Assessment of the efficacy of 3D virtual surgical planning compared with traditional planning in maxillofacial reconstruction for facial traumatic deformity.

BACKGROUND: Maxillofacial reconstruction is a critical surgical procedure aimed at restoring function and aesthetics following trauma. Traditional surgical planning methods have been widely used, but the adoption of 3D virtual surgical planning (VSP) systems has shown potential for improving clinical outcomes. Our study aimed to compare the effectiveness of traditional surgical planning with that of VSP in patients undergoing maxillofacial reconstruction. METHODS: A retrospective cohort study was conducted involving patients treated from January 2020 to December 2023. Patients were divided in

Head Spear Gun Injury: An Atypical Suicide Attempt.

Weapon injuries with spear gun are rare. The aim of this case report is to report the emergency and surgical management when this event occurs. A 35-year-old man attempted suicide with a spear gun. The entry of the shaft was localized through the submental area without an obvious exit point. The projectile passed through the tongue and palatal bone. A tracheotomy was performed. Preoperative cranial computed tomography (CT) showed the harpoon was gone upward through the submental area, the oral cavity, the ethmoid paranasal sinus, the cribriform plate, and the frontal region without vessel dama

Serious facial fractures in New Zealand from 1979 to 1998.

We present data on the incidence, aetiology, age, sex and ethnic distribution of facial fractures in New Zealand for the 20-year period from 1979-1998. Most facial fractures (78.9%) occurred in males with a rate of 65.5/100,000, person-years compared with 21% in females with an incidence of 17/100,000. While the injury rate peaked in males between the ages of 20-24 years (200/100,000), it peaked between 15-19 years (34.7/100,000) in females. The most common causes of facial fracture in both genders were assault (14/100,000) and being unintentionally struck by an object or person (9.5/100,000)

A retrospective analysis of gunshot injuries to the maxillo-facial region.

This study analysed the prevalence, demography, soft- and hard-tissue injury patterns, management and complications of gunshot injuries to the maxillo-facial region in 301 patients treated at Groote Schuur Hospital, Cape Town. The number of patients presenting with maxillo-facial injuries caused by gunshot increased over the 15-year study period. The majority were caused by civilian type low-velocity hand-guns and were purposefully and intentionally inflicted by others. Males in their third decade of life and of low socioeconomic status were most often the victims. The wounding effects of thes

Patterns of Third-Molar-Pericoronitis-Related Pain: A Morphometrical Observational Retrospective Study.

BACKGROUND: Mandibular third molar (M3M) removal and the management of postoperative complications represent a common matter of interest in oral and maxillofacial surgery. Pain represents a great symptom for patients affected by pericoronitis and it is the most common indication for third molar removal. The aim of the present article is to search for patterns of pre-operative pain in patients before undergoing third molar surgery and to test for a relation between some patterns of symptoms, such as pain intensity, site of symptomatic tooth, and referred area of pain. METHODS: This retrospecti

Comparative analysis of five AI platforms for mandibular canal segmentation on CBCT images.

OBJECTIVES: Accurate mandibular canal (MC) identification on cone-beam computed tomography (CBCT) is vital to prevent inferior alveolar nerve injury during oral and maxillofacial procedures. Manual segmentation is time-consuming and operator-dependent, while artificial intelligence (AI) offers automated, reproducible alternatives. This study compared the accuracy of automated MC segmentation across five AI platforms using standardized quantitative and qualitative evaluations. METHODS: A total of 120 anonymized CBCT scans (240 MCs) were analyzed using five fully automated AI-based segmentation

Evaluation of Surgical Approaches to Condylar Fractures: A Systematic Review and Network Meta-Analysis.

BACKGROUND: Condylar fractures are common maxillofacial injuries. Although both open and closed treatments are used, the optimal strategy remains debated due to variable functional outcomes and complication rates. PURPOSE: This network meta-analysis compared the functional outcomes and complications of closed treatment versus various open approaches for mandibular condyle fractures. We evaluated and ranked 6 distinct open approaches: mandibular angle approach (MAA), retromandibular anteroparotid approach (RMAA), endoscopic transoral approach (ETOA), retromandibular through parotid approach (R…

Neuropsychologic outcomes in patients treated for complex maxillofacial trauma.

Complex fractures of the craniofacial skeleton are caused most commonly, in Australia, by motor vehicle accidents, falls, and interpersonal violence. Significant force is required to fracture the facial skeleton, and the long-term effect these forces have on higher brain function is unclear. The study aim was to assess long-term neuropsychologic changes associated with complex fractures of the facial skeleton. Patients managed for complex fractures of the facial skeleton by the Australian Craniofacial Unit, South Australia, between 2002 and 2011, with at least 1-year follow-up, were assessed u

Usefulness of endoscopy in craniofacial trauma.

To determine the extent of endoscopy utilized in craniofacial trauma, a questionnaire was mailed to 400 surgeons in the United States. A 40% response was obtained; 21.3% of the surgeons stated that they had used endoscopy to treat facial fractures; 33% of them had been in practice for 6 to 10 years, and 47% of those who had used endoscopy in craniofacial trauma were in full-time private practice. In facial fractures, endoscopes were used primarily to treat Le Fort fractures (55.9%), followed by zygomatic arch fractures (52.9%). Lack of accompanying instrumentation, such as appropriate drills,

Priorities in the management of penetrating maxillofacial trauma in the pediatric patient.

Penetrating facial trauma is uncommon in children; a large series published by Cooper et al revealed that only 1% to 2% of the total population of infants and children admitted for trauma during their study period had a diagnosis of penetrating trauma to the head or neck. Little has been published specifically addressing these injuries in the pediatric population. The records of 20 patients treated for penetrating facial injuries at Kosair-Children's Hospital in Louisville, Kentucky from January 1991 through December 1994 were reviewed. The location, mechanism and extent of injury, as well as

Microvascular approach to function and appearance of large orbital maxillary defects.

Forty-three patients with major three-dimensional orbitomaxillary defects underwent 48 free-tissue transfers for defects arising from resection of a neoplasm or trauma. Defects were complex, involving the malar skin, mandible, lateral nasal wall, orbit, palate, and brain. A three-dimensional approach attempting to recreate the midface by folding the flaps was the usual method, with subsequent revisions if necessary. Ten patients had simultaneous free bone grafts and 6 had vascularized bone grafts. There were three flap failures and four perioperative deaths. Excluding the 4 deaths, the results

Mapping the Maxillary Artery and Lateral Pterygoid Muscle Relationship: Insights from Radiological and Meta-Analytic Evidence.

Background/Objectives: Variations in the course of the maxillary artery (MA) relative to the lateral pterygoid muscle (LPM) pose critical challenges in surgical, anesthetic, and interventional procedures involving the infratemporal fossa (ITF). These variations can increase the risk of hemorrhage, nerve injury, or incomplete anesthesia. The present study aimed to elucidate the topographic relationship between the MA and LPM by combining high-resolution radiological imaging with a comprehensive analysis of anatomical literature. Materials and Methods: A retrospective review of 250 brain compute
