Kinesio taping in stroke: improving functional use of the upper extremity in hemiplegia.

The purpose of this article is to present the Kinesio taping method used to improve the upper extremity function in the adult with hemiplegia. The article discusses various therapeutic methods used in the treatment of stroke patients to achieve a functional upper extremity. The only taping technique for various upper extremity conditions that has been described in the literature is the athletic taping technique. In this article, some interpretation is offered on proper assessment of the nonfunctional upper extremity, including the emphasis on postural alignment, trunk control, and scapula alig

Romberg and his sign.

Moritz Heinrich Romberg (1795-1873) acquired much of the wisdom and attitudes prevailing in English medicine when in 1820 he translated into German, Andrew Marshall's (1742-1813) The Morbid Anatomy of the Brain and Charles Bell's The Nervous System of the Human Body. He revolutionised European neurology, publishing his Lehrbuch der Nervenkrankheiten des Menschen: the first systematic textbook in neurology. Romberg's contribution to neurology, and his establishing tabes dorsalis as a distinctive disease were of crucial importance. Romberg's sign, once synonymous with tabes dorsalis, became reco

Prosthetic treatment of oligodontia with a tooth-supported overdenture--a case report.

An unusual case of oligodontia is reported in a 20 year old Nigerian male. It is unusual because of the isolated occurrence of the oligodontia which is unassociated with a family history or any other clinical features suggestive of specific syndromes or severe systemic abnormality. The patient presented with conically or peg shaped anterior and malformed posterior teeth in both the maxilla and mandible. Full mouth periapical radiographs revealed stunted roots, widening of the periodontal space and an unerupted tooth in the right mandibular premolar region. Tooth-supported upper and lower remov

Role of vision on early motor development: lessons from the blind.

For a better understanding of the contribution vision makes to the development of other sensory systems and to movement and posture, we studied effects of early blindness by examining video recordings of 14 totally blind infants. Infants were born at term or preterm and showed no evidence of brain damage. During preterm and term periods no noticeable changes in motor activity were observed. Around 2 months postterm all infants showed clear delay in head control and abnormal, exaggerated type of 'fidgety movements'. Later, postural control was characterized by a prolonged period of ataxic featu

[Right parietal lesions, spatial neglect and egocentric reference].

Using a proprioceptive "straight-ahead" pointing task, we determined the position of the subjective sagittal middle in thirty unselected patients with unilateral vascular lesions in the right hemisphere and twenty-two normal controls. Patients with extensive right parietal damage (n = 16) showed an ipsilesional (rightward) deviation of their egocentric reference, whereas patients with lesions that substantially spared the right parietal lobe (n = 14) showed a contralesional (leftward) deviation. No significant correlation emerged between the position of the egocentric reference and the perform

[The anamnesis in old people with equilibrium disorder].

Equilibratory troubles in aged individuals are the consequence of changes in vestibular, visual, proprioceptive, neuro-muscular and skeletal systems. Often related as well, to iatrogenic effects induced by pharmacological treatments. Its assessment is not easy at all because elderly population undergo physiological inability and so offer great difficulties in both diagnosis and management. At least informative data related to the equilibration must be collected: feelings, positional and postural unleashing factors, evolutive pattern, intercrises symptoms, cochlear and neurologic findings. The perception of head rotation in Parkinson's disease.

The accuracy in detecting angular displacements of the head and the neck was investigated in 14 patients with idiopathic Parkinson's disease (PD) and 16 age matched normal controls by the technique of vestibular and cervical "remembered" saccades. It was found that although the remembered saccades in PD patients were multiple-step and showed low initial saccadic gain, the final eye position of the eyes matched the rotational stimulus as accurately as in normals. This indicates that perception of head/neck rotation is normal in PD which is inconsistent with views that vestibulo/proprioceptive d

A case of congenital non-progressive sensory neuropathy with tonic pupils.

One case of a non-progressive congenital neuropathy is reported. Clinical findings included subtotal analgesia, and diminished temperature, vibration and proprioceptive sense in arms and legs. The sensory nerve action potentials were absent. Autonomic dysfunctions were restricted to tonic pupils. Sural nerve biopsy taken at the age of 8 1/2 showed fascicular hypoplasia, subtotal loss of myelinated nerve fibers and severe loss of unmyelinated nerve fibers with a pathological size distribution. In the skin biopsy, including several cutaneous nerves, no myelinated nerve fibers were present, the u

Motor control in humans with large-fiber sensory neuropathy.

Upper limb motor control was evaluated in a series of patients with a large-fiber sensory neuropathy associated with impaired position, vibration and cutaneous sensation and absence of deep tendon reflexes. Muscular strength was normal or only minimally affected. In studies of wrist movement it was found that both postural maintenance and accuracy of wrist displacement were heavily dependent on visual guidance. Without vision the limb would drift in almost random directions, and during intended movements both the trajectory and movement end-point were abnormal. The defects in posture and volun

EMG biofeedback in stroke: a 1-year follow-up on the effect of patient characteristics.

Outcomes from emg biofeedback training to 28 upper and 26 lower extremities among 34 patients who were hemiplegic were reassessed at varying intervals and at 1-year after completion of feedback sessions. Examinations took into account both neuromuscular measures of muscle activity and total limb function. There was virtually no deterioration in performance among the 19 upper and 20 lower extremities in which the outcome had previously been graded as "success" or "moderate." Therefore, age, sex, affected side, duration of injury or previous rehabilitation, or the presence of proprioceptive invo

Segmental neurophysiological mechanisms in scoliosis.

Segmental spinal reflexes (stretch reflexes) were studied in patients with scoliosis. The proprioceptive responses to the phasic stretch of the paraspinal muscles were asymmetric in all patients, and were increased on the convex side. The asymmetry was more pronounced when the patients were standing. The observed asymmetry of the reflex responses was taken to indicate asymmetry in the tone and postural activity of the superficial layer of the paraspinal muscles. A reciprocal relationship was found in the segmental reflex organisation between the superficial and deep layers of the paraspinal mu

Narcolepsy with tinnitus aura: interpretation.

A fifty-five year old man was admitted to our ward for evaluation of narcoleptic-cataplectic attacks which were signaled by a "hissing sound like high pressure steam leaking out of a pipe." This tinnitus was exclusively localized to the right ear. It started about sixty-to-ninety seconds before the cataplectic episode (affecting mostly the jaw muscles) and lasted through part of it as it became progressively louder toward the end and ceased abruptly. It is postulated that, along with the collapse of tonic musculature of the mandible, a decrease of proprioceptive tone also affected the tensor t

A Propeller with a Flexible Twist: A Computational Analysis of Intrinsically Disordered Regions in PIEZO Gating and PIEZO-Associated Channelopathies.

BACKGROUND: Mechanosensitive ion channels PIEZO1 and PIEZO2 are key mediators of mechanotransduction, which converts physical forces into cellular signals involved in proprioception, touch, vascular function, and other physiological processes. Mutations in human PIEZO proteins are linked to various diseases, such as hereditary xerocytosis, lymphatic dysplasia, and proprioceptive dysfunction. However, the role of intrinsic disorder in the regulation of these proteins and their susceptibility for disease-associated mutations remains unclear. METHODS: We analyzed canonical human PIEZO1 and PIEZO…

Skin and not dorsal root stimulation reduces hypertonus in thoracic motor complete spinal cord injury: a single case report.

On demand and localized treatment for excessive muscle tone after spinal cord injury (SCI) is currently not available. Here, we examine the reduction in leg hypertonus in a person with mid-thoracic, motor complete SCI using a commercial transcutaneous electrical stimulator (TES) applied at 50 or 150 Hz to the lower back and the possible mechanisms producing this bilateral reduction in leg tone. Hypertonus of knee extensors without and during TES, with both cathode (T11-L2) and anode (L3-L5) placed over the spinal column (midline, MID) or 10 cm to the left of midline (lateral, LAT) to only acti

Integration of vestibular and hindlimb inputs by vestibular nucleus neurons: multisensory influences on postural control.

We recently demonstrated in decerebrate and conscious cat preparations that hindlimb somatosensory inputs converge with vestibular afferent input onto neurons in multiple central nervous system (CNS) locations that participate in balance control. Although it is known that head position and limb state modulate postural reflexes, presumably through vestibulospinal and reticulospinal pathways, the combined influence of the two inputs on the activity of neurons in these brainstem regions is unknown. In the present study, we evaluated the responses of vestibular nucleus (VN) neurons to vestibular a

Firing properties of muscle spindles supplying the intrinsic foot muscles of humans in unloaded and freestanding conditions.

We recently developed an approach for recording from muscle spindles in the intrinsic muscles of the foot in freestanding humans by inserting a tungsten microelectrode into the posterior tibial nerve behind the medial malleolus of the ankle. Here we characterize the behavior of muscle spindles in the small muscles of the foot in 1) seated subjects with the leg horizontal and the foot naturally plantarflexed and 2) standing subjects. In the first study, recordings were made from 26 muscle spindle afferents located within flexor digiti minimi brevis ( n = 4), abductor digiti minimi ( n = 3), qua

The implementation effectiveness of the 'Strengthen your ankle' smartphone application for the prevention of ankle sprains: design of a randomized controlled trial.

BACKGROUND: Ankle sprains continue to pose a significant burden to the individual athlete, as well as to society as a whole. However, despite ankle sprains being the single most common sports injury and despite an active approach by various Dutch organisations in implementing preventive measures, large-scale community uptake of these preventive measures, and thus actual prevention of ankle sprains, is lagging well behind. In an attempt to bridge this implementation gap, the Dutch Consumer Safety Institute VeiligheidNL developed a freely available interactive App ('Strenghten your ankle' transl

Therapeutic advantage in selective ligament augmentation for partial tears of the anterior cruciate ligament: results in an animal model.

BACKGROUND: As a result of recent studies describing the double-bundle anterior cruciate ligament (ACL), selected ACL augmentation procedures, either anteromedial (AM) or posterolateral (PL), have been introduced as the treatment of choice for partial ACL ruptures. The preserved mechanoreceptor and vascularity of the remnant ACL are considered to provide additional biological benefits. Although enhanced knee joint proprioception in ACL augmented patients has been previously reported, there is no study assessing biological healing advantages of the graft after the ACL augmentation procedure.
