Congenital central alveolar hypoventilation syndrome in six infants.

Six infants with congenital alveolar hypoventilation syndrome (CCHS) were seen and observed over several years. Two had an association of CCHS with Hirchsprung's disease. All infants were treated by tracheostomy and mechanical ventilation. Three infants have survived (including one with CCHS and Hirchsprung's disease). However, all survivors have required frequent rehospitalization. The infant with the longest survival (now 4 years of age) has developed significant daytime problems involving the "behavioral control" of ventilation. One infant was considered as a "near miss for sudden infant de

Mediastinal dissection. A multi-institutional assessment.

Mediastinal dissection was first described by George A. Sisson, MD, in 1962. However, there are no other reports in the literature describing the experience of other surgeons with this procedure. This study retrospectively analyzes the multi-institutional experience with mediastinal dissection to examine critically its effectiveness in controlling superior mediastinal neoplastic disease and to evaluate the complications of the procedure. This article describes 21 patients from three medical centers who have undergone mediastinal dissection by three surgeons for stomal recurrences. There are si

Tracheomalacia in an infant with multiplex congenita (Larsen's) syndrome.

An infant born with multiplex congenita (Larsen's) syndrome developed respiratory distress 30 days following tracheostomy for relief of upper airway obstruction. The infant had structural and functional abnormalities of the thoracic cage. Tracheobronchoscopy revealed excessive compliance of the trachea with a tendency for collapse of the tracheal rings and obliteration of the tracheal lumen. Continuous positive airway pressure in the range of 20-25 cm H2O was used to maintain patency of the tracheal lumen and assure adequate ventilation. Hemodynamic and pulmonary barometric complications often

Hypersomnolence and intermittent upper airway obstruction. Occurrence caused by micrognathia.

A patient with hypersomnolence, micrognathia, and respiratory insufficiency had been treated eight years for narcolepsy. Sleep apnea because of upper airway obstruction was found, and a tracheostomy was performed. Following this the hypersomnolence and respiratory insufficiency resolved. This case emphasizes the importance of carefully evaluating the condition of patients complaining of hypersomnolence to detect those with intermittent upper airway obstruction occurring during sleep.

Bilateral abductor paralysis: 30 years experience with arytenoidectomy.

Modifications of the technique originally reported in 1946 are presented, which may help in improving the percentage of successful decannulations. The importance of a change in the level of the vocal cord by this procedure is emphasized. The ten cautions to be followed are reviewed in doing this procedure.

[Treatment of alkyl phosphate poisoning with purified serum cholinesterase (author's transl)].

A 26-years-old man injected himself intramuscularly with 3.75 gram of demeton methyl in an attempt to commit suicide. After 36 hours severe symptoms of poisoning developed with loss of consciousness and respiratory arrest. As symptomatic treatment failed to improve his condition has given injections of purified serum cholinesterase. This was followed by a rise in the cholin esterase levels and the increased activity was accompanied by a rapid and definite improvement in the clinical findings. The results clearly indicate the usefulness of this method of treatment. Transfusions of fresh blood t

Factors influencing choice between tracheostomy and prolonged translaryngeal intubation in acute respiratory failure: a prospective study.

One of the problems of prolonged ventilatory therapy in acute respiratory failure (ARF) is the need to choose between tracheostomy after 48 to 72 hours of translaryngeal (TL) tracheal intubation or the continuous use of the TL tube for a period of 10 days. Too often the choice has been based on retrospective studies or personal preference. To investigate this problem prospectively, 52 adults in ARF were divided sequentially into 2 groups on their 3rd day of TL intubation. Patients in group I (G-I) retained the TL tube for a total of 11 days; those in group II (G-II) were tracheostomized on the

Rapid awake airway stabilization by direct tracheal intubation through an open cervical wound in severe laryngotracheal injury: a case report.

BACKGROUND: Penetrating neck injuries with open laryngotracheal disruption are rare but represent an extreme airway emergency in which stand … tubation and rapid sequence intubation were contraindicated due to distorted anatomy, and immediate tracheotomy was not feasible under awake conditions. Therefore, awake tracheal intubation through the open cervical wound was deliberately selected as a nonconventional airway management strategy to achieve rapid airway stabilization. Following successful airwa …
