b cohort, including 196 who conceived via assisted reproductive technology (ART), 17

Changes in inflammatory factor levels in follicular fluid of patients with PCOS and their impact on assisted reproductive outcomes: A systematic review and meta-analysis.

This systematic review and meta-analysis examines the association between inflammatory factors in the follicular fluid and assisted reproductive technology (ART) outcomes among women with polycystic ovary syndrome (PCOS). Multiple databases were comprehensively searched, which identified 37 studies, including 24 case-control, 7 cohort, and 6 cross-sectional studies, with a total of 1421 patients with PCOS and 1458 controls. Methodological quality was rated as high in 86.49% (32/37) of included studies. The meta-analysis revealed significantly higher levels of proinflammatory cytokines, includi

Male- and female-factor infertility and partners' quality of life during assisted reproductive technology: A dyadic actor-partner analysis.

BACKGROUND: Infertility constitutes a dyadic stressor, as it inherently involves both members of a couple; nevertheless, its psychological impact is frequently evaluated from an individual perspective. We tested whether etiologic diagnosis (male and female factors) is associated with both partners' quality of life (QoL) during assisted reproductive technology (ART). METHODS: We conducted a retrospective cross-sectional study at the assisted reproduction center of a tertiary university hospital in southern Taiwan, enrolling married heterosexual couples undergoing ART who completed the Fertilit

Risk of non-reproductive cancer in men from couples conceiving through assisted reproduction: a Swedish nation-based register study.

Whereas the link between impaired male fertility and risk of testicular and prostate cancer is well established, studies investigating non-reproductive cancer risk among men with impaired reproductive function are scarce and show conflicting results. The aim of the study was to compare the risk of developing non-reproductive cancers in men achieving paternity through assisted reproduction as compared to those conceiving naturally. All first-time fathers n = 1 137 829 in Sweden during the period January 1994 to December 2014 were included and followed from the time of conception until date of c

Hysteroscopic platelet-rich plasma and medically assisted reproduction outcomes: a systematic review and SWOT analysis.

BACKGROUND: Platelet-rich plasma (PRP) has been proposed as an adjuvant treatment in reproductive medicine. While most evidence refers to blind intrauterine instillation, subendometrial administration under hysteroscopic guidance allows targeted delivery under direct visualisation. This systematic review aimed to synthesise the available evidence on hysteroscopic PRP administration and its impact on clinical medically assisted reproduction (MAR) outcomes. METHODS: A systematic search was conducted from inception to December 2025 across major databases. Studies were included if they evaluated

Experiences of family building counseling and perceptions of reproductive technology among adults with sickle cell disease.

INTRODUCTION: Sickle cell disease (SCD) is an autosomal recessive blood disorder that impacts about 100,000 Americans. With increasing life expectancy and more people with SCD reaching reproductive age, there is growing evidence that SCD and some of its therapies decrease fertility. While partner testing and assisted reproductive technology (ART) can enable people with SCD to choose if and how to have biological children, their use remains limited. In this study, we aimed to explore how people with SCD understand the potential fertility implications of SCD and their perceptions of partner test

Theranostic potential of extracellular vesicles in reproductive tracts: implications for recurrent implantation failure.

Embryo implantation is a critical step at the beginning of pregnancy, occurring during a specific and limited period known as the "implantation window". Successful implantation involves various signaling pathways and molecular interactions. Recent studies have highlighted the importance of extracellular vesicles (EVs) in mediating these complex interactions. Different cell types release EVs to transfer signals to other cells or tissues. Additionally, emerging evidence suggests that EVs regulate signaling between the developing embryo and endometrium. In this review, we summarize current findin

Special considerations in assisted reproductive technology for patients with cardiovascular disease.

Many individuals with cardiovascular disease can benefit from the availability of assisted reproductive technologies (ARTs) for family building. Its use for this population may be because of underlying infertility, a need for fertility preservation before disease worsening or heart transplant, a genetic cardiac condition they believe to avoid passing to offspring, or the need to use a gestational surrogate in the setting of cardiovascular contraindications to pregnancy. Cardiovascular disease exists on a spectrum, from mild and common diseases that pose minimal threat to maternal health during

The impact of thyroid function, autoimmunity, and subsets of Th cells in follicular fluid on ovarian reserve and embryo quality in assisted reproductive technology (ART).

Both thyroid function and autoimmunity have the potential to influence fertility and pregnancy outcomes; however, the underlying mechanisms remain inadequately understood. The purpose of this study is to explore the effects of thyroid function, autoimmunity, and subsets of Th cells in follicular fluid on ovarian reserve and embryo quality. In this prospective cohort study, 104 patients were enrolled and divided into an observation group (TSH >2.5 μIU/mL or diagnosed hypothyroidism) and a control group, each with 52 patients. Follicular fluid samples from infertility treatments were analyzed fo
