Comparation of the effectiveness of conventional needle irrigation and photon-induced photoacoustic streaming with sodium hypochorite in the treatment of teeth with apical periodontitis: a randomized clinical trial.

Photon-initiated photoacoustic streaming (PIPS) with an Er: YAG laser has been introduced in root canal treatment to improve irrigation and facilitate the removal of bacteria in the root canal system. This study aimed to compare the antibacterial effectiveness of two different root canal irrigation techniques, conventional needle irrigation (CNI) and PIPS, using 1% sodium hypochlorite (NaOCl), in the treatment of teeth with apical periodontitis. Sixty patients with a total of sixty teeth affected by apical periodontitis were included in this study. The teeth underwent root canal therapy, and a

MicroRNAs Markedly Expressed in Apical Periodontitis Cooperatively Regulate Cytokines and Growth Factors Promoting an Anti-inflammatory Response.

INTRODUCTION: MicroRNAs have been shown to play a role in the pathogenesis of apical periodontitis. Upregulation of miR-10a-5p and downregulation of miR-891a-5p were previously reported in apical periodontitis samples. This study aims to perform a functional characterization of miR-10a-5p, investigating its capacity to regulate the expression of inflammatory cytokines and growth factors, as well as a possible co-regulation mechanism with miR-891a-5p in the development of apical periodontitis. METHODS: miR-10a-5p mimics/controls and miR-891a-5p inhibitors/controls were introduced to human K-56

Histologic examination of teeth with necrotic pulps and periapical lesions treated with 2 scaffolds: an animal investigation.

INTRODUCTION: Traditional pulp regeneration procedures that use a blood clot as a scaffold have produced histologic evidence of bone, cementum, and connective tissue growth within the root. Platelet-rich plasma (PRP) is a bioactive scaffold containing growth factors that enhance wound healing. AIM: The aim of this study was to histologically compare the tissues generated when PRP or a blood clot is placed into teeth with preexisting necrotic pulps and periapical lesions. METHODS: Twenty-four canine teeth from 6 immature ferrets were used. Two ferrets served as positive controls. Sixteen expe

Frequency of localization of the painful tooth by patients presenting for an endodontic emergency.

INTRODUCTION: Endodontic pain patients sometimes have difficulty in accurately identifying the painful tooth. We determined the frequency in which patients presenting with endodontic pain can correctly localize the painful tooth and the effect of periradicular symptoms. METHODS: The frequency of localization of the painful tooth in 79 endodontic emergency patients was assessed by using patient and dentist assessment of presenting pain along with the verbal numeric rating scale (VNRS). RESULTS: The results show that patients presenting with odontogenic pain can localize the painful tooth 73.3

Intentional replantation of a mandibular molar: case report and 14-year follow-up.

This case report describes the nonsurgical endodontic treatment and intentional replantation of a mandibular molar and its nearly 14-year follow-up. A 56-year-old woman sought treatment with complaint of discomfort and sensitivity in the mandibular left area. The radiographic examination showed that the mandibular left first molar was endodontically treated, with over-instrumentation of the distal root and unsatisfactory obturation on the mesial root. A large periapical lesion was observed surrounding the roots. In view of the patient's limited interocclusal space and objection to undergo an a

Post-operative symptoms and healing after endodontic treatment of infected teeth using pulsed Nd:YAG laser.

Forty-four teeth in 38 patients, diagnosed with chronic apical periodontitis, were endodontically treated. Root canals were shaped using a step-back technique with 5% NaOCl and 3% H2O2 as irrigants. In half of the teeth the canal terminus was irradiated with pulsed Nd:YAG laser (1 W, 15 pps, 1 s). All canals were then obturated with laterally condensed gutta-percha points and sealer, and final radiographs were obtained. Occurrence of spontaneous pain was recorded 1 day after treatment. Percussion pain was recorded after 1 week, and then at 3 and 6 months after treatment. Radiographic follow-up

Distribution of Ia antigen-expressing nonlymphoid cells in various stages of induced periapical lesions in rat molars.

Periapical lesions were experimentally produced in rat lower first molars by exposing the pulp to the oral environment for 1 to 56 days. Temporal changes in the number and distribution of la antigen-expressing nonlymphoid cells in the periapical tissue were examined immunohistochemically on decalcified cryostat sections using OX6, a monoclonal antibody against rat la antigen. Influx of la-positive exudative cells into the periapical tissue was observed from 1 day postoperatively. Between 14 and 28 days when expansion of the periapical lesion was most evident, numerous la-positive macrophage-li

Direct detection of Prevotella intermedia and P. nigrescens in suppurative oral infection by amplification of 16S rRNA gene.

A specific 16S rDNA PCR and subsequent hybridisation reaction was designed to discriminate between strains of Prevotella intermedia (n = 15) and P. nigrescens (n = 15). This technique was then used to detect the presence of these two bacterial species in acute suppurative oral infection. A total of 36 pus samples aspirated from 26 peri-apical abscesses, three root canals, three periodontal abscesses, two cases of refractory periodontitis, one cyst and one haematoma was examined. A portion of the pus sample was processed by PCR and the remainder of the specimen was subjected to routine culture.

Interim endodontic therapy for alveolar socket bone regeneration of infected hopeless teeth prior to implant therapy.

The immediate placement of implants in the fresh extraction sockets of infected teeth with periradicular and periapical lesions is contraindicated because of both the infection and the loss of architecture required for proper implant placement. There are 4 approaches for implant replacement of a hopeless tooth with lesions: (1) extraction and delayed implant placement; (2) extraction, debridement, guided bone regeneration (GBR), guided tissue regeneration (GTR), and delayed implant placement; (3) extraction, intrasocket debridement, and immediate implant placement; or (4) extraction, debrideme

Subgingival Microbial Signatures Associated With Apical Periodontitis Identified by Next Generation Sequencing and Predictive Modelling.

AIMS: To assess the relationship between endodontic and subgingival bacterial communities in individuals with apical periodontitis (AP), and to identify disease-associated subgingival microbial signatures. We propose that subgingival microbial communities exhibit a dysbiotic profile, defined by distinct bacterial signatures, which may provide complementary biological insights into AP. METHODS: In this cross-sectional study, DNA was extracted from paired endodontic and subgingival samples from mesiobuccal sites of first molars in patients with AP (n = 25 sample pairs), and from subgingival sam

Robot-Assisted Endodontic Retreatment: A Case Report with Clinical Considerations.

Fiber posts present significant challenges for nonsurgical endodontic retreatment, as improper removal may result in iatrogenic root perforation or even root fracture. Recently, robotic technology has attracted considerable attention in dentistry and active dental robotic (ADR) systems can perform procedures based on preset instructions, minimizing reliance on the dentist's experience. This case report describes the application of an ADR system for fiber post removal through an existing zirconia crown. A 26-year-old female was diagnosed with previously treated, symptomatic apical periodontitis
