Larger-Volume Silicone Breast Implants Are Safe in Breast Reconstruction: The Athena Multicenter Prospective Study of 400 Patients.

BACKGROUND: For women in the United States desiring implant-based breast reconstruction, the maximum volume of commercially available silicone breast implants has, to date, been 800 cc. The authors evaluated the safety of Mentor larger-size MemoryGel Ultra-High Profile breast implants (≥930 cc) in women undergoing postmastectomy breast reconstruction. METHODS: This 10-year, multicenter, open-label, prospective, investigational study assessed the safety and effectiveness of larger-volume implants in women undergoing postmastectomy 2-stage primary or revision reconstruction. Three-year Kaplan-M

Scripted success, unscripted consequences: How assessments in undergraduate medical education shape physician identity formation.

INTRODUCTION: Professional identity formation (PIF) is increasingly understood not only as becoming a physician but as a matter of being and belonging within the profession. The development of a holistic professional identity can enhance a physician's confidence, wellbeing and performance. Assessment is a central site of the socialisation through which PIF occurs, yet how learners experience assessment when it is felt to be biassed or exclusionary, and what this means for their identity formation, is not well understood. We aimed to explore if and how medical students experience identity-relat

Physician-Led Teams: A Model Curriculum for Emergency Medicine Residency.

BACKGROUND: Physician-led teams in emergency medicine (EM) are associated with higher quality and value than independent non-physician practice, yet most residents receive little formal preparation in leading inter-professional teams that include nurse practitioners (NPs) and physician assistants (PAs). Clear, scalable training guidance for residency programs is lacking. METHODS: We conducted a narrative synthesis of national policy positions, scope-of-practice trends, and published literature on team leadership training in EM and related disciplines, including other specialties, nursing, and

Drivers of bat researchers' intent to adopt field hygiene practices.

Infectious disease is a growing threat to wildlife, with zoonotic transmission most likely at the human-wildlife interface. One underappreciated activity at this interface is fieldwork with wild animals, but associated risks can be mitigated through field hygiene (FH) practices, such as using personal protective equipment and other appropriate behaviors. Following the dissemination of International Union for Conservation of Nature FH guidelines for bat researchers, we investigated factors that affect bat researchers' intent to use FH practices under a theory of planned behavior (TPB) framework

Contextualizing Racial and Ethnic Inequities in COVID-19 Vaccination Uptake During Pregnancy Through a Community-Led Evaluation.

BACKGROUND: Massachusetts Department of Public Health (MDPH) disaggregated data on vaccine coverage for pregnant people by their specific racial and ethnic identities, finding lower coverage among several subpopulations, including Puerto Rican and Haitian pregnant people. Quantitative data could not explain contexts shaping vaccine decisions; thus, MDPH partnered with Tufts Interdisciplinary Evaluation Research (TIER) to conduct a community-led evaluation to understand COVID-19 vaccine perceptions and decision-making among Hispanic/Latina and Haitian residents pregnant when COVID-19 vaccine ac

Impact of a simulation-based training program on the acquisition of ultrasound window competencies for the initial assessment of polytrauma patients in advanced prehospital nursing care.

INTRODUCTION: Point-of-care ultrasound is essential in the initial assessment of polytrauma patients. The E-FAST protocol enables rapid detection of intra-abdominal free fluid, pericardial effusion, and pneumothorax, with particular usefulness in prehospital settings. However, ultrasound training among emergency nursing staff remains limited, especially within Advanced Life Support (ALS) mobile units. OBJECTIVE: To evaluate the effect of a brief, structured training program on the acquisition of competencies for obtaining E-FAST windows in ALS mobile units nurses, assessing knowledge, technic

Patients refusing a permanent colostomy for rectal cancer: clinical outcome and psychological aspects.

PURPOSE: For locally advanced rectal tumors affecting the lower rectum, anal sphincter, or levator muscles, abdominoperineal resection (APR) is one of the surgical options. This involves extensive tissue removal and the creation of a permanent colostomy. However, colostomy is perceived by certain patients as a serious challenge, affecting multiple aspects of their lives. Thus, some refuse surgery and accept the risks of leaving the tumor untreated. Our study aims to explore refusal rates and reasons for declining surgery. METHODS: Patients who were recommended to undergo APR surgery as part o

The Role of Perceived Social Support in the Association Between Stress and Creativity Self-Efficacy Among Adolescents With Attention Deficit Hyperactivity Disorder.

INTRODUCTION: Adolescents with attention deficit hyperactivity disorder (ADHD) face significant challenges related to stress, inattention and impulsivity, which can impair their creativity self-efficacy. Social support is a critical protective factor that mitigates stress and enhances resilience, yet its role in fostering creativity self-efficacy among adolescents with ADHD remains underexplored. AIM: To examine the levels of stress, perceived social support and creativity self-efficacy among adolescents with ADHD and to explore the mediating role of social support in the relationship between

Gender equity and career progression among Croatian oncology professionals compared with the ESMO global survey.

BACKGROUND: Based on the 2016 and 2021 ESMO Women for Oncology (W4O) surveys, which revealed persistent gender disparities in oncology, we conducted a national survey to examine career-related challenges among oncology professionals in Croatia and compare them with international trends. METHODS: We administered an anonymous online survey adapted from the 2021 ESMO W4O questionnaire, contextualized for the Croatian setting. The survey explored the perceived impact of gender, political affiliation, religion, and sexual orientation on career development. Responses were analyzed by gender and age.

The role of social networks in supporting self-management in CKD: a narrative synthesis.

BACKGROUND: Chronic kidney disease (CKD) is a growing global health concern requiring effective self-management to mitigate progression and improve quality of life. While self-management is increasingly recognised as a socially embedded practice, the specific contributions of social network members to this process in CKD are not well understood. OBJECTIVE: To synthesise and interpret existing literature on how social networks support individuals living with early-stage CKD and identify gaps in understanding. METHODS: We conducted a narrative review. Fourteen studies involving 560 participant

What drives adolescent girls and young women's decisions to persist on PrEP? Results of a comparative qualitative study from a DREAMS program in western Kenya.

Oral pre-exposure prophylaxis (PrEP) is a valuable tool to help end HIV, but persistent PrEP use is low amongst adolescent girls and young women (AGYW) in sub-Saharan Africa (SSA), who are at high risk for HIV. To improve persistent PrEP use in AGYW, more research is needed to understand what drives their decisions to continue with PrEP. We conducted a thematic, comparative qualitative analysis of in-depth interviews with 32 AGYW who participated in PEPFAR's DREAMS program in western Kenya: 16 were randomly sampled among those who persisted with PrEP and 16 among those who discontinued. We com

The identification of requirements for competency development during work-integrated learning in healthcare education.

BACKGROUND: Work-integrated learning (WIL) is widely accepted and necessary to attain the essential competencies healthcare students need at their future workplaces. Yet, competency-based education (CBE) remains complex. There often is a focus on daily practice during WIL. Hereby, continuous competency development is at stake. Moreover, the fact that competencies need to continuously develop is often neglected. OBJECTIVES: To ultimately contribute to the optimization of CBE in healthcare education, this study aimed at examining how competency development during WIL in healthcare education cou

Occupational Radiation Exposure During Pregnancy: A Survey of Urologists on Perception, Experience, and Practice Patterns.

Introduction: The field of urology is predominantly male; however, there has been an increasing number of women in the workforce. Peak reproductive years frequently overlap with residency training and early attending career timelines. Exposure to ionizing radiation is a common occupational hazard in many procedural specialties. The use of radiation, for example, in interventional cardiology and interventional radiology, has shown little adjustments in practice patterns, with no adverse outcomes reported among pregnant physicians in their fields in the setting of appropriate radiation safety me

Experiences of alcohol use during pregnancy: A qualitative study of pregnant women at risk of acquiring HIV in Cape Town, South Africa.

In South Africa, alcohol use during pregnancy is prevalent and associated with increased HIV risk. Developing locally sensitive and contextually appropriate evidence-based interventions to address alcohol use among pregnant and breastfeeding women in South Africa requires comprehensive understanding of the context of perinatal alcohol use and how relationships and lived environments may serve as barriers or supports for alcohol reduction. We conducted twenty in-depth qualitative interviews with isiXhosa speaking women who reported alcohol use during their recent pregnancy and/or recent intimat

Scale-Up of a Newborn Resuscitation Capacity-Building and Skill Retention Program Associated With Improved Neonatal Outcomes in Gandaki Province, Nepal.

BACKGROUND: Intrapartum events leading to asphyxia at birth are a leading cause of neonatal morbidity and mortality in Nepal. In response, the Nepal Ministry of Health and Population adopted the Helping Babies Breathe (HBB) training curriculum in 2015 as a tool to improve neonatal resuscitation and outcomes. Although the effectiveness of HBB training has been well documented, challenges remain in maintaining skills over time. Safa Sunaulo Nepal (SSN) designed an evidence-based intervention for scaling up newborn resuscitation training and skill retention. We report on its implementation and th

Digital delivery of behavioural activation therapy to overcome depression and facilitate social and economic transitions of adolescents in South Africa (the DoBAt study): protocol for a pilot randomised controlled trial.

INTRODUCTION: Scalable psychological treatments to address depression among adolescents are urgently needed. This is particularly relevant to low-income and middle-income countries where 90% of the world's adolescents live. While digital delivery of behavioural activation (BA) presents a promising solution, its feasibility, acceptability and effectiveness among adolescents in an African context remain to be shown. METHODS AND ANALYSIS: This study is a two-arm single-blind individual-level randomised controlled pilot trial to assess the feasibility, acceptability and initial efficacy of digita

The Impact of a Structured Virtual Reality Simulation Training Curriculum for Novice Endoscopists.

BACKGROUND: Current evidence supports the use of virtual reality (VR) simulation-based training for novice endoscopists. However, there is still a need for a standardized induction programme which ensures sufficient preparation, with knowledge and basic skills, before their approach to patient-based training. We designed a structured progressive programme in upper endoscopy and colonoscopy and aimed to determine its impact on cognitive and technical performance. METHODS: Prospective, multicentre study, focused on "Endoscopy I, 2018," a course with a theoretical and a hands-on module (20 h) in
