Feasibility of artificial intelligence assisted quantitative muscle ultrasound in carpal tunnel syndrome.

BACKGROUND: In case of focal neuropathy, the muscle fibers innervated by the corresponding nerves are replaced with fat or fibrous tissue due to denervation, which results in increased echo intensity (EI) on ultrasonography. EI analysis can be conducted quantitatively using gray scale analysis. Mean value of pixel brightness of muscle image defined as EI. However, the accuracy achieved by using this parameter alone to differentiate between normal and abnormal muscles is limited. Recently, attempts have been made to increase the accuracy using artificial intelligence (AI) in the analysis of mus

Agrin mutations lead to a congenital myasthenic syndrome with distal muscle weakness and atrophy.

Congenital myasthenic syndromes are a clinically and genetically heterogeneous group of rare diseases resulting from impaired neuromuscular transmission. Their clinical hallmark is fatigable muscle weakness associated with a decremental muscle response to repetitive nerve stimulation and frequently related to postsynaptic defects. Distal myopathies form another clinically and genetically heterogeneous group of primary muscle disorders where weakness and atrophy are restricted to distal muscles, at least initially. In both congenital myasthenic syndromes and distal myopathies, a significant num

Effects of renal denervation on renal pelvic contractions and connexin expression in rats.

AIMS: The renal pelvis shows spontaneous rhythmic contractile activity. We assessed to what extent this activity depends on renal innervation and studied the role of connexins in pelvic contractions. METHODS: Rats underwent unilateral renal denervation or renal transplantation. Renal pelvic pressure and diuresis were measured in vivo. Spontaneous and agonist-induced contractions of isolated renal pelves were investigated by wire myography. Rat and human renal pelvic connexin mRNA abundances and connexin localization were studied by real-time PCR and immunofluorescence respectively. RESULTS:

[Overactive bladder: prospects and limitations of botulinum toxin].

Botulinum toxin is the most potent poison known to man. It is produced by Clostridium botulinum and consists of a heavy chain which is responsible for the internalization of the toxin into the cytosol and a light chain that has the ability to cleave proteins within the nerve terminal. As those proteins are essential for normal vesicular transport and fusion of acetylcholine, botulinum toxins are able to prevent its release at the presynaptic membrane, resulting in a chemodenervation of the detrusor muscle after intravesical injection of the toxin and an impressive reduction of symptoms of over

Trigeminal nerve activity and buccal pressure as an index of total inspiratory activity in the bullfrog.

In bullfrogs with glottis occluded and lungs artificially ventilated, the buccal cavity contractors (BCconts) were found to contract isometrically, meaning that peak buccal pressure (OBP) well represents the total inspiratory nerve activities (TIAs) of BCconts. Unilateral sectioning of the trigeminal (V), hypoglossal, facial, or vagal nerve reduced the magnitude of OBP by 43.3, 28.8, 10.9, or 4.3%, respectively. The integrated peak V activity (I-ENGv) showed a linear relationship to OBP (r = 0.944) in the glottis occluded frog, but a curvilinear relationship to peak buccal pressure (peak BP) i

Quantitative STIR of muscle for monitoring nerve regeneration.

PURPOSE: To assess whether short tau inversion recovery (STIR) MRI sequences can provide a tool for monitoring peripheral nerve regeneration, by comparing signal intensity changes in reinnervated muscle over time, and to determine potential clinical time points for monitoring. MATERIALS AND METHODS: For this prospective study, 29 patients with complete traumatic transection of the ulnar or median nerves in the forearm were followed up to 45 months postsurgery. Standardized 1.5 Tesla STIR-MRI scans of hand muscles were obtained at fixed time intervals. Muscle signal intensities were measured s

The evolution of an experimental distal motor axonopathy. Physiological studies of changes in neuromuscular transmission caused by cycloleucine, an inhibitor of methionine adenosyltransferase.

Cycloleucine (CL), a synthetic amino acid is known to cause degeneration of motor nerve terminals. This paper describes the changes in neuromuscular transmission, the morphology of motor end-plates and the responses of muscle spindles after a single dose of CL was administered to weanling and adult mice. Animals were allowed to survive for between 12 h and 7 days. Twitch and tetanic responses of muscles stimulated through their nerves fell dramatically within 24 h in both young and adult mice and intracellular recordings revealed that a large proportion of end-plates in calf muscles became den

Neuropathological studies on the toxic syndrome related to adulterated rapeseed oil in Spain.

Biopsies of muscle and sural nerves, and autopsies of patients affected by the toxic and neuromuscular syndrome produced by ingestion of adulterated rapeseed oil were studied using morphological, histochemical and ultrastructural methods. In muscle, two pathological pictures were distinguished according to their temporal sequence. In the early phase, the neuromuscular syndrome was characterized by myalgia and an inflammatory infiltration of the perimysium, the capsules of muscle spindles and intramuscular nerves. The muscle fibres exhibited small subsarcolemmal zones of fibril disintegration a

Electrodiagnostics: MUNE and MUNIX as methods of estimating the number of motor units - biomarkers in lower motor neurone disease.

Routine quantitative electromyography is used for the assessment of the presence of lower motor neurone involvement and its consequences, including primary denervation and compensatory reinnervation of muscle fibres. However, it is not useful for the assessment of the motor unit number reserve. The need for a valid biomarker to evaluate lower motor neurone disease progression in such diseases as amyotrophic lateral sclerosis, and for use in clinical trials, has led to a number of studies of the methods that allow assessment of the number of motor units. In this review, motor unit number estima

Clinically definite ALS presenting weeks after mild electric injury: causality or coincidence?

Motor neuron syndromes including typical ALS develop very rarely after electrotrauma, with possible causality discussed but not confirmed. We report on a 44-year-old male who developed clinically definite ALS by the revised El Escorial criteria with onset weeks after mild electric injury. He presented with asymmetric upper limb amyotrophy and weakness beginning around the entry point of the current. Over 1 year he developed generalized wasting, weakness and fasciculations, including the bulbar and thoracic muscles, with prominent spasticity and pyramidal tract signs. Electrodiagnostic studies

[Treatment of anal fissures with botulinum toxin].

Chronic anal fissure is a common proctological disease. Botulinum toxin (BTX) can be used for temporary chemical denervation. The administration is by intramuscular injections into either the external or the internal anal sphincter muscles. The mode of action, administration techniques and possible complications or adverse effects of BTX therapy are discussed. The healing rate is dependent on the BTX dosage. The short-term healing rate (< or = 6 months) is between 60 and 90 %. In long-term follow-up studies (> 1 year), about 50 % of patients show a complete response. Adverse effects are genera

Matrix metalloproteinase-9 is elevated in serum of patients with amyotrophic lateral sclerosis.

Matrix metalloproteinase-9 (MMP-9) and its specific inhibitor, tissue inhibitor of matrix metalloproteinase-1 (TIMP-1), were analysed by enzyme-linked immunosorbent assay (ELISA) and by zymography in serum and cerebrospinal fluid (CSF) of patients with amyotrophic lateral sclerosis (ALS). In contrast to patients with inflammatory diseases, MMP-9 levels were not elevated in CSF of ALS patients. In serum, however, compared to healthy donors, MMP-9 was significantly (p = 0.0003) increased up to levels as high as those of viral meningoencephalitis (VM) or bacterial meningitis (BM) patients. MMP-9

Indirect effect of sciatic nerve injury on the epidermal thickness of plantar glabrous skin in rats.

In patients with nerve injuries, the skin over the denervated part tends to atrophy. Few experimental studies have dealt with this issue. In this paper we examine the effects of nerve injury on the epidermis in plantar glabrous skin in rats. Analysis of paraffin sections showed that the epidermis becomes abnormally thin after sciatic neurotomy and suture or neurectomy, but not after nerve crush. Autoradiographic analysis showed a subnormal number of thymidine-labelled epidermal cells in plantar skin of rats subjected to sciatic neurectomy. However, after selective division of the sciatic foot

The enigma of autonomic failure in diabetes.

The extent of autonomic failure in diabetes and the damage that results both in terms of structure and function is seldom realised. This article focuses on the consequences of autonomic denervation of several different forms of smooth muscle, notably those of arterial wall, vas deferens, stomach, iris and bronchial wall. The clinical implications of these changes are described, in particular the problems arising from vascular (and vas deferens) calcification and abnormalities of vascular reactivity; insulin-induced postural hypotension and the splanchnic circulation; the anaemia of autonomic n
