Staged extensor tendon reconstruction in the finger.

Staged extensor tendon reconstruction using a silicone implant followed by tendon grafting was done to restore proximal interphalangeal (PIP) joint extension in 6 fingers with severe injuries to the dorsal skin and extensor mechanism. Abrasions to the joint capsule and cortical surfaces were also present. To avoid finger stiffness, the reconstruction was delayed and range of motion exercises were initiated early. The skin injury was managed by split-thickness skin grafting or allowed to heal by secondary intention to avoid prolonged immobilization. During surgery, the peritendinous fascia of t

[Treatment problems in old and recent rupture of the lateral knee ligament].

The authors have treated 31 fresh ruptures of the lateral ligament and 104 cases of chronic instability of the lateral side of the knee. In recent ruptures the results obtained after surgical repair were not as gratifying as those obtained after injuries of the medial ligament. The cases operated upon were tears of the lateral ligament associated with ruptures of one or both cruciate ligaments. The frequency of rupture of the popliteus tendon is emphasized. The authors recommend, when necessary, a double surgical approach both laterally and posteriorly and plaster cast immobilization for more

The safest location for steroid injection in the treatment of carpal tunnel syndrome.

Steroid injections are routinely performed for carpal tunnel syndrome. Direct needle injury of the median nerve is the major complication of these injections. The safest location of the injection remains controversial. The purpose of this study is to define safe guidelines to avoid nerve injury. The distances between the Median nerve, Palmaris Longus, Flexor Carpi Ulnaris and Flexor Carpi Radialis tendons were measured pre-operatively, 1cm proximal to the distal wrist crease in 93 endoscopic carpal tunnel releases. We found that the median nerve extended ulnarly beyond the Palmaris Longus tend

Pitfalls and How to Avoid Misdiagnosis in Magnetic Resonance Imaging of the Ankle and Foot in Athletes.

Magnetic resonance imaging is a vital tool in sports medicine for evaluating ankle and foot injuries in athletes, offering detailed insights into bone, cartilage, ligament, and tendon pathology. However, the findings in athletes, especially those who are asymptomatic, can present pitfalls that may lead to misdiagnosis and unnecessary treatment. Common findings such as bone marrow edema, joint effusions, and tendon sheath fluid are frequently observed in athletes without symptoms and may represent physiologic adaptations rather than pathology. This article emphasizes the importance of correlatin

The use of amniotic tissue-derived products in orthopedic surgery: A narrative review.

Amniotic-derived products have been used for decades in various medical subspecialties and have proven to be a safe method of allograft tissue transplantation. These products have shown promising preclinical and early clinical results in the treatment of tendon/ligament injuries, cartilage defects, and osteoarthritis. The therapeutic benefits of amniotic-derived products are likely due to intrinsic properties, such as their structure as an extracellular matrix and concentration of growth factors, as well as anti-inflammatory, antifibrotic, and antimicrobial molecules. We performed a narrative

Quadriceps reconstruction with suture anchor and Achilles allograft combination in quadriceps tendon re-rupture after primary surgical repair: A novel technique.

Quadriceps ruptures are one of the pathological conditions of the knee extensor mechanism, accounting for 3% of all tendon injuries. These injuries cause substantial disability of the extensor mechanism. Primary repair is the treatment of choice in acute presentation. In the setting of chronic conditions, the treatment becomes more challenging. Available surgical options include lengthening procedures, and reconstruction with auto graft or allografts. The traditional Scuderi and Codivilla techniques are challenging to perform in degenerative or traumatic retracted ruptures. There is no standar…

Avulsion Fractures at the Base of the 2(nd) Metacarpal Due to the Extensor Carpi Radialis Longus Tendon: A Case Report and Review of the Literature.

INTRODUCTION: Simultaneous contraction of the extensor carpi radialis longus (ECRL) with forced hyperflexion of the wrist can result in avulsion of the tendon and its bony attachment at its insertion at the dorsum of the base of the second metacarpal. This is a rare and often unreported fracture pattern. CASE PRESENTATION: We present a 31- year- old male who sustained a hyperflexion injury. He was managed surgically and had good post-operative outcomes. A literature search revealed 16 papers covering 18 cases of similar injuries. 12 were initially managed surgically and 6 were managed conserv

Repair of the flexor pollicis longus tendon in children.

This is a retrospective study of primary repairs of flexor pollicis longus in 16 children under 15 years of age. Patients with injuries to the median or ulnar nerve at the wrist, crush injuries, skin loss or fracture were excluded. Repairs were carried out within 24 hours using a modified Kessler technique. The mean follow-up was for two years. The final results were evaluated using the criteria of Buck-Gramko and Tubiana. They were good or excellent in all except one patient who had a secondary tendon rupture. When compared with the non-injured thumb, however, there was a significant decrease

Electrophysiologic basis of dynamic extensor splinting.

The use of dynamic extension splinting in the rehabilitation of extensor tendon injuries is gaining interest because of its apparent ability to improve clinical results. With such splinting, the extensor musculature is theoretically quiescent during both active flexion and rubber band-mediated extension; the gliding produced thereby decreases adhesion formation. Our initial intent was to perform electromyographic studies on volunteers in dynamic extension splinting to test this electrophysiologic principle. It was found, however, that only 16% of normal volunteers have quiescent extensor digit…

SLAP lesions of the glenoid labrum: CT arthrographic and arthroscopic correlation.

Tears in the superior part of the glenoid labrum in the anterior and posterior directions (SLAP, superior labrum anterior, posterior) result from injuries that place excessive stress on the tendon of the long head of the biceps brachii muscle. The intimate relationship of the tendon and the superior glenoid labrum promotes injury to the latter structure. Four types of SLAP lesions are seen at arthroscopy. Images from 17 patients with surgically proved SLAP lesions who had undergone preoperative computed tomographic (CT) arthrography were retrospectively reviewed to evaluate the value of CT art

Flexor Pollicis Longus Tendon Rupture after Distal Radius Volar Plating and Reconstruction with Interpositional Palmaris Tendon Graft: A Case Report.

INTRODUCTION: Distal radius fractures constitute one of the most prevalent upper extremity orthopedic injuries, predominantly resulting from falls onto an outstretched hand with an extended wrist. Despite volar locking plate fixation emerging as the gold standard treatment for distal radius fractures, it is associated with well-documented complications, such as flexor pollicis longus (FPL) tendon irritation and even rupture. The surgical management of FPL tendon rupture following volar plate fixation poses significant challenges due to hardware-induced tendon attrition and compromised surgical

MR features of longitudinal tears of the peroneus brevis tendon.

OBJECTIVE: Our objective is to describe the characteristic MR imaging features of longitudinal tears of the peroneus brevis tendon and to describe pathologic conditions and normal variants that are associated with these tears which may require surgical intervention at the time of primary tendon repair. MATERIALS AND METHODS: We retrospectively reviewed 10 MR images of asymptomatic volunteers and 31 MR images of 27 patients with evidence of longitudinal splits of the peroneus brevis tendon. Seven of these cases were surgically proven. In addition, we assessed five pathologic conditions and nor

Managing complex hand, wrist, and forearm injuries with WALANT: A case-based technical report.

Wide-Awake Local Anaesthesia No Tourniquet (WALANT) is an effective and increasingly utilised method of anaesthesia for the surgical treatment of injuries to the hand, wrist, and forearm. Despite its proven safety, efficacy, and cost-effectiveness, the adoption of WALANT has been limited, particularly in Western healthcare systems where general and regional anaesthesia predominate, especially in complex injuries. We report the application of WALANT in a variety of hand surgical procedures for the treatment of complex injuries ranging from complex fractures to multiple soft tissue injuries in a

Hand trauma in shop class.

The environment and equipment used in shop class are potential sources of serious injury. There has been little published to date on injuries sustained in shop class, with no reports examining injuries to the hand. The authors report a case series collected from a health records database at a pediatric and hand surgery referral center. Fifteen patients who sustained injuries to their wrist or hand in shop class were identified. Sixty percent of the injuries were caused by table saws. Eighty percent required treatment from a hand surgeon. Sixty-seven percent of patients sustained a serious inju

Efficacy of a new flossing intervention using a floss band for severe adhesion after digit tendon repair.

BACKGROUND: Tendon adhesions after hand surgery can severely impair motion and function. Although early mobilization is preferred, re-ruptures or complex trauma cases often require immobilization, increasing the risk of adhesion. PURPOSE: This report presents two cases-one involving flexor pollicis longus (FPL) tendon re-rupture and another with multiple extensor tendon injuries-treated with a novel flossing intervention aimed at improving tendon gliding and soft tissue flexibility. METHODS: In both cases, flossing therapy using a rubber compression band was introduced at 8 weeks postoperati…

Technique tip: EDL-to-EHL double loop transfer for extensor hallucis longus reconstruction.

BACKGROUND: Extensor hallucis longus (EHL) tendon injuries often occur in the setting of lacerations to the dorsum of the foot. End-to-end repair is advocated in acute lacerations, or in chronic cases when the tendon edges are suitable for tension free repair. Reconstruction with allograft or autograft is advocated for cases not amenable to a primary direct repair. This is often seen in cases with tendon retraction and more commonly in the chronic setting. In many countries the use of allograft is very limited or unavailable making reconstruction with autograft and tendon transfers the primary…
