Alveolar bone resorption following coronal versus apical approach in a mucoperiosteal flap surgery procedure in the rat mandible.

BACKGROUND: In several publications, we have reported that a distinct resorptive phase of alveolar bone developed during 3 weeks following elevation of a mucoperiosteal flap in rats by a coronal surgical approach. In the present study, we compared the alveolar bone loss after mucoperiosteal flap surgery by a coronal approach (COR) to a surgical approach incising the mucosa close to the tooth apices and separating the mucoperiosteum toward the tooth crown (apical approach, AP). METHODS: Eighteen Wistar rats were divided into 2 experimental groups; each group consisted of 9 rats. In the first g…

Implant or tooth? - A cost-time analysis of managing "unrestorable" teeth✰.

OBJECTIVES: Retaining and restoring severely compromised teeth with subcrestal defect extensions or removing and replacing them using implant-supported crowns (ISC) remains controversial, and economic analyses comparing both strategies remain scarce. We performed a cost-time analysis, comparing the expenditures for retaining "unrestorable" teeth using forced orthodontic extrusion and restoration (FOE) versus extraction and ISC, in a clinical prospective cohort study. METHODS: Forty-two patients (n = 21 per group) were enrolled from clinical routine at a university into this study. Direct medi

Restorative Alveolar Interface Management Combined With Tunnel Technique and Connective Tissue Graft: A Conservative Approach for Root Fracture Treatment.

INTRODUCTION: The infringement of supracrestal tissue attachment by subgingival restorative margins, extensive caries, and root fractures, can compromise the periodontal health, resulting in inflammation and loss of periodontal supporting tissues. This report describes a case of a root fracture on the upper left central incisor that was successfully treated using a conservative approach, by applying the restorative alveolar interface (RAI) management combined with tunnel technique and a subepithelial connective tissue graft (SCTG). CASE PRESENTATION: A 24-year-old male patient presented with

Tissue preservation through socket-shield technique and platelet-rich fibrin in immediate implant placement: A case report.

RATIONALE: In this report, a combination of socket-shield technique (SST) and platelet-rich fibrin (PRF) technique was used for immediate implant placement on a fractured central incisor. During the follow-up visit, cone beam computed tomography (CBCT) and clinical observation were used to evaluate the preservation outcome of peri-implant bone and gingiva. PATIENT CONCERNS: The patient was a 28-year-old healthy female patient who desired her fractured 21 to be replaced with an implant-supported single crown; the fractured 21 comprised a post-core crown with insufficient residual bone at the l

Management of a gunshot wound to the face resulting in a mandibular body fracture with burying of a bicuspid crown into the tongue.

Gunshot wounds to the maxillofacial region are unpredictable and run the gamut from minor injuries to severe mutilating and life threatening injuries. This patient although unfortunate to have been the victim of mistaken identify resulting in the gunshot wound, was fortunate that the bullet hit his bicuspid, which probably served to deflect its path away from vital structures, thus saving his life. This accounts for the buried bicuspid crown found in the midline of the body of the tongue. Rigid internal fixation of maxillofacial fractures minimizes risks to the airway that may occur if patient

Periodontal health as related to preformed crowns: report of case.

General studies have suggested that clinical gingivitis might be associated with radiographically detectalble deficiencies around the margins of stainless steel crowns, or with cement remaining in the gingival sulcus, or with a combination of these. The case of a nine-year-old girl reported here is an example of the high toleration potential of young periodontal tissues--even when a significant amount of zinc phosphate cement had been pushed into the lingual sulcus during cementation.

Re-evaluation of the age at death of immature fossil hominids.

We report here revised chronological ages at death of immature fossil hominids demonstrating for the first time that Plio-Pleistocene hominids had markedly abbreviated growth periods relative to modern man, similar to those of the modern great apes. Previous estimates of age at death for fossil hominids have principally been based on dental eruption, maturation and dental wear criteria for modern man and hence reflect their age in 'human' years. We are now able to estimate the absolute duration of permanent incisor crown formation by observing gross incremental growth features in enamel and th

Syndromic gingival fibromatosis associated with pathogenic variation in the voltage-gated potassium channel gene KCNH1: a case report and proposed treatment protocol.

BACKGROUND: The KCNH1 gene (OMIM #603,305) encodes a voltage-gated potassium channel primarily found in the central nervous system. Recent discoveries have linked pathogenic variations in this gene to Temple-Baraitser syndrome (TMBTS, OMIM #611,816) and Zimmermann-Laband syndrome (ZLS, OMIM #135,500). A common manifestation of these syndromes is gingival fibromatosis, which may partially or completely cover tooth crowns, leading in some cases to functional and aesthetic problems, as well as delayed tooth eruption. CASE PRESENTATION: A four-year-old boy and his parents first consulted for dela…

Pediatric maxillary fractures.

Pediatric craniofacial structures differ from those of adults in many ways. Because of these differences, management of pediatric craniofacial fractures is not the same as those in adults. The most important differences that have clinical relevance are the mechanical properties, craniofacial anatomy, healing capacity, and dental morphology. This article will review these key differences and the management of pediatric maxillary fractures. From the mechanical properties' perspective, pediatric bones are much more resilient than adult bones; as such, they undergo plastic deformation and ductile

Results of transplanting developing third molars as part of orthodontic space management. Part 1: clinical and radiographic results.

PATIENTS AND METHOD: The aim of the present study was to contribute to a more accurate indication and better integration of developing third molar transplantation into orthodontic space management, using the results gleaned from the authors' own pool of patients. For this purpose, a clinical and radiographic examination of 61 transplanted developing molars in 57 patients was carried out after an average of 3.3 years. RESULTS: The overall success rate was 85%. Failure was due in 10% of cases to impaired periodontal healing (inflammatory root resorption, ankylosis, or increased pocket probing d

Long-term assessment of early alveolar bone grafts using three-dimensional computer-assisted tomography: a pilot study.

Fifteen patients with complete unilateral cleft lip and palate who had primary alveolar bone grafting were studied with computer-assisted tomography at a mean age of 12 years. Keeping the maxillary alveolar crest parallel to the plane of the scan, 1.5-mm cuts of the maxilla were made from the infraorbital rim to the gingival third of the crowns of the teeth. A single operator reformatted the data into three-dimensional images using the Maxiview 3200 computer workstation. This allowed examination of the position, size, and spatial relationship of the grafted area and quantification of the amoun

Deep Margin Elevation and Lithium Disilicate Overlay Restoration of an Endodontically Treated Molar: A Case Report.

Restoring endodontically treated teeth with deep subgingival margins is challenging due to difficulties in isolation, moisture control, and adhesive cementation. Conventional approaches such as surgical crown lengthening are invasive and may compromise periodontal support. Deep margin elevation (DME) offers a conservative alternative by relocating the cervical margin coronally using composite resin. This case report describes a 38-year-old patient with a repeatedly debonded partial indirect restoration on tooth 47, which had previously undergone endodontic treatment. After removal of the exist

Odontogenic Pain Mimicking Trigeminal Neuralgia: The Diagnostic Value of Cone-Beam Computed Tomography: A Case Report.

BACKGROUND: Trigeminal neuralgia (TN) is characterized by severe and often relentless pain in the trigeminal nerve distribution. It is primarily a diagnosis of exclusion, highlighting the importance of a thorough workup to rule out treatable or secondary causes of atypical facial pain. CASE REPORT: An 89-year-old woman experienced 2 years of relentless odontogenic pain that had been misdiagnosed as TN. She was referred to endodonticsfor a cone-beam computed Tomography (CBCT) scan. CBCT led to the diagnosis of an abscess and nerve impingement due to an old crown post. Following tooth extractio

Treating apical fenestration in a previously endodontically treated tooth.

Apical fenestration is a defect in the alveolar cortical plate, exposing the root without involving the alveolar bone margin, often linked to trauma, periodontal disease, and orthodontic treatment, leading to symptoms such as pain and abscesses from endodontic infections. This case report describes managing a mucosal fenestration in an endodontically treated tooth with nonsurgical root canal therapy and periodontal surgery. A 44-year-old male presented with mucosal fenestration and pain in the upper front jaw due to trauma and an inadequately treated root canal. Examination revealed an ill-fit

Can coronectomy of wisdom teeth reduce the incidence of inferior dental nerve injury?

The aim of the study was to compare the surgical complications and neurosensory disturbance of coronectomy and conventional excision of lower third molars with roots in close proximity to inferior dental nerve. We conducted a randomized clinical trial of patients with lower third molars showing specific radiographic signs was conducted. Patients were randomized to undergo either coronectomy or total excision (control) of the lower third molar. It was concluded that coronectomy can significantly reduce the incidence of inferior dental nerve deficit when compared with conventional excision when
