The sleep apnoea syndrome is reviewed, defined, and classified. Particular emphasis is placed on the identification of a patient population that is prone to sleep apnoea and the diseases and syndromes that are associated with it. For anaesthetists, direct enquiry into daytime and nighttime sleep abnormalities and careful examination of the upper airway are important for preoperative detection of these patients and especially patients with obstructive sleep apnoea who might present for anaesthesia and operation. A typical case is reported and details of the preoperative, peroperative and postop…

The relationship between arrhythmias and sleep states was investigated in patients with night-type arrhythmias, which were detected by 24-hour electrocardiogram recordings using the Holter Avionics system. These patients were examined using all-night polygraphic recordings and sleep states were divided in to 5 groups according to the standard sleep stage scoring manual as stage I, II, III, IV, and REM sleep. Patients were divided into 5 groups: (1) grade 2 ventricular premature contractions (VPCs), (2) grade 4a VPCs, (3) grade 4b VPCs, (4) supraventricular premature contractions, and (5) parox…

In treating the symptomatology of patients who are chronically ill, it is likely that the physician will encounter a significant number of patients who have sleep disorders. This article has described an approach for the assessment, differential diagnosis, and clinical management of the most prevalent sleep disorders, which are often chronic in nature and have significant psychosocial or medical consequences for the patient and his or her family. We have emphasized the importance for the physician to complement his general medical and diagnostic skills by taking a careful sleep history from th…

Three- to 4-hour polygraphic sleep studies were carried out in 16 infants aged between 1 and 6 months during and after recovery from acute bronchiolitis. During bronchiolitis 35% of total sleep time was active sleep compared with 31% after recovery. Respiration rate was increased during bronchiolitis and was higher in active sleep and quiet sleep irrespective of the stage of the illness. Apnoeic pauses were invariably shorter than 15 seconds, the mean duration for active sleep and quiet sleep being similar during infection and after recovery. Apnoeic episodes were central in type and generally…

Sleep and body movements were studied in 2 hydranencephalic neonates using visual analysis and polygraphic recording. They manifested periodic alternation of active and quiet sleep with peculiar movements of bilateral synchronized patterns. Generalized phasic movements were significantly increased and localized tonic movements were reduced. In view of the complete absence of cerebral hemisphere in this disorder, the movement of bilaterally synchronized pattern or increase of generalized phasic movement are considered to be generated from the basal ganglia and/or brain stem without cerebral hem…

All night sleep EEG, EKG, and respiration were recorded from six young men during 2 nights at sea level and 4 nonconsecutive nights at high altitude (14.110 ft., 4301 m). Sleep at high altitude was chraacterized initially by a significant decrease in Stages 3 and 4, a significant increase in the number of arousals, and a trend towards more time spent awake. In terms of actual time spent sleep, however, a relatively good night's sleep was obtained, which suggests that the objective sleep disturbance was not commensurate with the marked subjective complaints of sleeplessness. Periodic respiratio…

Two experiments compatible with that used to investigate the effect of clinical doses (200 mg.) of amylobarbitone were set up to investigate … g withdrawal records were obtained.Chloral hydrate was found to depress rapid eye movement (R.E.M.) sleep appreciably though less consistently than amylobarbitone. No withdrawal R.E.M. sleep rebound was found.Neither dichloralphenazone nor Mandrax was found consistently to depress R.E.M. sleep, though occasional nights when R.E.M. sleep was low occurred …

Warfighter readiness encompasses personnel, training, equipment and supplies, and maintenance and necessitates service members operate physically and mentally at the highest levels. Total force fitness focuses on eight key domains to support service members' fitness and well-being, with a strong focus on physical, psychological, and social functioning, commonly referred to as biopsychosocial factors, to ensure optimal readiness. Due to its role in biopsychosocial functioning, adequate sleep is paramount for mission success. Yet, many service members are chronically sleep deprived for a myriad …

Obstructive sleep apnea (OSA) is a highly heterogeneous disorder of sleep-related breathing, and intermittent hypoxia during sleep constitutes a key pathophysiological mechanism underlying the increased risk of cardiovascular disease (CVD) in affected individuals. The sleep breathing impairment index (SBII) is a novel, multidimensional index for assessing OSA severity. By integrating the frequency and duration of respiratory events along with the corresponding oxygen desaturation area, SBII quantitatively captures both the occurrence of respiratory disturbances and the associated hypoxemic bur…
