In the Boytchev technique the conjoined tendons of the coracobrachialis and short head of the biceps, together with the tendon of the pectoralis minor and the detached tip of the coracoid process are rerouted behind the subscapularis muscle, and reattached to the coracoid process with a screw. During a 10-year period 37 patients with recurrent anterior dislocation underwent a Boytchev procedure. Twenty-six patients (with 27 shoulders) were available to follow-up. In 120 cases of severe, neglected or recurrent club foot, a modified conservative method of treatment has yielded encouraging results. The method consists of blind calcaneal fasciotomy, repositioning of the foot in a corrected position and plaster cast application from the upper thigh to the toes. Nineteen recurrences in this series were treated by Dwyer's calcaneal osteotomy, or medial soft tissue release and/or tendon transposition.

There are currently no surgical models for learning index finger pollicization. This led us to develop and evaluate a cadaveric model for index finger pollicization. Recurrent dislocation of the elbow resulted from laxity of the lateral capsule and collateral ligament in four patients. An intra-articular lesion was usually also present. Elbow arthrotomy for removal of loose bodies and repair of the lateral capsuloligamentous structures are thought to offer an excellent prognosis for satisfactory relief from symptoms of recurrent dislocation. More complicated techniques, such as bone blocks or tendon transfers, are believed to be less effective and unnecessary.

Paralytic hip dislocations in 37 children with myelomeningoceles have been treated by open reduction, iliopsoas tendon transfer and various other procedures about the hip. At follow-up, less than half the patients were community ambulators and had maintained their reductions. The level of the spinal lesion, alignment of the lower extremities in relation to the trunk and pelvis and the presence of scoliosis including pelvic obliquity were more important factors in determining the patient's degree of function than the status of the hip reduction.

Congenital absence of the median-innervated intrinsic muscles and flexor pollicis longus was seen in varying degrees of severity in eleven hands of eight patients. Three members were involved in one family in which the anomaly was probably an autosomal dominant. There were no chromosomal abnormalities. Surgical treatment of seven hands, including release of the adduction contracture and transfer of the ring-finger superficialis tendon to provide opposition and to reinforce the ulnar collateral ligament of the metacarpophalangeal joint, resulted in significant improvement in pinch and grasp.
