The role of medial patellofemoral ligament (MPFL) repair versus reconstruction in the treatment of patellar instability continues to undergo debate. In the injured athlete, several criteria must be considered before return to play. There are separate considerations for knee injuries, shoulder injuries, and general upper and lower extremity injuries, as well as concussion, hypertrophic cardiomyopathy, mononucleosis, and spondylolysis. A 28-year-old woman fell and sustained an isolated anterolateral dislocation of the proximal fibula. This injury generally presents with a characteristic history and associated physical examination and radiographic findings, but the subtle nature of the salient findings contributes to a high degree of misdiagnosis. Emergency physicians should be familiar with this injury because early reduction reduces long-term morbidity.

After review of the pertinent literature authors report on a new tendon transfer technique not having been found in the available literature. The method has been considered suitable for the treatment of patellar dislocations due to erroneous stretching direction occurring before the end of growth. The semitendinous muscle was separated and transfosed in the tunnel drilled into the patella. By means of this technique the function of the muscle was used against the lateralization tendency of the patella. The paper presents the follow-up of 9 cases.

The authors describe a new procedure for replacement of both cruciate ligaments. They use a single anterior approach and a unique transplant, consisting of a long band including part of the patellar tendon, part of the patella and part of the quadriceps tendon. A posttraumatic, recurrent luxation of the knee joint is presented. The cause for the first luxation was a severe trauma. As the treatment of the ligament and capsule ruptures of the first luxation was inadequate, the effect was a completely unstable knee joint. By hyperextension trauma or hyperextension following weakness of the muscles of the thigh recurrent luxations of the knee were caused. The importance of a sufficient treatment of ligament and capsule damage following a knee luxation is emphasized.

The classical technic for radiological examination of femoro-patellar joint cannot visualize a lot of real chondromalacia. Their diagnosis, and, therefore, their treatment, usually surgical, rely on doubtful results of clinical examination. Axial projections combined with external rotation of the knee is not perfect, but it is a good improvement. It reveals many femoro-patellar syndroms without any radiological expression until this time.
