Bone repair by autograft is effective in clinical practice. However, serious problems arise when a considerable volume of transplant is needed, as with spinal fusion procedures. The use of bone substitutes combined with osteoinductive agents may contribute to the solution of such problems. In this study, the effectiveness of such a procedure was tested in an experimental model of interbody fusion in rabbits in which the incorporation of a porous hydroxyapatite block (HA) was enhanced by the addition of allogeneic demineralized bone matrix (DBM). The latter was used as a delivery system for the

Different isolation models have been used for qualitative and quantitative studies on the individual roles of periosteum, cortical bone, endosteum and bone marrow in the repair of long bone defects. Corresponding skull defects were studied to evaluate also the dural osteogenetic capacity. A total of 53 operations were performed on the tibia and skull of 33 growing rabbits. The animals were killed 10-12 weeks after surgery. Ordinary rough histological methods were used for studying bone formation, and quantitative estimates were made using serial sections. The cortex, endosteum and bone marrow

LEARNING OBJECTIVES: After reading this article, the participant should be able to: 1. Describe the concept of early protected movement with Kirschner-wired finger fractures to the hand therapist. 2. Choose the most appropriate method of fracture fixation to achieve the goal of a full range of motion. 3. Describe the methods of treatment available for the most common fractures and dislocations of the hand.
BACKGROUND: The main goal of treatment of hand and finger fractures and dislocations is to attain a full range of wrist and nonscissoring finger motion after the treatment is accomplished.

A 7-yr-old girl presented with progressive shortening of the right upper arm and limitation of shoulder motion. Pseudohypoparathyroidism associated with Albright's hereditary osteodystrophy was diagnosed by biochemical, hormonal and radiographic studies. Her condition was complicated by severe humerus varus on the right side. Proximal humeral valgization osteotomy and concomitant humeral lengthening resulted in an improvement of the shoulder joint motion and activity in daily life.

27 patients (28 cases), were treated for perilunate dislocations in 20 and fracture-dislocations in 8. Capitate displacement was dorsal in 27 cases and palmar in 1 case, and in 16, the scaphoid was fractured. There was no substantial delay of treatment. In 8 patients treated with closed reduction, follow-up averaged 6 (1-15) years. 19 patients (20 cases) who underwent early open reduction with K-wire stabilization and ligamentous repair, had an average follow-up of 2 (0.5-7) years. The clinical outcome was evaluated using a scoring system based on pain, occupation, ROM and grip strength, while

In cases of joint fractures as well as in dislocated and open fractures of the hand special standards concerning operation methods have to be set. Treatment of soft tissue damage accompanying the fracture is in no other region of the body so important. Therefore the current efforts for development of an AO-classification for fractures of the hand taking into account the soft tissue damage are of major significance. Special features of miniosteosynthesis compared to conventional osteosynthesis are the subject of this study. The use of the miniplate especially in juxtaarticular fractures of the

Eleven consecutive patients, ten men and one woman (median age, 26 years) presenting with Kienböck's disease, were treated with scaphocapitate arthrodesis. Ten patients had grade III, and one had grade II disease by Lichtman classification. The average follow-up period was 36 months (minimum, 1 year). The lunate was left in situ and scaphocapitate arthrodesis was performed with two lag screws and corticocancellous bone graft. The ulnar variance of the affected wrist was negative (-2.23 mm) and significantly different from the unaffected side. Ten patients had complete pain relief and one had p

OBJECTIVE: To establish bone involvement in reflex sympathetic dystrophy syndrome (RSDS).
METHODS: Bone mineral density (BMD g/cm2) and bone mineral content (BMC g) were measured by dual X-ray absorptiometry. We studied 12 patients with phase I RSDS and 18 controls.
RESULTS: BMD and BMC were significantly lower in the involved side of patients (28.4 and 45.1%, respectively) compared with the contralateral normal limb. Controls only showed a minimal variation of 2.17% in BMD and 4.38% in BMC between right and left measures. The difference between patients and controls was highly significant (

A detailed examination of a stillborn fetus with Apert's syndrome showed several unexpected findings, which prompted a reevaluation of the heretofore generally accepted hypotheses regarding the cause of the dysmorphic craniofacial features in this syndrome. (1) The characteristic clinical features of Apert's syndrome were present at 24 to 26 weeks of gestation, and presumably much earlier. (2) However, there was no evidence, clinical or histological, of premature fusion of any of the cranial sutures or basilar synchondroses in this fetus. From our observations, and from a critical review of th

The hand and wrist are among the most common anatomical areas involved in rheumatic diseases, especially seropositive and seronegative rheumatoid arthritis (RA) and psoriatic arthritis (PsA). The purpose of this study was to identify the most differentiating radiographic characteristics of PsA, seropositive RA, and seronegative RA, particularly in the early stages. A retrospective analysis of radiographic hand findings was performed on 180 seropositive RA patients (29 males, 151 females, mean age at the point of acquisition of the analyzed radiograph of 53.4 y/o, SD 12.6), 154 PsA patients (45

Hand osteomyelitis is a complex condition to diagnose and treat, with an opportunity to improve care through organization of existing evidence. The literature was systematically searched for series of hand osteomyelitis between 1990 and 2022 for evidence regarding diagnosis and treatment, to formulate recommendations. Twenty-one series reported at least 5 cases of hand osteomyelitis in adults, with a total of 666 cases. Surgical debridement is central to treatment and oral antibiotics are sufficient for individuals without diabetes, renal or vascular disease, after debridement and resolution o

The diversity that exists in the types of trauma, in the investigated anatomical structures, in the sites of trauma, in the outcomes of traumatic injuries, and in the general reaction of the body lead to a symptomatological polymorphism of post-traumatic sequelae. Therefore it is impossible to establish a well-defined nosological entity of these sequelae. The damaged tissues react under a very similar scheme, irrespective of the type of tissue, trauma or area, namely, through an inflammatory process, that causes various sequelae depending on certain parameters. Südeck post-traumatic osteodystr

Desbuquois dysplasia is a rare autosomal recessive chondrodysplasia characterised by short stature, joint laxity, facial dysmorphism, a "Swedish key" appearance of the proximal femur, advanced carpal and tarsal bone age, and hand anomalies consisting of phalangeal dislocations and an extra ossification centre distal to the second metacarpal. However, the latter changes are not consistently observed in all Desbuquois patients, defining two distinct groups, based on the presence or absence of hand anomalies. We have performed a genome wide search in four inbred Desbuquois families with typical h

Background: Metacarpal neck fractures account for approximately half of all hand fractures; however, there is no consensus on which surgical approach should be used for these fractures. This study aimed to compare the clinical and radiological outcomes of open reduction internal fixation (ORIF) and closed reduction percutaneous pinning (CRPP) for fifth metacarpal neck fractures. Methods: A total of 71 patients who underwent ORIF or CRPP for fifth metacarpal neck fractures between 2020 and 2024 were retrospectively evaluated. Patients were divided into two groups: ORIF (n = 31) and Kirschner (K

Frequent bone changes in progressive systemic sclerosis are the marginal osteolysis, the arthritic erosions and also the reactions at the numerous entheses of the hand, where the resorptive reactions predominate the osteoproliferative ones. The most frequent resorptions occur at the proximal corners of the tufts. At the margin of the tufts there are also enthesitic reactions, but they do not mark the beginning of an acroosteolysis.
