Anidulafungin Versus Micafungin in the Treatment of Candidemia in Adult Patients.

BACKGROUND: Echinocandins are recommended for the treatment of invasive candidiasis and candidemia. However, there are few studies comparing anidulafungin and micafungin in terms of efficacy and safety. The objective of this study was to evaluate the clinical efficacy and safety between anidulafungin and micafungin treatment for adult patients with candidemia. METHODS: This retrospective cohort study performed on adult candidemia patients diagnosed from January 2006 through December 2018 at a tertiary medical center. The study subjects included adult patients ≥ 19 years with candidemia who we

Antifungal Resistance: a Concerning Trend for the Present and Future.

PURPOSE OF REVIEW: The global emergence of antifungal resistance among Candida spp. and Aspergillus spp. is a growing threat to public health, driven largely by the expanding use of antifungals in both the clinical and agricultural settings. As treatment options remain limited, understanding mechanisms and risk factors for antifungal resistance is essential to retaining their clinical utility. RECENT FINDINGS: Invasive candidiasis is increasingly caused by non-albicans Candida species with reduced susceptibility to first-line antifungals, making empiric treatment decisions difficult. Echinoca

Successful treatment with caspofungin of candiduria in a child with Wilms tumor; review of literature.

Symptomatic candiduria often occurs in patients with indwelling bladder catheters or immunocompromised host. Isolation of Candida in urine in high-risk patients should primarily be considered as a marker for candidemia. Hematological and genitourinary malignancies are one of the main risk factors associated with Candida urinary tract infections (CUTI). Fluconazole is a choice for initial treatment of CUTI, but it is fluctuate depending on the patient's condition including renal failure, site of urinary infection and Candida species. Antifungal Extraction by the Extracorporeal Membrane Oxygenation Circuit.

Invasive candidiasis is common and often fatal in patients supported with extracorporeal membrane oxygenation (ECMO), and treatment relies on optimal antifungal dosing. The ECMO circuit can extract drug and decrease drug exposure, placing the patient at risk of therapeutic failure. This ex vivo study determined the extraction of antifungal drugs by the ECMO circuit. Fluconazole and micafungin were studied separately in three closed-loop circuit configurations to isolate the impact of the oxygenator, hemofilter, and tubing on circuit extraction. Each circuit was primed with human blood, and flo

Diagnostic of Fungal Infections Related to Biofilms.

Fungal biofilm-related infections, most notably those caused by the Candida and Aspergillus genera, need to be diagnosed accurately and rapidly to avoid often unfavorable outcomes. Despite diagnosis of these infections is still based on the traditional histopathology and culture, the use of newer, rapid methods has enormously enhanced the diagnostic capability of a modern clinical mycology laboratory. Thus, while accurate species-level identification of fungal isolates can be achieved with turnaround times considerably shortened, nucleic acid-based or antigen-based detection methods can be con

Pga13 in Candida albicans is localized in the cell wall and influences cell surface properties, morphogenesis and virulence.

The fungal cell wall is an essential organelle required for maintaining cell integrity and also plays an important role in the primary interactions between pathogenic fungi and their hosts. PGA13 encodes a GPI protein in the human pathogen Candida albicans, which is highly up-regulated during cell wall regeneration in protoplasts. The Pga13 protein contains a unique tandem repeat, which is present five times and is characterized by conserved spacing between the four cysteine residues. Furthermore, the mature protein contains 38% serine and threonine residues, and therefore probably is a highly

Increase of virulence and its phenotypic traits in drug-resistant strains of Candida albicans.

There is concern about the rise of antifungal drug resistance, but little is known about comparative biological properties and pathogenicity of drug-resistant strains. We generated fluconazole (FLC; CO23 RFLC)- or micafungin (FK; CO23 RFK)-resistant strains of Candida albicans by treating a FLC- and FK-susceptible strain of this fungus (CO23 S) with stepwise-increasing concentrations of either drug. Molecular analyses showed that CO23 RFLC had acquired markedly increased expression of the drug-resistance efflux pump encoded by the MDR1 gene, whereas CO23 RFK had a homozygous mutation in the FS

[Central nervous system mycotic infections].

INTRODUCTION: There has been an increased number of cases of fungal diseases of the central nervous system (CNS) during the past few years. This paper reviews current literature about these conditions, with emphasis on recent advances on diagnosis and therapy. DEVELOPMENT: While some fungi may cause disease in normal hosts, most of these microorganisms are opportunistics and affect immunocompromised hosts. With the exception of Candida albicans, that is a normal inhabitant of the intestinal tract, most fungi enter the body by inhalation or through skin abrasions. Common fungal diseases includ

Diagnosis and monitoring response to treatment of hepatosplenic candidiasis in patients with acute leukemia using magnetic resonance imaging.

Hepatosplenic candidiasis (HSC) is a morbid complication encountered in immunocompromised individuals, especially, those being treated with intensive chemotherapy protocols for acute leukemia. Immediate recognition of this complication and initiation of appropriate treatment is crucial in order to control the infection, decrease the morbidity and mortality, and avoid delays in treatment of the underlying condition. The definitive diagnosis requires either positive blood cultures for yeasts in the presence of abnormal findings on imaging studies consistent with HSC, or liver biopsy demonstratin

[Plasma (1-->3)-beta-D-glucan determination for screening deep mycosis].

Plasma (1-->3)-beta-D-glucan determination has been developed for the diagnosis of fungemia and deep mycosis to make up for the low yield of blood culture. This method uses Factor G, a coagulation enzyme of the horseshoe crab that is highly sensitive to this glucan. Since (1 -->3)-beta-D-glucan is a characteristic fungal cell-wall constituent which is common to virtually all fungi, the method aims to screen fungal infections as a whole. This is advantageous today when new fungi are appearing one after another as a causative agent of opportunistic deep fungal infections. Plasma (1-->3)-beta-D-g

Suspected Caspofungin-Induced Visual Toxicity Manifesting as Temporary Decreased Vision: A Case Report.

INTRODUCTION: Caspofungin is an echinocandin antifungal agent widely used for the treatment of invasive candidiasis because of its broad antifungal activity. Although ocular adverse events are listed among uncommon reactions in the Summary of Product Characteristics, detailed clinical descriptions of acute visual impairment temporally associated with caspofungin therapy are scarce. CASE PRESENTATION: We report the case of a 21-year-old man who developed candidemia due to Candida guilliermondii following complex spinal surgery. Antifungal therapy with caspofungin was initiated in accordance wi

Machine learning-guided QSAR screening of fluconazole analogs and FDA-approved drugs against Candida albicans, with docking, molecular dynamics, and ADMET analysis.

Invasive candidiasis caused by Candida albicans is a critical-priority fungal disease associated with high mortality, and the increasing resistance to fluconazole underscores the urgent need for new antifungal agents. In this study, a machine learning (ML)-guided quantitative structure-activity relationship (QSAR) workflow was developed to prioritize fluconazole analogs and FDA-approved drugs with predicted antifungal activity against Candida, followed by a structure-based exploration of a cell wall target. Bioactivity (IC50) data for the CHEMBL366 target were curated to 1000 compounds, encode

Resolution of painful refractory soft palate mucoceles following post-surgical onset of satellite lesions: a rare three-year course.

Painful refractory mucoceles erupting along the soft palate are uncommon and management pathways are not well established. In general, oral mucoceles represent common non-neoplastic, non-infectious lesions of the minor/accessory salivary glands and are categorized histopathologically as mucus extravasation phenomena or mucus-retention cysts. We report a rare 3-year treatment-resistant course of painful soft palate mucocele clusters in an adult male causing significant impairment in quality of life, affecting speaking, swallowing, and psychological well-being. The case began as two isolated sof

Harnessing Lactiplantibacillus plantarum as an anti-fungal candidate against drug resistant Candida albicans.

BACKGROUND: Candida albicans is a human opportunistic pathogen responsible for superficial and invasive candidiasis. It is one of the emerging nosocomial pathogens infecting immunocompromised and debilitated individuals, increasing the rate of secondary infections in diseases like HIV. The emerging drug resistance among species is making treatment cumbersome. Also, biofilm-forming ability of C. albicans is one of the primary factors contributing to its virulence as its blocks the penetration of drugs, therefore leading to drug inefficacy. Lactiplantibacillus plantarum, a part of the normal flo

Volatile organic compound profiling by HS-GC-IMS for vaginal infection identification.

BACKGROUND: Bacterial vaginosis (BV) and vulvovaginal candidiasis (VVC) are common vaginal infections; however, the existing techniques for diagnosing these infections are often subjective and inefficient. This study examined the ability of headspace-gas chromatography-ion mobility spectrometry (HS-GC-IMS) to diagnose BV and VVC by analyzing volatile organic compounds (VOCs) in vaginal swabs. METHODS: A total of 616 study participants were recruited, comprising 152 patients with BV, 174 patients with VVC, and 290 healthy controls. Participants were randomly allocated into a training cohort (n

Translation regulation promotes stress adaptation in the human fungal pathogen Candida glabrata.

Invasive candidiasis presents a significant healthcare challenge. The human opportunistic fungal pathogen Candida glabrata, a cause of mucosal and deep-seated infections, resists key antifungal drugs and rapidly proliferates within host macrophages, where it withstands high oxidative stress and amino acid starvation. Unlike C. albicans, C. glabrata lacks true hyphae and relies more on stress adaptation mechanisms than filamentation for virulence. This study explores the molecular mechanisms underlying stress adaptations in C. glabrata that contribute to its pathogenicity. Our findings revealed

An integrated bioinformatics and machine learning-based approach to depict key immunological players associated with candidemia during immunodeficiency.

It is evident that a robust immune system keeps Candida albicans infection in check, but weakened immunity opens the door for shifting from a benign yeast form to an invasive hyphal form which leads to systemic candidiasis with high mortality rate. However, the crucial players contributing to the increased susceptibility of immune-deficient individuals to Candida infection remain obscure. To uncover the molecular differences between these conditions, blood-associated proteins from the NDEx database and differentially expressed genes from GEO datasets of immunocompetent and immune-deficient ind
