[A case report of bilateral cerebral peduncular infarction manifesting tetraparesis and pseudobalbur palsy].

A 63-year-old female had presented with right hemiparesis and slight dysarthria. MRI had showed the infarction of left pons and left peduncle. Three months later she further presented with left hemiparesis, severe dysarthria and swallowing disturbance. MRI showed bilateral cerebral peduncular infarction. And the angiogram showed the occlusion of basilar artery at the just distal portion of the superior cerebellar artery. We recognized our case as the infarction due to the basilar artery occlusion. The 16 cases of bilateral cerebral peduncular infarction were reported. In these reports, the sym…

[Functional structure and cerebrovascular circulation of the vestibulo-ocular formations of the brain stem of the human cerebrum].

This work is dedicated to the study of anatomo-physiological peculiarities of the construction and cerebrovascular circulation of the vestibulo-ocular formations of the brain stem of the human cerebrum. The analysis of anatomo-physiological data enable to convince that the arches of the vertical and horizontal vestibulo-ocular reflexes include many structures of the brain stem of the human cerebrum, which are located along its longitude from the superior colliculi to caudal sections of brain stem. There is an existence of separate mechanisms of horizontal and vertical vestibulo-ocular interact

A 15-year-old boy with central nervous system vasculopathy presenting with dysarthria-clumsy hand syndrome.

Dysarthria-clumsy hand stroke syndrome has been described frequently in adults but not in children. We report a 15-year-old right-handed boy with sudden onset of dysarthria, dysphagia, right facial weakness, and mild right-hand clumsiness. Computed tomographic scan and magnetic resonance imaging demonstrated infarction in the genu and posterior limb of the left internal capsule. Magnetic resonance angiography and conventional angiography demonstrated stenosis of the supraclinoid portion of the left internal carotid artery and the origin of the left ophthalmic artery. Lacunar infarction in an o

[Visual hallucinosis and hyperhedonism in pontine and thalamic infarction].

The syndrome of peduncular hallucinosis in characterized by transient visual hallucinations which often consist in animated and mobile chara … thalamus. No lesion was seen in the mesencephalon with MRI. This case confirms that the lesions responsible for peduncular hallucinosis are not restricted to the mesencephalon, which suggests that several interconnected neural structures are probably involved in the genesis of this type of visual hallucinations. During the days following the stroke, o …

[Non-paralytic pontine exotropia with alternating exotropia].

We report a patient with pontine infarction who showed non-paralytic pontine exotropia (NPPE) with alternating exotropia. An 81-year old wom … m with involvement of the medial longitudinum fasciculus (MLF) and in the ventral site of the upper pons. On the 16th day, right exotropia disappeared on eye opening without forward gaze, but forward gaze easily induced outward deviation of the right eye. On the 39th day, right exotropia on forward gaze had almost disappeared. However, left MLF syndrom …

[A case of hemorrhagic mixed cerebrovascular malformation of the brainstem draining through a transpontine vein].

A case of mixed cerebrovascular malformation of the brainstem with pontine hemorrhage is reported. The mixed cerebrovascular malformation was composed of medullary venous malformation, one hemorrhagic and another non-hemorrhagic mass. The latter masses were thought to be cavernous venous malformations by their MR findings. The medullary venous malformation drained to the anterior pontomesencephalic vein through a transpontine vein. A 70-year-old man, complaining of aggravation of left hemiparesis, was admitted to our department. His past history included traumatic cervical myelopathy and diabe

[Unilateral paralysis of saccades caused by pontine tuberculoma].

A unilateral lesion of the paramedian pontine reticular formation results in an ipsilateral laterality paralysis with abolition of all saccadic movements directed to the side of the lesion. It is generally accepted that the oculocephalic reflex alone enables the eyes to be deviated beyond the median line of the affected side. In the present case of a paramedian pontine tuberculoma, a paralysis of laterality in accordance with the above mentioned opinion was observed initially. Treatment led to recovery of an ipsilateral pursuit of normal amplitude, contrasting with the persistent abolition of

Central bilateral sixth nerve palsy associated with a unilateral preganglionic Horner's syndrome.

Unilateral peripheral sixth nerve palsy associated with an ipsilateral postganglionic Horner's syndrome has previously been described. Isolated bilateral sixth nerve palsy due to a pontine hemorrhagic lesion, however, is a rare occurrence. We describe such a case whose only other neurological finding was a unilateral preganglionic Horner's syndrome.

Behavioral manifestations of central pontine myelinolysis.

A young woman with a clinical history and magnetic resonance imaging scan consistent with central pontine myelinolysis came to medical attention because of prominent behavioral symptoms. Marked clinical recovery occurred despite persistent radiologic abnormalities. Rapid correction of hyponatremia was probably related to the development of the central pontine myelinolysis. A normal computed tomographic scan and the absence of brain-stem signs delayed accurate diagnosis.

Distribution in areas 18 and 19 of neurons projecting to the pontine nuclei: a quantitative study in the cat with retrograde transport of HRP-WGA.

Following large injections of horseradish peroxidase - wheat germ agglutinin in the pontine nuclei, corticopontine neurons in areas 18 and 19 were quantitatively mapped and flat maps showing the distribution of retrogradely labeled cells were constructed. The areal borders were defined either cyto- and myeloarchitectonically or from standard retinotopic maps presented in frontal sections (Tusa et al. 1981). Maps of the retinotopic organization in areas 18 and 19 (Tusa et al. 1979) were transferred to the present flat maps. Thus, the number and distribution of pontine projecting cells could be
