Acute inflammatory neuropathy with monoclonal anti-GM2 IgM antibodies, IgM-κ paraprotein and additional autoimmune processes in association with a diffuse large B-cell non-Hodgkin's lymphoma.

Lymphoproliferative disorders are often associated with autoimmune processes preceding or following the occurrence of a lymphoma. Here, we describe a patient with a history of recurrent diffuse large B-cell non-Hodgkin's lymphoma who suffered from an acute inflammatory neuropathy with specific monoclonal anti-GM2 IgM antibodies and associated IgM-κ paraprotein. It was possible in this case to prove that both, anti-GM2 IgM antibodies and IgM-κ paraprotein, share the same binding characteristic. In addition, the patient possibly suffered from an immune thrombocytopenia and an early-stage bullous

Symptoms of activity-induced weakness in peripheral nervous system disorders.

Activity-induced weakness was reported in multifocal motor neuropathy (MMN) and chronic inflammatory demyelinating polyneuropathy (CIDP). This was attributed to activity-dependent conduction block (CB) arising in demyelinated axons. It is not known if activity-induced weakness is common, nor if it is specific for MMN and CIDP. We, therefore, carried out an investigation by questionnaire in 64 MMN patients, 52 CIDP patients, 48 progressive spinal muscular atrophy (PSMA) patients, and 30 normal subjects. Subjects were asked if they experienced an increase in weakness when performing 10 common ta

[Recurrent Guillain-Barré syndrome after surgery].

INTRODUCTION: Recurrent Guillain-Barré syndrome is a very rare entity. Few cases have been reported. CASE REPORT: We report two patients who have developed a recurrent Guillain-Barré syndrome where one or even two episodes occurred after the same type of surgery, a hip prosthesis replacement. DISCUSSION: Occurrence of Guillain-Barré syndrome after surgery has mainly been reported in relation to epidural anesthesia and less frequently general anesthesia. Although underlying pathogenic mechanisms remain unknown, a molecular mimicry phenomenon has been proposed.

[Immune-mediated neuropathies].

Such neuropathies as Guillain-Barré syndrome (GBS), chronic inflammatory demyelinating polyneuropathy (CIDP), and IgM paraproteinemic neuropathy are caused by autoimmune mechanisms. IgM paraproteinemic neuropathies are known to be intractable. Rituximab has recently been reported to be effective for IgM paraproteinemic neuropathy with anti-MAG IgM M-protein in a placebo-controlled trial. The effect should be confirmed with a larger trial. The use of this drug also may be tried for other type of IgM paraproteinemic neuropathy and for intractable CIDP in future. Antiganglioside IgG antibodies ar

Utility of somatosensory evoked potentials in chronic acquired demyelinating neuropathy.

Chronic acquired demyelinating polyneuropathy (CADP) is a heterogeneous syndrome that may be classified into a number of subtypes. Somatosensory evoked potentials (SSEPs) assess proximal segments of sensory nerves, inadequately assessed by routine nerve conduction studies (NCSs). The aim of the present study was to determine the utility of SSEPs in diagnosing and classifying different CADP subtypes. Forty-seven patients with CADP were studied and classified in five groups based on conventional NCSs and SSEPs. Some patients in Group 1 were initially misdiagnosed as having either motor neuron di

[Immunoglobulins and physiopathology: actual indications].

Intravenous Immunoglobulin (IVIg) are therapeutic preparations of normal human IgG that are obtained from pools of plasma from healthy blood donors. IVIg can be used at high dose as an immunomodulatory agent in a large number of autoimmune and/or systemic inflammatory diseases, particularly in hematologic or neurologic diseases. Mechanisms of action of IVIg are multiple and intricate. The development of new therapeutic trials in association with analyses of mechanisms of action should help to define new indications of IVIg therapy.

Long-term evolution of anti-ganglioside antibody levels in patient with chronic dysimmune neuropathy under IVIg therapy.

The authors retrospectively examined the anti-ganglioside antibody (AGA) IgM level changes from 14 patients with chronic dysimmune neuropathy (5 with multifocal motor neuropathy and 9 with chronic inflammatory demyelinating polyneuropathy) treated with maintenance doses of intravenous immunoglobulins (IVIg). The median follow-up was 5 years. At last follow-up, 93% of the patients had an increment of AGA levels, and five patients with initial AGA values within normal range became positive during follow-up. Overall, median AGA titers significantly increased from the first to the last samples, de

Multifocal motor neuropathy.

Multifocal motor neuropathy (MMN) is a recently identified peripheral nerve disorder characterized by progressive, predominantly distal, asymmetric limb weakness mostly affecting upper limbs, minimal or no sensory impairment, and by the presence on nerve conduction studies of multifocal persistent partial conduction blocks on motor but not sensory nerves. The etiopathogenesis of MMN is not known, but there is some evidence, based mostly on the clinical improvement after immunological therapies, that the disease has an immunological basis. Antibodies, mostly IgM, to the gangliosides GM1, and th

Multifocal inflammatory demyelinating neuropathy: a distinct clinical entity?

BACKGROUND: Several patients have been reported with an asymmetric sensory or sensorimotor demyelinating neuropathy not fulfilling the diagnostic criteria for chronic inflammatory demyelinating polyneuropathy or multifocal motor neuropathy. OBJECTIVE: To present the clinical, electrophysiologic, radiologic, and pathologic features of six patients with an asymmetric sensory or sensorimotor demyelinating neuropathy. RESULTS: All six patients were initially affected in only one limb; in four patients the neuropathy progressed to other limbs in an asymmetric fashion during several years. On elec

[Paraneoplastic syndromes with peripheral nerve involvement].

OBJECTIVE: To review the paraneoplastic complications described affecting the peripheral nervous system. DEVELOPMENT: Different types of tumors, especially secretory carcinomas and some lymphomas may affect the peripheral nerves, and this may often precede clinical signs of the tumor. Depending on the clinical features we may consider sensory neuropathies, motor neuropathies, senso-motor neuropathies, autonomic neuropathies, mononeuritis, and neuromyotonia. They may present in an isolated form or as part of a more generalized involvement of the nervous system. CONCLUSIONS: The presence of ce

Autoantibodies to GM1b and GalNAc-GD1a: relationship to Campylobacter jejuni infection and acute motor axonal neuropathy in China.

IgG antibodies to the minor gangliosides GM1b and GalNAc-GD1a frequently are present in sera of Japanese patients with Guillain-Barré syndrome. The relationship between these autoantibodies and Campylobacter jejuni infection, the type of disease (acute motor axonal neuropathy [AMAN], or acute inflammatory demyelinating polyneuropathy [AIDP]) has yet to be established. Sera samples were obtained from 55 Chinese patients with clinically defined Guillain-Barré syndrome. An electrophysiology study showed nine AIDP, 28 had AMAN, and 18 unclassified. C. jejuni serology was positively correlated with

Schmidt-Lanterman's incisures--the principal target of autoimmune attack in demyelinating Guillain-Barré syndrome?

We used immunocytochemical staining of peripheral (trigeminal) nerve to screen sera of patients with Guillain-Barré syndrome (GBS) for the presence of autoantibodies, using sera from patients with other neurological diseases and healthy volunteers as controls. Most sera mildly reacted with axons, myelin sheaths, or sensory neurons without correlation to a specific disease. A characteristic staining, however, was found in 23 demyelinating cases (89%) out of 26 investigated GBS sera. With these sera, dark, oval and often paired small blobs were observed throughout the sections. A similar picture

Ophthalmoplegic and lower cranial nerve variants merge into each other and into classical Guillain-Barré syndrome.

We delineated the place of cranial nerve variants within the concept of clinically defined Guillain-Barre syndrome (GBS). In the ophthalmoplegic variant (n = 7) the oculomotor nerves were early involved. In a lower cranial nerve variant (n = 9) the cranial nerves IX, X, and XI were early involved. During progression considerable overlap occurred between these two variants, but also with the classically ascending variant of clinically defined GBS. These findings indicate common immune mechanisms in all GBS variants.

Anti-GD1a ganglioside antibodies in peripheral motor syndromes.

High titers of anti-GD1a antibodies have been found in patients with Guillain-Barre syndrome or motor neuropathy. To determine the possible diagnostic relevance of these antibodies, we measured serum anti-GD1a IgG and IgM antibodies by enzyme-linked immunosorbent assay in 195 patients with different motor syndromes and in 335 control subjects. Moderately high antibody titers (1/1,280-1/5,120) were occasionally found in patients with chronic inflammatory demyelinating polyneuropathy (5%), multifocal motor neuropathy (18%), lower motor neuron disease (3.8%), or amyotrophic lateral sclerosis (1.8

Respiratory insufficiency in neuronopathic and neuropathic disorders.

Twenty-nine patients with a neuronopathic or neuropathic disorder were referred for assessment of respiratory insufficiency between 1978 and 1994. Diagnoses included spinal muscular atrophy (6), chronic idiopathic demyelinating neuropathy (4), Vialetto-van Laere syndrome (3), hereditary motor and sensory neuropathy (3) and a miscellaneous group (5). We also describe seven patients with Guillain-Barré syndrome (GBS) who required long-term ventilatory support for over 6 months to 7 years after the initial illness. Respiratory insufficiency occurred as a consequence of respiratory muscle weakness

Acute childhood ataxia: 10-year experience.

Forty cases of acute childhood ataxia were retrospectively assessed for main etiologies and for factors that can be used in planning the most effective and cost-efficient management. The most common discharge diagnoses were acute cerebellar ataxia, ingestion, and Guillain-Barré syndrome, encompassing 80% of all cases. The remaining 20% included various isolated causes. Acute cerebellar ataxia was primarily seen in children less than 6 years of age who had preceding viral syndromes or varicella. Ingestions were also most frequent in children less than 6 years of age, but a second peak occurred

Inflammatory sensory polyradiculopathy and reactivated peripheral nervous system infection in a genital herpes model.

To study effects of herpesvirus reactivation on the nervous system, mice with latent genital herpes simplex virus type 2 (HSV-2) infections were immunosuppressed. Reactivated infection was detected by virus isolation or by antigen screening in histological sections of spines that contained cords, roots, and dorsal root ganglia. In ganglia, viral antigen was restricted to 2 distinct groups at T9-L2, and L6-S2 levels. In some ganglia, antigen was found in up to 4% of non-contiguous neurons, in their axons, and in endoneurial and satellite cells. Nerve roots distal to ganglia contained a few anti

Electrophysiologic evaluation of phrenic nerves in severe respiratory insufficiency requiring mechanical ventilation.

Diaphragmatic paralysis in patients with respiratory insufficiency compounds the problems in the management. In the presence of lower lobe atelectasis, pleural effusion, or a patient's poor respiratory effort, fluoroscopic examination is often not a reliable way to diagnose diaphragmatic paralysis. We observed that transcutaneous phrenic nerve stimulation in the neck and recording the diaphragmatic potentials from electrodes placed on the lower part of the chest is a simple, reliable, and noninvasive technique to diagnose diaphragmatic dysfunction at the bedside in critically ill patients. Spinal radiculoneuropathy in aging rats: demyelination secondary to neuronal dwindling?

Temporal development of radicular demyelination was studied in male albino rats examined sequentially throughout the lifespan of the animals. The rats were perfusion-fixed with paraformaldehyde and glutaraldehyde and areas of their nervous system including the lumbar spinal roots, the spinal cord, and the peripheral sciatic nerve, were embedded in epoxy resin and submitted to microscopic examination in semithin and ultrathin sections. In addition, a vital fat stain, teasing of single nerve fibers, and estimates of axon diameter and fiber number were obtained. Degenerative changes occurred earl
