In 44 men with a history of repeated occupational lead poisoning multidisciplinary clinical and laboratory investigations were carried out. The number of lead poisoning episodes was from 2 to 7. Apart from peripheral hypalgesia on the extremities (in 6 cases) no other neurological abnormalities were found. The motor nerve conduction velocity in the median, ulnar, radial, peroneal and tibial nerves was slightly slower in 27 cases and considerably slower in one case. Slowed down conduction velocity was most often observed in 2 nerves (12 cases) and in one nerve (9 cases), less frequently in thre

Inclusion body myositis (IBM) has been viewed as a distinct and rare form of inflammatory myopathy. Previously reported findings from series of IBM patients have suggested that clinical and pathologic features are present which readily distinguish it from idiopathic polymyositis. We report 4 cases of IBM presenting clinically and pathologically as polymyositis, each of which was refractory to therapy. Our data suggest that IBM may be a more common and heterogeneous form of inflammatory myopathy than has been previously suggested. Furthermore, IBM may be clinically and electrophysiologically in

Neurologic signs dominate the manifestations of the neonatal abstinence syndrome (NAS). To help delineate this dysfunction, peripheral nerve conduction studies (NCS) were made in 25 neonates born to methadone-maintained mothers; 12 of the mothers abused other controlled substances concomitantly. Median and common peroneal motor nerve conduction velocities (NCV) in these infants were normal, both at three to seven days and three to four weeks of age, and were unaffected by maternal drug intake pattern, severity of neonatal abstinence symptoms, treatment with either camphorated tincture of opium

A noninvasive technique has been developed for recording from the the facial nerve within the fallopian canal. Following transcutaneous stimulation of the nerve on the face, an antidromic nerve potential can be detected with electrodes placed on the eardrum and enhanced by the technique of averaging. In studies conducted on cats and normal human subjects it has been determined that the primary recording site of the nerve potential is along the tympanic segment of the nerve just distal to the geniculate ganglion. Disturbances in nerve conduction caused by an experimental lesion produced changes

Nerve injuries associated with inversion sprains of the ankle have been mentioned in the literature on a case presentation basis only. Sixty-six consecutive patients with Grade II (30) and Grade III (36) ankle sprains were examined by electromyography 2 weeks after injury to determine the presence and distribution of nerve injuries. Ankle active range of motion (AROM) and the number of weeks postinjury when the patient could heel/toe walk and return to full activity were also noted. Five patients (17%) with Grade II sprains had mild peroneal nerve injury and three (10%) injured the tibial nerv

The effects of different arm positions during open-heart surgery on the incidence of ulnar nerve injury were studied prospectively in 35 patients. It was concluded that ulnar nerve damage was common and was due to nerve compression at the elbow. A significant reduction in the incidence of nerve injury can be effected by placing the forearms above the head rather than at the patient's side.

The function of sensory and motor neurons below the level of a spinal cord injury (SCI) was assessed in 23 patients by recording the nerve conduction velocities and the amplitudes of evoked nerve and muscle compound action potentials for sural and posterior tibial nerves. Sensory and motor conduction velocities were generally normal when obtainable. Two patients had a unilateral reduction in the sural nerve and abductor hallucis muscle compound action potentials which may have been due to peripheral nerve compression. In 11 patients the abductor hallucis muscle compound action potential was be

The authors report two cases of Besnier -Boeck- Schaumann sarcoidosis with a neurological presentation. The first case was a 25 year old woman with a 5 year history of polyneuritis affecting the cranial and spinal nerves with very low nerve conduction times and without any detectable visceral disease. The diagnosis was made by distal bronchial biopsy. The second case was of an elderly man with tremor, difficulty in keeping his balance and deafness with chronic meningitis. The diagnosis was made by proximal bronchial biopsy, chest X-Ray showing a benign hilar lymphoma. In the first case, therap

The purpose of this study was to determine the effect of wrist position on distal sensory and motor latencies of the median nerve. An orthosis was devised to maintain the wrist either in extension or in flexion while measuring these latencies. Fourteen patients with presumptive carpal tunnel syndrome (CTS) and 12 control subjects were evaluated following extension or flexion of the wrist for 5 and 10 minutes. Results showed an increase in distal latencies for both groups; the highest increment was found among CTS patients following 5 minutes of wrist flexion. Since it is not uncommon for patie

Three children, from different kinships, with generalized insensitivity to pain, showed unusual manifestations of congenital, presumably inherited, sensory and autonomic neuropathy. The first child appeared to have a syndrome resembling those previously described as congenital indifference to pain, congenital universal loss of pain sensation from infancy without other apparent neurological deficit. Unlike most types of hereditary sensory and autonomic neuropathies (types I, II, III), but like type IV, she had normal sensory nerve action potentials. Abnormalities of sudomotor function and of so

The tarsal tunnel syndrome is not as commonly diagnosed as is its counterpart in the hand, the carpal tunnel syndrome. Electrodiagnostic evaluation has shown that reduced amplitude and increased duration of motor evoked potentials are more sensitive indicators of the presence of tarsal tunnel syndrome than is the distal motor latency. The lateral plantar branch of the posterior tibial nerve is probably affected earlier than is the nerve's medial plantar branch. Surgical release usually results in complete relief of the compression neuropathy. Electrodiagnostic evaluation also may help to separ

Isometric contraction of the adductor pollicis by stimulating the ulnar nerve was determined in 20 normal adult subjects. We measured mechanical latency, electro-mechanical latency, contraction time, time to half relaxation and isometric twitch force. Besides that we registered the compound muscle action potential and the nerve conduction velocity. The median nerve was used to elicit the nerve action potential and to determine conduction velocity and relative refractory period. In 6 pat. with hypothyroidism and in 7 pat. with hyperthyroidism the above mentioned parameters were measured. There

We used the F-wave conduction velocities to estimate the motor nerve conduction along the proximal segment (spinal cord to knee) of the axon of tibial and common peroneal nerves in ten patients with alcoholic polyneuropathy and compared these to the motor nerve conduction velocities in ten normal subjects. All the studies so obtained showed a normal conduction in the proximal segments even though in three patients the distal nerve conduction values (knee to ankle) revealed moderate to severe neuropathy. These data support the hypothesis that in alcoholic polyneuropathy, motor conduction in pro

A 47-year-old woman experienced loss of strength, sensation, and sweating distal to a pneumatic tourniquet applied to her upper arm during operation for a Dupuytren contracture. Electrophysiologic tests showed a severe conduction block of sensory and motor fibers, well localized to the presumed lower margin of the tourniquet, with little evidence of axonal degeneration. Reversal of block began at 5 weeks and was complete in 6 months, coincident with almost full clinical recovery. Sweating was absent before 5 weeks and then returned, accompanied by a causalgic syndrome that did not disappear un
