OBJECTIVE: This retrospective study aimed to compare the clinical effects of reverse digital artery island flaps and antegrade homodigital neurovascular island flaps in fingertip reconstruction.
PATIENTS AND METHODS: We retrospectively analyzed the data of 30 consecutive patients with fingertip defects who had undergone 2 types of surgery from January 2016 to January 2019. We used reverse digital artery island flaps and antegrade homodigital neurovascular island flaps in 14 and 16 patients, respectively. Flap sensitivity was evaluated using the Semmes-Weinstein monofilament test and static 2-

BACKGROUND: The aim of the study was to evaluate patients with household meat grinder-related hand injuries who presented to our hospital between 2009 and 2020, investigate the causes of these injuries, and discuss prevention methods.
METHODS: Sixty-four patients injured by a meat grinder were retrospectively screened. The patients aged one to 18 years were evaluated in the pediatric group and those over 18 years were evaluated in the adult group. The relationships between age, gender, location of injury (right hand, left hand, wrist, and fingers), and amputation level were analyzed.

In congenital hand differences, microsurgical toe transfer involves the creation of one or several rays in a child that never possessed them before. Second toe transplantation was uniformly used in this series that include 65 consecutive cases. Etiologies were represented by symbrachydactyly, constriction band syndrome or central cleft hand. Second toe transfer was performed with or without the MTP joint and followed by proximal ray resection of the donor toe. In the vast majority of theses cases, transplantation was performed in adactylic or monodactylic hand, the ultimate functional goal was

Distraction-lengthening technique is quite useful in a variety of congenital hand deformities with hypoplastic, or primary normal but secondary shortened (constriction ring syndrome) finger rays. It appears that around the age of two years is the earliest practical time to start distraction; certainly distraction and secondary surgical procedures to improve function should be completed before school entry age whenever possible. Between June 1990 and March 1993, nine distraction lengthening procedures (5 thumbs, 1 index, 3 little fingers) in five patients presenting with congenital hand deformi

Patients with high-level spinal cord injuries (SCIs) that maintain wrist movement but lack the ability to close the hand volitionally may preserve some degree of hand function by using the tenodesis grasp. The tenodesis grasp consists of extending the wrist to passively shorten the finger flexors, resulting in the fingers closing. However, the tenodesis grasp is unavailable to patients who lack wrist function. To address this, we developed a voice-controlled, dual-site, functional electrical stimulation system that evokes the tenodesis grasp by stimulating the wrist extensors and finger flexor

Individuals with neurological disorders, such as stroke or spinal cord injury, often have weakness and/or spasticity in their hand and wrist … which targeted the proximal segment of the radial nerve bundle proximal to the elbow. The wrist and finger joint kinematics were used to classify the different movement patterns through a cluster analysis, and electromyogram signals from the wrist and finger extensors were recorded to investigate the muscle activation patterns. The results showed that the fing …

In forty-four patients with collagen diseases accompanied with Raynaud's phenomenon(systemic lupus erythematosus(SLE)10, progressive systemic sclerosis(PSS)15, primary Sjögren's syndrome(PSJS)18 and mixed connective tissue disease(MCTD)1), we analyzed the thermography of fingers after thermal stimulation with cold water(15 degrees C, 30 seconds). In normal controls, the temperature of the distal points of fingers was higher and recovered more quickly than that of the proximal points(proximal interphalangeal joint; PIP). But in the patients with Raynaud's phenomenon, the temperature of distal p

RATIONALE: Pachydermodactyly is a rare, benign disease that can manifest in healthy adolescent boys as painless, spindle-shaped, soft-tissue swelling of the proximal interphalangeal joints in the hand. It is usually bilateral, with symmetrical joint enlargement. There are relatively few documented cases of pachydermodactyly worldwide, signifying either a low incidence or lack of recognition by physicians; therefore, its diagnosis is challenging.
PATIENT CONCERNS: A 16-year-old boy with a 3-year history of painless unilateral swelling of the proximal interphalangeal joints of his left hand was

INTRODUCTION: Combined injuries of peripheral nerve of upper extremity are usually the result of severe trauma to the extremity, and are often associated with substantial soft tissue, vascular, and bony injuries. The most common form of such combined injury is a low median-ulnar palsy, usually due to laceration of the volar wrist. It is a devastating injury, and with delayed presentation the injury is going to be more difficult to reconstruct.

OBJECTIVE: Erosive hand osteoarthritis (eHOA) is a subtype of hand osteoarthritis (OA) that develops in finger joints with pre-existing OA and is differentiated by clinical characteristics (hand pain/disability, inflammation, and erosions) that suggest inflammatory or metabolic processes.
METHOD: This was a longitudinal nested case-cohort design among Osteoarthritis Initiative participants who had hand radiographs at baseline and 48-months, and biospecimens collected at baseline. We classified incident radiographic eHOA in individuals with ≥1 joint with Kellgren-Lawrence ≥2 and a central eros…

OBJECTIVE: The goal of this study was to determine the sensitivity of the new 2013 classification criteria for systemic sclerosis (SSc; scleroderma) in an independent cohort of SSc subjects and to assess the contribution of individual items of the criteria to the overall sensitivity.
METHODS: SSc subjects from the Canadian Scleroderma Research Group cohort were assessed. Sensitivity was determined in several subgroups of patients. In patients without the criterion of skin thickening proximal to the metacarpophalangeal (MCP) joints, we recalculated sensitivity after removing the individual cri

BACKGROUND: Subungual melanoma is a rare lesion comprising 1% to 3% of all melanoma cases.
METHODS: Records of twenty-two patients with subungual melanoma treated at Roswell Park Cancer Institute during the period September 1971 to September 1989 were reviewed in a retrospective manner. Most common sites of involvement were the great toe on the foot (n = 7), the thumb (n = 4), and the index (n = 3) and ring fingers (n = 3) on the hand. Common signs included pigmentation of the nail bed, nail loss, and nail destruction.
RESULTS: Thickness of the lesion could be determined in 10 patients. All

Postural sway during quiet stance is attenuated by actively maintained contact of the index finger with a stationary surface, even if the level of applied force (<1 N) cannot provide mechanical stabilization. In this situation, changes in force level at the fingertip lead changes in center of foot pressure by approximately 250 ms. These and related findings indicate that stimulation of the fingertip combined with proprioceptive information about the hand and arm can serve as an active sensor of body position relative to the point of contact. A geometric analysis of the relationship between han

A 68-year-old male with type II diabetes mellitus presented with a nodule over the metacarpophalangeal joint of his right second finger after being spurred on the hand by a domestic turkey 2 weeks prior to onset of clinical symptoms. He was diagnosed with cryptococcal tenosynovitis caused by Cryptococcus luteolus identified by DNA sequencing. Complete clinical resolution was achieved with synovectomy and debridement followed by 1 year of fluconazole 800 mg daily.
