Investigating the multifactorial etiology of supraspinatus tendon tears.

The purpose of this study was to develop a multivariable model to determine the extent to which a combination of etiological factors is associated with supraspinatus tendon tears. Fifty-four asymptomatic individuals (55 ± 4 years) underwent testing of their dominant shoulder. Diagnostic ultrasound was used to assess for a supraspinatus tendon tear. The etiological factors investigated included demographics (age and sex), tendon impingement during shoulder motion (via biplane videoradiography), glenohumeral morphology (via computed tomography imaging), family history of a tear (via self-report)

Bicipital groove cross-sectional area on ultrasonography: Does a correlation to intra-articular tendon pathology exist?

BACKGROUND: The etiology and relevance of long head of biceps tendon (LHBT) pathology is debated. As it can have important therapeutic consequences and physical examination can be misleading, various morphologic parameters have been described to try to predict it. HYPOTHESIS: We hypothesized that bicipital groove cross-sectional area (CSA), as assessed by ultrasonography, could be related to intra-articular tendon pathology. MATERIAL AND METHODS: Fifty-eight consecutive consenting patients who underwent arthroscopic shoulder surgery at our hospital were selected. Diagnosis consisted mainly o

[Angiosarcoma of the shoulder revealed by previous trauma: about a case and literature review].

Angiosarcoma is a rare tumor which can affect multiple sites. However, it most commonly arises from the skin. Then symptoms have been associated with clinical polymorphism. This tumor has a poor prognosis due to its high tendency to local recurrence and distant metastases. We here report the case of a 72-year old man in whom an angiosarcoma of the shoulder arose from a trauma which occurred one year before it was accidentally detected. The patient presented with persistent pain in a swelling area at the posterior portion of the shoulder involved in the trauma. Clinical examination showed previ

Anterior Instability: What to Look for.

Most first-time anterior glenohumeral dislocations occur as the result of trauma. Many patients suffer recurrent episodes of anterior shoulder instability (ASI). The anatomy and biomechanics of ASI is addressed, as is the pathophysiology of capsulolabral injury. The roles of imaging modalities are described, including computed tomography (CT) and MR imaging with the additional value of arthrography and specialized imaging positions. Advances in 3D CT and MR imaging particularly with respect to the quantification of humeral and glenoid bone loss is discussed. The concepts of engaging and noneng

Editorial Commentary: Yet Another Arrow in the Quiver for Surgical Treatment of the Rotator Cuff-Deficient Shoulder: Will It Fly Fast and Far or Fall Short Like Other Options Have?

Rotator cuff tears are common. When indicated, surgical repair is a highly successful procedure. There are circumstances when there is not enough tendon to perform an anatomic repair because of tear size, retraction, and/or atrophy. This clinical scenario, massive irreparable rotator cuff tear, has no perfect solution. Many options exist in treating massive irreparable rotator cuff tears: partial tendon repairs, debridement, tuberoplasty, intercalary allograft repairs, tendon transfers, superior capsular reconstruction, hemiarthroplasty, arthrodesis, and reverse total shoulder arthroplasty. Arthroscopic posterior portal closure.

Posterior "working" portals in arthroscopic posterior stabilization could result in defects of the posterior capsule if not repaired. We describe a single portal technique used to close the posterior portal defect after arthroscopic stabilization. It is a safe and easy-to-perform technique, which could strengthen the structural integrity of the repair.

Visualization of the anatomy of the rotator interval and bicipital sheath.

A systematic evaluation of the glenohumeral joint should be routinely performed with all shoulder arthroscopy and include all anatomic areas. However, to date, less attention has been given to the insertion of the subscapularis tendon, superior glenohumeral ligament (SGHL), and medial head of the coracohumeral ligament (MCHL). This article outlines arthroscopic techniques that may aid in the evaluation of the anatomy of the rotator interval and bicipital sheath. In this apical region, the CHL contributes fibers to the SGHL (forming the internal reflection of the bicipital groove-SGHL/CHL compl

Labrocapsular ligamentous complex of the shoulder: normal anatomy, anatomic variation, and pitfalls of MR imaging and MR arthrography.

Magnetic resonance (MR) imaging is a useful modality for evaluating the labrocapsular ligamentous complex (LCLC) of the shoulder. MR arthrography is an important and occasionally indispensable supplementary modality for accurate differentiation between normal and deranged glenohumeral joints. Because of the joint distention that occurs during MR arthrography, it is especially helpful in detecting subtle capsular derangement, as occurs in patients with atraumatic instability. Also, some of the pitfalls associated with MR imaging of the LCLC are less likely to occur with MR arthrography. Radiolo

Arthroscopic treatment of impingement of the shoulder.

Arthroscopic subacromial decompression is an alternative to open anterior acromioplasty in patients who have advanced stage II and selected stage III impingement syndromes. This procedure can be performed in an outpatient setting and postoperative morbidity is minimal; however, the procedure is technically demanding. Because the deltoid is not detached, immediate full range of active motion is possible, allowing an early return to activities of daily living. Substantial pain relief can be anticipated in a large majority of patients.

Anterior glenohumeral instability.

The understanding of and approach to anterior shoulder instability has changed and improved dramatically in recent years. It is now accepted that a shoulder can subluxate as well as dislocate and that chronic instability may or may not be caused by an initial traumatic event. An anteriorly unstable shoulder also can be unstable inferiorly and/or posteriorly (multi-directional instability). The author's diagnostic acumen has increased with the addition of glenohumeral axillary arthrotomography, glenohumeral CT arthrography, glenohumeral arthroscopy, and other studies. Surgical treatment has mov

Occult anterior subluxations of the shoulder in noncontact sports.

Athletes participating in noncontact sports involving abduction and external rotation of the shoulder (e.g., throwing) may develop occult recurrent subluxation manifested only as pain. The lack of contact trauma preceding symptoms, the failure of the athlete to appreciate the instability, the relative rarity that the lesion has been previously recognized and reported, and the lack of objective evidence of instability often lead to incorrect diagnosis by the physician. We report 30 shoulders in 28 patients with this lesion. Nineteen of these patients had been originally seen by other physicians

Posterior shoulder instability.

Understanding the anatomic restraints to posterior shoulder instability and the resulting pathophysiology helps the treating physician make a correct diagnosis and formulate an appropriate treatment plan. A nonoperative program directed at reducing pain and increasing stability through comprehensive shoulder strengthening methods has generally been successful in treating recurrent posterior shoulder subluxation. Surgical options for treatment are reserved for those patients who fail to recognize improvement after six months of therapy and have no evidence of a psychological disturbance as the

Minimum 10-Year Outcomes After Revision Anatomic Coracoclavicular Ligament Reconstruction for Acromioclavicular Joint Instability.

BACKGROUND: Revision surgery in cases of previously failed primary acromioclavicular (AC) joint stabilization remains challenging mainly because of anatomic alterations or technical difficulties. However, anatomic coracoclavicular ligament reconstruction (ACCR) has been shown to achieve encouraging biomechanical, clinical, and radiographic short-term to midterm results. PURPOSE: To evaluate the clinical and radiographic long-term outcomes of patients undergoing revision ACCR after failed operative treatment for type III through V AC joint injuries with a minimum 10-year follow-up. Chronic Overuse Injuries in Young Athletes-Upper Extremity.

Young athletes are uniquely susceptible to overuse injuries of the upper extremity because their open physes and apophyses are structurally weaker than adjacent bone, ligament, and tendon. Repetitive sport-specific loading-torsional stress in throwers, axial compression in gymnasts and climbers-can outpace the repair capacity of growing tissues, producing a spectrum of physeal and apophyseal stress injuries from widening and sclerosis to frank epiphysiolysis. This review examines the mechanisms, clinical presentation, and imaging findings of chronic overuse injuries at key upper extremity site

[One hundred years of orthopedics in the Netherlands. X. Sports injuries].

Sports injuries result from frequently repeated similar movements performed with submaximal force. In practice the term is also used, incorrectly, for many other injuries sustained during, or even outside, the practising of sports. Running may lead to injuries of muscles (rupture, chronic compartment syndrome), of tendons (peritendinitis, tendinosis, partial rupture, insertion tendinitis), of bone (stress fracture) and of cartilage (athrosis). Jumping mostly puts the ankle at risk, especially of development of an anterior or posterior impingement syndrome. Throwing puts much strain on the shou
